Developmental appearance and disappearance of cortical events and oscillations in infant rats.

Until recently, organized and state-dependent neocortical activity in infant rats was thought to commence with the emergence of delta waves at postnatal day (P)11. This view is changing with the discovery of several forms of cortical activity that are detectable soon after birth, including spindle bursts (SBs) and slow activity transients (SATs). Here we provide further evidence of surprisingly rich cortical activity patterns during early development and document, in P5-P13 rats, the appearance, disappearance, and transient expression of three cortical events and oscillations. EEG activity in

Vagus nerve stimulation for refractory status epilepticus: A systematic review.

RATIONALE: Refractory status epilepticus (RSE) is associated with high morbidity and mortality; however, evidence supporting standardized, effective medical treatments remains limited, and the role of vagus nerve stimulation (VNS) remains unclear. This systematic review aims to synthesize updated evidence on VNS therapy for RSE and evaluate its efficacy and functional outcomes. METHODS: This review included VNS outcome studies for any acute-to-subacute RSE (≤ 90 days from onset), excluding transcutaneous VNS, epilepsia partialis continua, and electrical status epilepticus in slow-wave sleep.

Transcutaneous auricular vagus nerve stimulation targets the brain-gut axis to treat functional dyspepsia: a scoping review of clinical evidence and mechanistic research.

BACKGROUND: Functional dyspepsia (FD) is a common gut-brain interaction disorder with suboptimal response to pharmacotherapy. Transcutaneous auricular vagus nerve stimulation (taVNS), a non-invasive neuromodulation targeting the vagus nerve, has emerged as a promising brain-gut axis therapy. This scoping review (PRISMA-ScR, JBI methodology) synthesizes clinical and preclinical mechanistic evidence on taVNS for FD. METHODS: PubMed, Embase, Web of Science, and Cochrane Library were searched up to December 20, 2025, following PRISMA-ScR. Using PICOS (adults with FD; taVNS; sham/non-vagal/active

[Expiratory pressure reduction (C-Flex Method) versus fix CPAP in the therapy for obstructive sleep apnoea].

INTRODUCTION: The REM star C-Flex (Fa. Respironics) was introduced in 2003. In contrast to the conventionel fix CPAP mode, the C-Flex mode is characterised by a pressure reduction at the beginning of expiration. In a randomised cross-over design, we investigated if this C-Flex-mode has advantages compared to the fix CPAP mode in terms of treatment quality and patient satisfaction. METHODS: In this prospective randomised single-blinded cross-over study we investigated 20 patients with obstructive sleep apnoea treated with fix CPAP versus treatment with the C-Flex mode (level 2) for 6 weeks, re

Pharmacotherapy of major depressive disorder in older adults: from an evidence-informed stepwise algorithm to precision medicine.

The pharmacotherapy of late-life depression (LLD) is characterized by clinical complexity. Older adults are living longer with multimorbidity, frailty and polypharmacy, and are taking more CNS-active medications and substances. These add to the risk and complexity of adding or changing depression treatment. At the same time, there are more medication options for depression and the use of many of them entails balancing potentials risks and benefits. As a result, the pharmacotherapy of LLD should be guided by tools such as algorithms or decision trees to maximize effectiveness and minimize risks

Beyond a Single Etiology: Tachy-Brady Syndrome and Possible Coexisting Focal Epilepsy Presenting as Transient Loss of Consciousness.

Transient loss of consciousness has overlapping cardiac and neurologic causes, and distinguishing syncope from seizure can be difficult. The co-occurrence of a primary cardiac rhythm disorder with a possible coexisting epileptic process is infrequently reported, as most cases instead describe one condition mimicking or triggering the other. We present a case of a 77-year-old woman with hypertension, hyperlipidemia, and paroxysmal atrial fibrillation who awoke with an abnormal chest sensation, a dream-like experience, and urinary incontinence and was admitted for evaluation of a possible seizur

Non-pharmacological interventions for sleep promotion in hospitalized children.

BACKGROUND: Healthy sleep is an important component of childhood development. Changes in sleep architecture, including sleep stage composition, quantity, and quality from infancy to adolescence are a reflection of neurologic maturation. Hospital admission for acute illness introduces modifiable risk factors for sleep disruption that may negatively affect active brain development during a period of illness and recovery. Thus, it is important to examine non-pharmacologic interventions for sleep promotion in the pediatric inpatient setting. OBJECTIVES: To evaluate the effect of non-pharmacologic

Normative high-frequency oscillation phase-amplitude coupling and effective connectivity under sevoflurane.

Resective surgery for pediatric drug-resistant focal epilepsy often requires extraoperative intracranial electroencephalography recording to accurately localize the epileptogenic zone. This procedure entails multiple neurosurgeries, intracranial electrode implantation and explantation, and days of invasive inpatient evaluation. There is a need for methods to reduce diagnostic burden and introduce objective epilepsy biomarkers. Our preliminary studies aimed to address these issues by using sevoflurane anesthesia to rapidly and reversibly activate intraoperative phase-amplitude coupling between

Gastric electrical stimulation: an evidence-based analysis.

OBJECTIVE: The objective of this analysis was to assess the effectiveness, safety and cost-effectiveness of gastric electrical stimulation (GES) for the treatment of chronic, symptomatic refractory gastroparesis and morbid obesity. BACKGROUND: GASTROPARESIS - EPIDEMIOLOGY: Gastroparesis (GP) broadly refers to impaired gastric emptying in the absence of obstruction. Clinically, this can range from the incidental detection of delayed gastric emptying in an asymptomatic person to patients with severe nausea, vomiting and malnutrition. Symptoms of GP are nonspecific and may mimic structural disor

Electrophysiological readout of ACTH-related neuroendocrine-immunomodulatory treatment response in infantile epileptic spasms syndrome: suppression of scalp ripple propagation networks.

BACKGROUND: Adrenocorticotropic hormone (ACTH) is a first-line neuroendocrine-immunomodulatory therapy for infantile epileptic spasms syndrome (IESS), but noninvasive electrophysiological markers that objectively reflect treatment-related network changes remain limited. We investigated whether scalp putative ripple propagation network metrics provide markers of adrenocorticotropic hormone-related network response and early relapse risk. METHODS: In this retrospective multicenter study, children receiving first-time ACTH therapy were included if paired pre- and post-treatment scalp electroence

Target-based biomarker selection - Mineralocorticoid receptor-related biomarkers and treatment outcome in major depression.

Aldosterone and mineralocorticoid receptor (MR)-function have been related to depression. We examined central and peripheral parameters of MR-function in order to characterize their relationship to clinical treatment outcome after six weeks in patients with acute depression. 30 patients with a diagnosis of major depression were examined 3 times over a 6 week period. Aldosterone and cortisol salvia samples were taken at 7.00 a.m. before patients got out of bed. Easy to use e-devices were used to measure markers of central MR function, i.e. slow wave sleep (SWS) and heart-rate variability (HRV).

Ketamine, magnesium and major depression--from pharmacology to pathophysiology and back.

UNLABELLED: The glutamatergic mechanism of antidepressant treatments is now in the center of research to overcome the limitations of monoamine-based approaches. There are several unresolved issues. For the action of the model compound, ketamine, NMDA-receptor block, AMPA-receptor activation and BDNF release appear to be involved in a mechanism, which leads to synaptic sprouting and strengthened synaptic connections. The link to the pathophysiology of depression is not clear. An overlooked connection is the role of magnesium, which acts as physiological NMDA-receptor antagonist: 1. There is ove

Electroclinical pattern in MECP2 duplication syndrome: eight new reported cases and review of literature.

PURPOSE: Duplications encompassing the MECP2 gene on the Xq28 region have been described in male patients with moderate to severe mental retardation, absent speech, neonatal hypotonia, progressive spasticity and/or ataxia, recurrent severe respiratory infections, gastrointestinal problems, mild facial dysmorphisms (midface hypoplasia, depressed nasal bridge, large ears) and epilepsy. Epilepsy can occur in >50% of cases, but the types of seizures and the electroclinical findings in affected male individuals have been poorly investigated up to the present. Herein we describe eight patients with

Cocaine action on peripheral, non-monoamine neural substrates as a trigger of electroencephalographic desynchronization and electromyographic activation following i.v. administration in freely moving rats.

Many important physiological, behavioral and subjective effects of i.v. cocaine (COC) are exceptionally rapid and transient, suggesting a possible involvement of peripheral neural substrates in their triggering. In the present study, we used high-speed electroencephalographic (EEG) and electromyographic (EMG) recordings (4-s resolution) in freely moving rats to characterize the central electrophysiological effects of i.v. COC at low doses within a self-administration range (0.25-1.0 mg/kg). We found that COC induces rapid, strong, and prolonged desynchronization of cortical EEG (decrease in al

Clinical observations of juvenile myoclonic epilepsy in 131 patients: a study in South India.

We studied clinical features of 131 patients with juvenile myoclonic epilepsy (JME). The prevalence was 7.7% among the epileptic patients registered. The mean age at onset was 13.37+/-4.93 years and the diagnosis was established at a mean age of 19.53+/-7.85 years. Absence seizures were reported by 27 (20.6%) patients, myoclonic jerks by 131 (100%) and generalized tonic-clonic seizures (GTCS) by 111 (84.7%). The triad of absence seizures, myoclonic jerks and GTCS was noted in 23 (17.5%) patients, 88 (67.2%) had myoclonic jerks and GTCS, 4 (3%) had absence seizures and myoclonic jerks and 16 (1

Rapid EEG desynchronization and EMG activation induced by intravenous cocaine in freely moving rats: a peripheral, nondopamine neural triggering.

Many important physiological, behavioral, and psychoemotional effects of intravenous (IV) cocaine (COC) are too fast and transient compared with pharmacokinetic predictions, suggesting a possible involvement of peripheral neural mechanisms in their triggering. In the present study, we examined changes in cortical electroencephalogram (EEG) and neck electromyogram (EMG) induced in freely moving rats by IV COC administration at low, reinforcing doses (0.25-1.0 mg/kg) and compared them with those induced by an auditory stimulus and IV COC methiodide, which cannot cross the blood-brain barrier. α5-containing nicotinic acetylcholine receptors regulate state transitions in cortical networks and modulate susceptibility to epileptiform discharges.

Spike-and-wave discharges (SWDs) of absence seizures and slowly oscillating Up-Down states of non-REM sleep and are both manifestations of large-scale synchronized cortical activity, yet the circuit mechanisms differentiating these phenomena remain poorly understood. Cholinergic modulation via nicotinic acetylcholine receptors (nAChRs) plays a key role in shaping cortical excitability. Here, we investigated the role of α5 subunit-containing nAChRs in regulating spontaneous cortical activity and the transition to paroxysmal states. We used acute brain slices from wild type (wt) and α5 knockout
