Pretemporal Transcavernous Approach to Cavernous Sinus Meningioma with Suprasellar Extension Synchronous with Sphenoid Wing Meningioma: Operative Technique: 2-Dimensional Operative Video.

Cavernous sinus meningiomas with suprasellar extension are challenging because of their proximity to the optic nerves, pituitary stalk, cranial nerves, and the cavernous/supraclinoid internal carotid artery (ICA). This complex anatomy necessitates precise microsurgical corridors for safe resection. The patient consented for the procedure and the publication of their images. Queen's University ethics board approved this publication. A 67-year-old woman who experienced a tonic-clonic seizure with left-sided weakness was found to have a right sphenoid wing meningioma and a synchronous meningioma

Intraoperative lumbar drainage can prevent cerebrospinal fluid leakage during transsphenoidal surgery for pituitary adenomas: a systematic review and meta-analysis.

BACKGROUND: Perioperative cerebrospinal fluid (CSF) leakage is a major complication of pituitary adenomas transsphenoidal surgery. Lumbar drainage (LD) is a common method of treating CSF leakage. But whether intraoperative LD can prevent CSF leakage during the perioperative period of pituitary adenomas transsphenoidal surgery remains controversial. Clarity on the appropriate use of LD is needed. METHODS: A systematic literature review was conducted in the PubMed, EMBASE, and Web of science databases. Articles were included when they compared intraoperative LD with intraoperative no-LD CSF lea

[Transcranial microsurgical decompression of the optic canal in surgical treatment of meningiomas of the sellar region].

RATIONALE: When removing the meningiomas of the sellar region, there is always a risk of visual impairment for various reasons, in particular, as a result of traction damage to the optic nerve. Decompression of the optic canal increases nerve mobility during tumor manipulation. In cases of meningioma growing into the canal, its decompression often seems necessary. AIM: Evaluation of the effectiveness and risks of performing decompression of the optic canal. MATERIALS AND METHODS: The study included patients with meningiomas of the parasellar location, who underwent surgical treatment at the

[Acute visual loss related to sphenoid meningioma].

Meningiomas represent about 20% of intracranial tumors. Involvement of the medial sphenoid wing includes anterior clinoid, cavernous sinus and superior orbital fissure meningiomas. Due to the proximity of these tumors to the optic nerve, typically progressive unilateral vision loss, over several months to years, is the classic clinical presentation. We report three cases of acute monocular vision loss, two transient and one permanent, ipsilateral to a sphenoid meningioma. Ophthalmological involvement with sphenoid meningiomas is most often chronic, due to interruption of axoplasmic flow and de

Computed Tomography Research: Relative Anatomy of Caldwell-Luc Approach in Pterygopalatine Fossa Surgery.

With the development of endoscopic technique, Caldwell-Luc approach has more wide applications and becomes a common method of pterygopalatine fossa (PPF) surgery. Few data can be used in this approach to avoid injuring the vessels and nerves within this area. In this study, the authors used computed tomography to get the coordinates of inferior orbital fissure, foramen rotundum, sphenopalatine foramen, internal opening of pterygoid canal, the strangulation of PPF, and the greater palatine foramen with canine fossa as the origin. Parameters of 60 patients (120 observations) between 35 and 55 ye

The conversational position in endoscopic pituitary surgery.

We describe a novel patient position for endoscopic transphenoidal surgery - the 'conversational position'. This position is a safe and effective alternative to the standard supine position, incorporating a semi-sitting position with the additional innovation of achieving a 'conversational position' by flexing the neck and turning the patient's head turned to face the surgeon. The 'conversational' position offers improvements in the surgical approach to sellar region, addressing specific intraoperative challenges such as maintaining a bloodless operative field, and enabling more intuitive and

A New Irrigation System (Endosplash) for a Rigid Endoscope in Trans-sphenoidal Endoscopic Surgery.

Obstruction of the visual field by blood is a major hindrance during endonasal endoscopic surgery, and a rapid and effective method for cleaning the lens is needed. We developed a new lens-cleaning system that does not employ a sheath or an irrigation-suction system. It is a 20-mm long cylinder with side holes that is attached to the barrel of the endoscope and is connected to a syringe containing saline. When the syringe is pressed, saline flows down to the tip along the barrel and washes the lens without requiring a sheath. We report the use of the system in six cases of endonasal endoscopic

Surgical anatomy of the cavernous sinus, superior orbital fissure, and orbital apex via a lateral orbitotomy approach: a cadaveric anatomical study.

BACKGROUND: Tumors of the middle fossa or cavernous sinus (CS), or intraorbital tumors, can penetrate each other through the superior orbital fissure (SOF) or neighboring tissue. These complicated pathologies are often treated with highly invasive surgical procedures. In this article, we demonstrate surgical anatomic dissections of the CS, SOF, orbital apex (OA), and dura mater extending to the periorbita from the middle fossa, by performing an epidural dissection via a lateral orbitotomy approach, and discuss findings that may provide guidance during surgery in these regions. METHODS: Latera

Composite Vascular Pedicled Middle Turbinate Flap for Reconstruction of Sellar Defects.

OBJECTIVES: Herein, we describe our experience in simple harvest of the vascular pedicled middle turbinate flap (MTF) sufficient for sellar defect reconstruction. METHODS: An anatomical feasibility study is done in 10 sides of 5 preserved injected cadaveric heads. The middle turbinate is separated from the skull base and the basal lamella with or without retrograde dissection of its tail as a composite flap based on the middle turbinate and posterolateral nasal arteries. The technique was applied in 25 cases of cerebrospinal fluid (CSF) leak after endoscopic transsphenoidal surgery. RESULTS:

[Utility of a bronchoscope and a gum elastic bougie for nasal intubation in a patient with a history of transsphenoidal surgery].

A 39-year-old man with a history of transsphenoidal surgery was scheduled for sagittal split ramus osteotomy. Nasal intubation was successfully performed using a bronchoscope (BF) and a gum elastic bougie (GB). inserted using a BE The BF was then replaced with a GB. We widened his nasal cavity using the nasal airway. Finally, intubation was performed with an endotracheal tube (7.0 mm) using a GB and a Macintosh laryngoscope. Thus, BF and GB could be safely used for nasal intubation in a patient with a history of transsphenoidal surgery.

Regional disparities and practice patterns in surgical approaches to pituitary tumors in the United States.

BACKGROUND: Traditional microscopic and endoscopic transsphenoidal approaches (TSAs) are the most common surgical techniques in pituitary surgery. Examining regional practice patterns in pituitary surgery can provide valuable insights into which surgical strategies are most accessible, effective, and cost-efficient. In this study we investigated regional variations in surgical approaches to pituitary tumors and evaluated evolving practice patterns in pituitary surgery. METHODS: The 2010 Medicare Part B Carrier Summary Database and Medicare Part B National Summary Database from 2003-2010 were

Accessory spine of the foramen ovale.

The objective of this study was to provide morphometric analysis of an accessory spine that was found within the lumen of the foramen ovale, as well as to find out whether this structure could mechanically irritate the mandibular nerve. A bifid spine was perceived in the macerated skull of an adult individual. It was located in the anterior part of the left foramen ovale. The overall length of the spine was found to be 1.8 mm. The spine had a homogenous structure, and showed high levels of mineralisation. We conclude that the accessory spine did not compress the mandibular nerve, and that the

Pterygoid implants for maxillofacial rehabilitation of a patient with a bilateral maxillectomy defect.

Bilateral maxillectomy is known to have serious esthetic and functional consequences. The retention and support of a maxillary obturator prosthesis in these patients is particularly challenging. Surgical placement of implants is also challenging because of the lack of available bone. Therefore, implant placement into remote sites such as zygoma has been advocated. Very few articles in the literature have discussed the use of pterygoid/pterygomaxillary implants in patients undergoing maxillectomy. This case report describes the maxillofacial rehabilitation of an elderly man who underwent a bila
