Chronic ankle instability has been defined as the development of recurrent ankle sprains and persistent symptoms after initial lateral ankle sprain. The diagnosis of ankle instability is usually established on the patient's history, physical examination, and radiographic assessment. Patients have signs of both functional and mechanical instability, and the repetitive, chronic nature of the injury may lead to intra-articular and periarticular pathologies. This article discusses the incidence, etiology, and magnetic resonance (MR) imaging of these pathologies, reviews the surgical treatment proc

Limping in a child is a frequent reason for consultation, sometimes as an emergency. A distinction should be made between a protective limp, to avoid painful weight bearing, and an equilibration limp, which corresponds to an adaptation to a disturbance of muscular activity with a neuromuscular or osteoarticular origin. Etiological analysis of a protective limp is essentially based on the history and examination. Further radiological or biochemical investigations contribute to the diagnostic process and are essential in limps of recent onset. Broadly speaking, in children aged between one and t

Emergency departments frequently encounter pathology resulting from injury to the foot and ankle, with approximately 6000 case per day in France. In an ankle sprain, 85% of the lesions involve the lateral collateral ligament. Many other, much rarer, types of lesion with different therapeutic consequences can present, however. Interpretation of the initial conventional radiographs is vital to establishing the type lesion and to proposing adapted and rapid treatment. The objective of this article is to review the various osteoarticular and ligament injuries encountered in the foot and the ankle.

BACKGROUND: Cryotherapy (the application of ice for therapeutic purposes) is one of the most common treatment modalities employed in the immediate management of acute soft tissue injury. Despite its widespread clinical use, the precise physiological responses to therapeutic cooling have not been fully elucidated, and effective evidence-based treatment protocols are yet to be established. Intermittent ice applications are thought to exert a significant analgesic effect. This could facilitate earlier therapeutic exercise after injury, potentially allowing for a quicker return to activity. The pr

The biomechanical anatomy of the ankle ligaments continues to be a subject of interest because detailed knowledge of these structures is essential for proper diagnosis and treatment of the injuries affecting them. Lesions to the ankle ligaments are one of the most common sports injuries and the origin of soft tissue impingement syndrome. Together with the ligaments of the tibiofibular syndesmosis, two large ligamentous complexes are the main static stabilizers of the ankle joint: the lateral collateral ligament and the medial collateral (or deltoid) ligament. This article provides an anatomic

An unloading reaction has been characterized as a modified flexor reflex (FR), in which the standing subjects decrease the load on the stimulated foot and increase the load on the contralateral side, but, without withdrawal of the stimulated foot. Different behavioral circumstances have been shown to modulate this reflex. It is not known whether unloading reactions can be modulated with a loaded supinated ankle position, which, in excess, may result in an ankle sprain injury. Since ankle sprain depends on the load applied to a supinated foot, our premise is that unloading reactions may protect

BACKGROUND: The optimal management for severe sprains (Grades II and III) of the lateral ligament complex of the ankle is unclear. The aims of this randomised controlled trial are to estimate (1) the clinical effectiveness of three methods of providing mechanical support to the ankle (below knee cast, Aircast brace and Bledsoe boot) in comparison to Tubigrip, and (2) to compare the cost of each strategy, including subsequent health care costs.
METHODS/DESIGN: Six hundred and fifty people with a diagnosis of severe sprain are being identified through emergency departments. The study has been d

Lateral column lengthening of the calcaneus has been a powerful tool used to correct peritalar subluxation in symptomatic flat feet. The mechanical basis and limits for correction with this procedure are not well understood. A flatfoot model was created on 8 fresh-frozen cadaver feet by sectioning the deltoid ligament, talonavicular capsule, and spring ligament. Strain-gauge analysis of the long plantar ligament was performed (on 6 specimens) as the lateral column was sequentially lengthened from 4 to 12 mm in 2-mm increments. Results showed that only the lateral most one-third of the long pla

BACKGROUND: Factors potentially causing chronic instability after ankle inversion sprains have rarely been examined during the injuring movement.
PURPOSE: To compare control of ankle movement during quiet stance and after inversion perturbation in chronically unstable ankles (n = 16) with healthy controls (n = 26).
METHODS: Movement control was measured as magnitude of lateral ankle oscillation, using 3SPACE Fastrak during single leg stance (baseline oscillation) in two foot positions, flat and demi-pointe. In both positions, time to resume baseline oscillation after inversion perturbation (

Peroneal tendon injuries should be considered in the differential diagnosis of lateral ankle pain and instability. The spectrum of injury to the peroneal tendons includes tenosynovitis, tendinitis, subluxation, dislocation and tears. The mechanism, presentation and treatment of isolated peroneal brevis and longus injuries has been described in the literature. This is a case study of a rare combined peroneus brevis and longus injury in a young healthy collegiate athlete.

Acute traumatic injuries are common in ballet dancers. A careful history, thorough examination, and appropriate imaging should allow for the diagnosis of most problems. The clinician must have a high index of suspicion for occult bony injuries, especially if the patient fails to recover as expected. Aggressive treatment of the sprained ankle is essential to maintain foot and ankle mobility and prevent prolonged disability and subsequent overuse injuries. Kinetic chain dysfunctions are common in ballet dancers with overuse injuries and commonly follow ankle sprains. They may represent a seconda

INTRODUCTION: The following article deals with the problem of peroneal clicking. This means a clicking of the two peroneal tendons against each other. It has to be differentiated from peroneal tendon dislocation or subluxation.
METHOD: In three patients, a peroneal click was found without symptoms of dislocation of the tendons over the fibula. In these cases the indication for resection of the peroneal brevis tendon was seen.
RESULTS: Postoperatively no pain, swelling, and peroneal click were present. The partial loss of function of the peroneal brevis tendon seems to be negligible.

The aim of this study was to investigate whether the number of patients with residual symptoms following ankle sprains could be reduced by training on a wobble board during a 12-week training recovery period. The effect of a 12-week training program was compared to no training. Forty-eight patients completed the study. In the follow-up period (mean 230 days) we found significantly fewer recurrent distorsions and fewer patients had functional instability in the study group compared to controls. There were no differences between the two groups concerning subjective complaints during the recovery

We observed 39 feet with a "too-long" anteromedial process of the calcaneus (TLAP) in 25 children and adolescents. The abnormality was diagnosed from symptoms (ankle sprains or persistent ankle or foot pain) in 33 cases. Six cases of TLAP were noted on radiographs taken of the patient's opposite, asymptomatic foot. Initial treatment by 3 weeks of strapping failed in seven of seven cases, and 3-6 weeks of primary plaster immobilization failed in 10 of 25 feet. Primary immobilization treatment failed in 17 of 33 symptomatic feet. Resection of the TLAP provided good results in 14 of 15 feet when

Functional ankle instability, orthoses, and passive resistive torque tolerated have not been researched. The purpose of the study was to evaluate the passive resistance torque exerted by a flexible and semirigid orthosis for individuals with chronic instability. Twenty-two subjects were evaluated on the passive ankle resistance unit during unbraced, flexible, and semirigid brace conditions. Data from the final three trials for each condition were analyzed using a multiple analysis of variance with repeated measures for resistive torque and inversion range of motion. The semirigid and flexible
