Specificity of autonomic influences on cardiac responses during myocardial ischemia.

A possible role of the autonomic nervous system in the left ventricular response to acute regional myocardial ischemia was sought in conscio … r changes in response to coronary occlusion. Results obtained in animals following surgical cardiac sympathectomy indicated reduced tachycardia and no effects on other parameters. The principal effect of parasympathetic blockade was to augment the increase in end diastolic diameter during occlusion Right atrial pacing indicated this change was due to higher ini …

Thoracoscopic delivery of therapeutics in the swine sympathetic chain: Implications for future neuromodulation.

The thoracic sympathetic chain is implicated in several disease processes including palmar hyperhidrosis and complex regional pain syndrome. These diseases are often medically refractory and require surgical treatments including sympathectomies and ganglion blocks. The use of chemogenetic or optogenetic technologies to modulate sympathetic chain activity may be a potential treatment for these diseases. However, there is no established thoracoscopic surgical approach to deliver viral vectors into the thoracic sympathetic chain and ganglia. Our objective was to evaluate the feasibility of thorac

Guidelines on surgery of the thoracic sympathetic nervous system.

Thoracic sympathetic nervous system (TSNS) surgery has increased in importance in the last few years, generating great expectations among the general population and the scientific community. This has been due to the excellent results obtained by videothoracoscopy-assisted thoracic sympathectomy in the treatment of essential hyperhidrosis and other TSNS disorders. This minimally invasive surgical technique has been shown to be effective, and with a low morbidity it is accepted as one of the best therapeutic options for the treatment of palmar and bilateral axillary hyperhidrosis and the number …

T3/T4 thoracic sympathictomy and compensatory sweating in treatment of palmar hyperhidrosis.

BACKGROUND: Compensatory sweating (CS) is one of the most common postoperative complications after thoracic sympathectomy, sympathicotomy or endoscopic sympathetic block (ESB) for palmar hyperhidrosis. This study was conducted to examine the relevance between CS and the sympathetic segment being transected in the surgical treatment of palmar hyperhidrosis, and thus to detect the potential mechanism of the occurrence of CS. METHODS: Between October 2004 and June 2006, 163 patients with primary hyperhidrosis were randomly divided into two groups, T(3) sympathicotomy (78 patients) and T(4) sympa…

[A successful cross-over femorotibial bypass for severe ischemic leg of thromboangiitis obliterans].

A case of thromboangiitis obliterans (TAO) successfully treated with a modified vascular reconstruction is reported. A 53 year-old man who had undergone lumber sympathectomy 9 years ago visited our hospital with complaints of rest pain and intractable ulcer at the right big toe. Preoperative angiography could not visualize distal arteries from the right common iliac artery and major amputation above knee was intended. Operative angiography, however, revealed collateral pathways from the posterior tibial artery to plantar arteries. Then, a long bypass from the left common femoral artery to the …

Tyramine-induced contractions in the rabbit ear and basilar arteries.

This study examined in vitro tyramine (TY)-induced contractions of the isolated ring segments of the rabbit basilar and ear arteries in relation to their sympathetic innervations. The TY dose--response relationship in the basilar artery was not affected by cocaine, chronic sympathectomy, or reserpine pretreatment. However, these treatments shifted the TY dose--response relationship in the ear arteries significantly to the right. These results indicate that the TY-induced contraction in the basilar artery is due solely to its direct stimulation of the postsynaptic alpha-receptors, but in the ea…

Ultrasound-Targeted Ablation of the Aortorenal Ganglion in Canine Model for Hypertension Treatment.

BACKGROUND: Precisely map and ablate the aortorenal ganglion (ARG) using noninvasive ultrasound technology to evaluate therapeutic effects and mechanisms in hypertension and related organ involvement. METHODS: A noninvasive dual-frequency ultrasound with a 3-dimensional targeting system was utilized to precisely map and ablate the ARG in a hypertensive canine model (2-kidney, 1-clip). Blood pressure changes from ultrasound stimulation served as a real-time evaluation metric, leading to an ultrasound stimulation-ablation-re-stimulation-supplementary ablation protocol for comprehensive ablation

[The long QT syndrome: a critical approach to diagnosis and management].

The long QT syndrome (LQTS) is a cardiac disease of genetic origin which, unless properly diagnosed and managed, carries a high risk for sudden cardiac death, especially among youngsters. At the same time, the available therapies are quite effective. Most lethal episodes occur as the sentinel event, stressing the fact that most of the time there is no room for error in diagnosis or management. Here, we endeavored to share the 50-year specific experience of our group, by critically discussing both the approach and management of LQTS. Following an analysis of the clinical impact of the genetic f

Preventing atrial fibrillation recurrence with combination of catheter ablation and renal denervation or ganglion plexus ablation: A systematic review and network meta-analysis.

BACKGROUND: Atrial fibrillation (AF), affects around 2 % of the global population and is projected to rise over the next 50 years. Catheter ablation (CA) is the primary treatment for symptomatic AF resistant to drug therapy. Despite its widespread use, CA has a failure rate of 20 %-50 %, often requiring repeat procedures, due to significant long-term recurrence rates. Combining CA with renal denervation (RDN) or ganglion plexus ablation (GPA) may effectively reduce the recurrence rates of AF. METHODS: Quality assessment was done using the Cochrane ROB 2.0 tool, network meta-analysis using RSt

Renal denervation: a key approach to hypertension and cardiovascular disease.

Sympathetic activation plays a critical role in the development of hypertension and cardiovascular disease, including heart failure and arrhythmias. Renal nerves contribute to the regulation of blood pressure and fluid volume through renal sympathetic efferent nerves, and to the modulation of sympathetic outflow through renal sensory afferent nerves. Previous studies including ours suggest that selective afferent renal denervation with preservation of efferent renal nerves can significantly decrease central sympathetic outflow in animal models of hypertension with renal damage. Renal Denervation in End-Stage Renal Disease: Current Evidence and Perspectives.

In patients with end-stage renal disease (ESRD) undergoing haemodialysis, hypertension is of common detection and frequently inadequately controlled. Multiple pathophysiological mechanisms are involved in the development and progression of the ESRD-related high blood pressure state, which has been implicated in the increased cardiovascular risk reported in this hypertensive clinical phenotype. Renal sympathetic efferent and afferent nerves play a relevant role in the development and progression of elevated blood pressure values in patients with ESRD, often leading to resistant hypertension. The current status of renal denervation for the treatment of arterial hypertension.

Despite the availability of safe and effective antihypertensive drugs, blood pressure (BP) control to guideline-recommended target values is poor. Several device-based therapies have been introduced to lower BP. The most extensively investigated approach is catheter-based renal sympathetic denervation (RDN), which aims to interrupt the activity of afferent and efferent renal sympathetic nerves by applying radiofrequency energy, ultrasound energy, or injection of alcohol in the perivascular space. The second generation of placebo-controlled trials have provided solid evidence for the BP-lowerin

Clinical outcomes of laparoscopic-based renal denervation plus adrenalectomy vs adrenalectomy alone for treating resistant hypertension caused by unilateral aldosterone-producing adenoma.

Previous studies describing renal denervation (RDN) from the intima of the renal artery for the treatment of resistant hypertension have reported variable efficacies, and RDN triggers renal intimal injury and atherosclerosis. This study aimed to evaluate the efficacy and safety of RDN from the adventitia of renal artery plus unilateral laparoscopic adrenalectomy to treat patients with resistant hypertension caused by unilateral aldosterone-producing adenoma (APA). A total of 60 consecutive patients with resistant hypertension caused by unilateral APA were enrolled in this study. Patients were
