BACKGROUND: Lower extremity soft tissue sarcoma treatment has evolved from primarily amputation procedures toward limb salvage. This series assesses whether soft tissue sarcoma tissue defects, extensive enough to require microsurgical reconstruction, can reliably result in preservation of ambulation, as well as objectively evaluate functional outcomes utilizing a patient-reported validated scale. It will also look at whether immediate functional muscle reconstructions and tendon transfers can be successful at restoring ambulation, potentially expanding the indications for limb salvage procedur…

Diabetic foot ulcers are a common and devastating complication of diabetes, often linked to equinus deformity, which raises plantar forefoot pressures and impairs healing. Conservative strategies like casting and orthoses are first-line but limited by high recurrence and compliance challenges. Surgical off-loading procedures-Achilles tendon lengthening, gastrocnemius recession, and lesser tendon recessions-target the root biomechanical cause and demonstrate superior durability in ulcer prevention. Evidence, including research by Laborde and others, supports their role in healing neuropathic an

The Bridle procedure consists of a posterior tibial tendon transfer through the interosseous membrane to the dorsum of the foot with a dual anastomosis to the tendon of the anterior tibial and a rerouted peroneus longus in front of the lateral malleolus. This procedure is performed to restore loss of dorsiflexion and to balance the foot, preventing a secondary varus or valgus deformity. The procedure was performed in 10 patients with 11 feet with paralysis of dorsiflexion of the foot. All are brace-free. The results are better in patients with peroneal nerve palsy secondary to trauma.

Twelve patients with congenital dislocation of the patella (CDP) and 23 patients with habitual dislocation of the patella (HDP) were followed for 2-15 years after surgical stabilization of the patella. The underlying pathology in both conditions was contracture of the quadriceps mechanism, which was more severe in CDP. Surgical stabilization in most cases included an extensive lateral release, medial plication, and transfer of the lateral half of the patella tendon. Lengthening of the rectus femoris tendon was required in many cases. With appropriate operative procedures, satisfactory results

The results of treatment of severe injuries to the proximal interphalangeal joint are unsatisfactory. The methods of joint reconstruction are discussed, including fusion, implant arthroplasty, perichondrial grafting and vascularized joint transfer. A patient is presented with a severe crush injury to the dorsum of the index finger with loss of skin and extensor tendon and proximal interphalangeal joint disruption. Immediate reconstruction of the finger is described using a composite free flap of skin, extensor tendon and proximal interphalangeal joint from the second toe. Follow-up at two year

Despite the long history of experience in peripheral nerve reconstruction, dating back to the 1870s, recent decades have seen an explosion of innovation. Advancements have been made in primary repair, secondary repair, nerve grafts, tendon transfers, and, most recently, nerve transfers. These innovations address the distance-to-target problem faced by surgeons and provide improved functional outcomes. Key principles for successful nerve transfers include proper patient selection, preoperative discussions, intraoperative decisions, and postoperative management. In this review, we discuss 3 comm…

OBJECT: In cases of irreparable injuries to the radial nerve or in cases in which nerves are repaired with little anticipation of restoration of function, tendon transfers are widely used. In this study, the authors searched for a more natural alternative for selectively restoring function, with the aid of a motor nerve transfer.
METHODS: Ten arms from five cadavers were used in the study. The posterior interosseous nerve and the median nerve together with their motor branches were exposed in the proximal forearm. The possibility of posterior interosseous nerve neurotization via the median ne…

The "spontaneous" subcutaneous rupture of the tendon of the extensor pollicis longus is reported and the etiology and the pathologie are discussed. In our opinion is the transfer of the extensor indicis the only operation to do in this case and gives the best results.

BACKGROUND: The routine use of augmentation in the surgical management of primary acute Achilles tendon rupture remains controversial. Biological, mechanical, and synthetic augmentation strategies have been adopted in clinical practice, yet their superiority over standard repair has not been established.
SOURCES OF DATA: A narrative review of peer-reviewed comparative studies, randomized controlled trials, systematic reviews, and meta-analyses retrieved from PubMed and MEDLINE, supplemented by the authors' clinical expertise.
AREAS OF AGREEMENT: Standard surgical repair of acute Achilles ten

BACKGROUND: Surgical management of chronic Tendo Achillis (TA) ruptures usually requires tendon grafting procedures. Several techniques have been described. We examined the outcome of three different less invasive (incisions length less than 3 cm) tendon transfer techniques in the management of patients with a chronic rupture of the TA.
METHODS: Of 62 patients (39 males and 23 females; mean age 44.8 years; range 29.3-62 years) with chronic TA ruptures managed operatively, 21 patients had a ≥6cm gap and underwent free ipsilateral semitendinosus (ST) graft (21 patients), whereas patients with s…

A 47 years-old man presented an atraumatic, spontaneous index flexor tendons rupture. This patient has been treated two years ago for a pulmonary tuberculosis. At surgery, the flexor tendons were infiltrated by a granuloma. Histologic examination was compatible with tuberculosis sequelae. According to literature review, this is the second reported case of a tendon rupture due to tuberculosis granuloma.

The extensor system of the hand is an intricate complex component that involves a working balance between the extrinsic and intrinsic muscles of extension. Injuries to the extensor tendons are typically repaired primarily or by tendon transfers or grafts at a second stage. A 16-year-old boy sustained a right hand crush injury following an all-terrain vehicle accident, resulting in the loss of dorsal skin and extensor tendons of digits 3, 4, and 5 involving extensor zones 4-6, and oblique fractures in the second and third metacarpal bones. Primary surgery involved fixation of aforementioned fra…

Tendons of the body differ dramatically in their function, mechanics and range of motion, but all connect to bone via an enthesis. Effective force transfer at the enthesis enables joint stability and mobility, with strength and stiffness arising from a fibrous architecture. However, how enthesis toughness arises across tendons with diverse loading orientations remains unclear. To study this, we performed simultaneous imaging of the bone and tendon in entheses that represent the range of tendon-to-bone insertions and extended a mathematical model to account for variations in insertion and bone

The presence and disposition of supernumerary tendons in the fourth extensor compartment of the wrist have been described by several authors. The aim of this communication was to describe a finding in a study of an anatomical dissection of an adult cadaver that presents a rare disposition of extensor tendons in the fourth extensor compartment of both wrists. In the right wrist, we found an extensor pollicis et indicis communis with three slips: two toward the index finger and one toward the thumb. We found no similar reports in a review of the literature. Additionally, we found an extensor ind

A 23 year old male kick boxer presented with a 24 hour history of pain and being unable to extend the interphalangeal joint of the left thumb. There was no history of trauma or any other risk factor for spontaneous rupture of the extensor pollicis longus tendon. On the previous day, he had been doing reverse press ups on the dorsum of his hands with his wrists hyperflexed as part of his training for kick boxing. At operation the extensor pollicis longus tendon was found to be divided at the level of the dorsal tubercle of the radius and was not directly repairable. The treatment was an extenso

The smooth gliding of the normal human digital flexor is maintained by synovial fluid lubrication and lubricants bound to the tendon surface. This system can be disrupted by degenerative conditions such as trigger finger, or by trauma. The resistance to tendon gliding after surgical repair of the lacerated digital flexor tendon relates to location of suture knots, exposure of suture materials, and type of surgical repair and materials. Restoration of a functioning gliding surface after injury can be helped by using low-friction, high-strength suture designs, therapy that enables gliding, and t

In Charcot-Marie-Tooth (CMT) cavovarus surgery, a regimented approach is critical to create a plantigrade foot, restore hindfoot stability, and generate active ankle dorsiflexion. The preoperative motor examination is fundamental to the algorithm, as it is not only guides the initial surgical planning but is key in the decision making that occurs throughout the operation. Surgeons need to be comfortable with multiple techniques to achieve each surgical goal. There is no one operation that works for all patients with CMT. A plantigrade foot is the most important of the surgical goals as hindfoo
