Root damage induced by intraosseous anesthesia. An in vitro investigation.

OBJECTIVES: The principle of the intraosseous anesthesia (IOA) relies on the perforation of the cortical plate of the bone for direct application of the local anesthetic solution into the underlying cancellous structures. During this procedure, IOA needles might accidentally come in contact with the tooth roots. The aim of the current in vitro study was to examine the consequences of this 'worst case scenario' comparing five commercially available IOA systems. MATERIAL AND METHOD: Extracted human roots were randomly perforated using five different IOA systems with a drilling time ≤5s. To simu

Effect of composite resins with and without fiber-reinforcement on the fracture resistance of teeth with non-carious cervical lesions.

OBJECTIVE: Non-carious cervical lesions (NCCLs) are commonly observed in clinical dentistry, leading to tooth fractures, sensitivity, and compromised pulp vitality. Therefore, their restoration is essential for both the aesthetic and structural integrity of teeth. This study aimed to compare the fracture resistance of NCCLs restored using different materials: an injectable universal composite, flowable bulk-fill composites with or without fiber-reinforcement. METHODS: Seventy-five double-rooted maxillary premolars were selected for the study. Fifteen teeth were left intact as a control. A wed…

The Unfavourable Split: a novel classification and an 11-year retrospective study looking at alternative methods for management of this well-known complication.

An unfavourable split is a well-known complication following a sagittal split osteotomy (SSO) of the mandible. Our aim was to analyse all unfavourable mandibular splits that had occurred when carrying out a SSO with the aim to design a classification which can facilitate management. We carried out a retrospective study analysing all orthognathic surgery from January 2010 until April 2021. Data surrounding unfavourable splits during this period were specifically analysed. Orthognathic surgery during this period was performed by a single OMFS unit with osteotomies performed by a single surgeon a

Concomitant Third Molar Removal During Sagittal Split Osteotomy of the Mandible.

It is described in textbook about management of third molar in orthognathic surgery that "ideally, the third molars should be removed 9 to 12 months before sagittal split osteotomy (SSO)." At the Chang Gung Craniofacial Center, the authors always remove mandibular third molars during SSO, because: removal of third molars at the same time of SSO could reduce psychological stress on patients by saving 1 surgical procedure under local anesthesia, better exposure of impacted third molars could be facilitated by sagittal split of buccal cortical plate, rigid fixation could be performed without diff

Management of intraoral injuries.

Primary closure of intraoral wounds can be done up to 24 hours after the injury. A mucosal seal decreases the possibility of infection and allows the wound to heal more rapidly and less painfully than a wound left open to heal by secondary intention. Defects of the lips must be closed with exact alignment of the vermilion-cutaneous margins. Nerve repairs must be done without tension at the repair site. Parotid duct repairs are mandatory to prevent cyst and fistula formation. Anterior permanent teeth may be replanted.

Comparative Evaluation of Fracture Resistance of Teeth Instrumented by Self adjusting File, Protaper NEXT, and Hyflex EDM: An In vitro Study.

INTRODUCTION: The primary aim of endodontic treatment is to preserve the structural integrity of teeth while enhancing their resistance to fractures. Endodontically treated teeth are more susceptible to fractures due to the removal of tooth structure and radicular dentin. This study evaluates and compares the fracture resistance of teeth instrumented using three rotary file systems: self-adjusting file (SAF), ProTaper NEXT, and Hyflex EDM. METHODOLOGY: Sixty extracted human mandibular premolars were randomly divided into four groups: Group I (control, uninstrumented), Group II (SAF), Group II

Full-mouth rehabilitation of a patient with bulimia nervosa. A case report.

Anorexia and bulimia nervosa are eating disorders seen mainly in adolescents or young patients. The dentist should be in the position to recognize early signs of the disorder and alert the patient (and the patient's parents, if necessary) of the possible physical, psychologi cal, and dental consequences. Such dental treatment may help motivate the patient to confront the problem. In this paper the full-mouth rehabilitation, using a combination of galvano- and metal-ceramic restorations, of a young patient suffering from bulimia nervosa is described.

Oral surgery: problems.

Having listened to, examined, tested, diagnosed and recommended a course of action to the patient, treatment will usually follow at the same or subsequent visits. In the vast majority of cases, the foregoing will have proceeded without a hitch, and the treatment completed to the satisfaction of both the practitioner and the patient. Years of study, and the acquisition of knowledge, skills and expertise, will have been condensed into a short amount of clinical time. Unfortunately, this highly desirable state of affairs cannot apply every time; problems can arise anywhere along the pathway leadi

[The traumatized immature permanent incisor with pulpal involvement: what conservative treatment options?].

INTRODUCTION: Traumatology is the second most common dental pathology, after caries and before periodontal pathologies. It is currently accepted that one child in two suffers an alveolar-dental trauma before leaving school, with a peak incidence between the ages of 8 and 12, all types of trauma combined. The maxillary central incisors are most affected (79.6%), with mainly "simple" or "complex" coronal fractures when the pulp is involved. The main objective in managing these complex fractures is to preserve pulpal vitality within an acceptable timeframe. MATERIAL AND METHOD: The author presen

Management and Follow-Up of Complicated Crown Fractures with Intrusive Luxation of Maxillary Incisors in an 8-Year-Old Boy.

Intrusive luxation is a severe form of dental injury which causes damage to the pulp and supporting structures of a tooth because of its dislocation into the alveolar process. This paper shows the case of the reeruption of maxillary incisors accompanied by complicated crown fractures after 3 months. An 8-year-old boy patient was referred to the Department of Pedodontic Dentistry of Tehran University of Medical Science, Tehran, Iran, 18 hours after a fall at school. Clinical and radiographic examinations revealed intrusive luxation of both incisors with complicated crown fractures. Cervical pul
