The triad of obesity, hypoventilation and inordinate hypersomnolence characterizes the obesity-hypoventilation syndrome. The reasons for daytime hypoventilation appear related to decreased chemical drives to breathe combined with the added physical impediment of extreme obesity. When the physiology of sleep was investigated in patients with this syndrome, intermittent nocturnal obstructive apneas were documented that produced blood gas abnormalities, arrhythmias and serious elevations of both pulmonary and systemic pressures. The obstructive apneas are due to intermittent loss of muscle tone o

In 23 patients with laryngotracheal trauma at the Louisville General Hospital during a ten-year period, 19 survived. One death was directly atributable to the airway injury. The most common postinjury complication was hoarsensess attributable to direct injury to the cords or recurrent nerve paralysis. One patient had further operation for subglottic stenosis. In general, blunt injuries were more severe than penetrating injuries. Subcutaneous emphysema, aphonia or dyshonia, hemoptysis, signs of injuries, a sucking wound, are presumptive findings of laryngotracheal injury. Appropriate endoscopy

A toddler with upper airway obstruction was admitted to a peripheral hospital, with a differential diagnosis of laryngitis subglottica, unresponsive to treatments and deteriorating. The child was brought to theatre for airway management. After induction, the glottic opening was not visible. Several attempts using different techniques, adjuncts and staff were not successful and a cannot intubate cannot oxygenate scenario, severe hypoxemia and cardiac arrest occurred. After minutes, a scalpel tracheotomy led to reoxygenation and return of circulation. After transfer to a tertiary center, rigid l…

A 50-year-old Japanese man, who experienced cold symptoms for 11 days, presented to our hospital complaining of hoarseness and dyspnea from the morning of the day of the visit. Laryngoscopy revealed bilateral vocal cord paralysis, and tracheotomy was performed. Specific post-admission interviews revealed that he had suffered from postural finger tremor from 30 years of age, fasciculations of his facial muscle from 47 years of age, and mild dysphagia from 49 years of age. Blood tests showed high serum CK levels, chest computed tomography (CT) revealed gynecomastia, and needle electromyography s…

Extraskeletal chondromas (ESCs) are rare benign tumours composed of mature cartilage that typically develop in soft tissues. Their occurrenc … aging ruled out bone involvement. The mass was surgically excised under general anaesthesia, with a tracheotomy performed to secure the airways; a histopathological diagnosis of ESC was achieved. Postoperative recovery was uneventful, and at six months follow-up, the horse had fully returned to normal feeding behaviour, without signs of recurrence. This appea …

BACKGROUND: This case series explores the management of respiratory failure in patients with large anterior tracheal thyroid tumors where tracheostomy is not an option. To our knowledge, this study is the first to address the challenges associated with caring for such patients.
CASE SUMMARY: We present the clinical courses of four intubated adults with advanced thyroid cancer and complex airway issues that preclude surgical tracheostomy. Interventions included custom airway stents, long-term intubation, and oncological therapies. Ethical quandaries around patient autonomy and capacity emerged

INTRODUCTION: We describe a first case of human congenital crico-thyroid dysplasia associated to a right sided aortic arch and an aberrant subclavian artery.
CASE PRESENTATION: Our patient presented with a two-weeks history of acute dyspnea, and reported hoarseness since his childhood. An urgent tracheotomy was performed, followed by direct laryngoscopy. Endoscopic examination showed a deviation of the dorsoventral axis of the larynx, with an obstructive submucosal swelling the area of the right false cord and aryepiglottic fold. Computed tomography conducted the following day confirmed the c…

BACKGROUND: Squamous cell carcinoma of unknown primary (CUP) in the head and neck is difficult to diagnose and treat. This report outlines 1 … ge diameter of the primary tumour was 1.65 cm. All patients resumed eating by mouth within 24 h, no tracheotomy, no pharyngeal fistula and no postoperative death. The 3-year disease-free survival rate was 91%.
CONCLUSIONS: TORS can improve the diagnostic efficiency of primary tumour of CUP and achieve good oncology and functional results.
