OBJECTIVE: Transfers to adult care can be problematic, resulting in postponement due to the protective nature of pediatric care and patient dependency. It is unknown whether these findings apply specifically to urology patients. Our department is taking part in a national general transition project. In this light, our aim was to investigate the specific needs of adolescent urologic patients, regarding their independence and transition.
PATIENTS AND METHODS: 80 patients, born in 1975-1998, with a chronic bladder condition received a questionnaire. They were divided into pre- and post-transfer

INTRODUCTION: Early establishment of a primary care practitioner (PCP) may enhance young adults' ability to navigate the health care system, particularly those new to integrated care.
METHODS: The study population included 18- to 26-year-old individuals newly enrolled in 2023 at Kaiser Permanente Northern California (N = 30,099). This cross-sectional study examined associations between early establishment of a PCP (indicated in the electronic health record) and several outcomes: creation of secure messaging accounts, secure messaging use, appointment and advice call center use, e-visit use, e

OBJECTIVE: The COVID-19 Public Health Emergency prompted multiple policies that expanded telehealth care, including buprenorphine treatment for opioid use disorder (OUD). Prior research has reviewed these policies, but not their structural relationships. We used a system mapping approach to visualize how those policies interacted to influence care delivery.
METHOD: We developed a diagram based on system dynamics conventions to capture patient status and care transitions and provider capacity change, telehealth adoption, and attrition, informed respectively by the cascade-of-care framework and

PURPOSE: The shift from child-centered to adult-focused healthcare presents social and psychological challenges for adolescents and young adults with chronic conditions, which can affect their participation in follow-up care. This study aims to investigate the factors that influence patient-driven motivations for adhering to follow-up recommendations, while also exploring the barriers and supports that impact the transition process for pediatric cancer survivors.
METHODS: We developed interview guidelines grounded in the Theory of Planned Behavior (TPB) and the stereotype priming model (SPM).

PURPOSE: Polycystic ovary syndrome (PCOS) is the most common endocrine disorder among females. The foundation of PCOS self-management is engaging in healthy lifestyle habits, however, there is limited literature regarding adolescents' transition to PCOS self-management. The purpose of this study, therefore, is to explore parental and adolescent views of the transition to PCOS self-management.
DESIGN AND METHODS: A qualitative descriptive approach was used through focus groups (N = 4) with adolescents diagnosed with PCOS and their parents.
RESULTS: A total of seven adolescents and eight paren

In Japan, medical care for patients with muscular dystrophy has improved through multidisciplinary care provided by specialized institutions, resulting in a marked increase in life expectancy. Today, most patients with muscular dystrophy live in their own homes and receive medical care in various non-specialized institutions. Some muscular dystrophy patients have associated central nervous system disorders, which include neurodevelopmental syndromes. In addition, many patients are physically and mentally unstable during adolescence, when the transition from pediatric neurology to adult neurolo

Birth before 32 weeks' gestation, once associated with early mortality, now is commonly associated with long-term survival. Following discharge from the neonatal intensive care setting, the medical home transitions to the primary care setting. As such, primary care clinicians should be empowered with up-to-date knowledge of the spectrum of outcomes experienced by children born prematurely. This review highlights growth, pulmonary, developmental, and neurosensory outcomes in infancy and toddlerhood. For later childhood, neurological outcomes broaden to include behavioral, psychiatric, academic,

BACKGROUND: Understanding the relationship among changes in Clinical Dementia Rating (CDR), patient outcomes, and probability of progression is crucial for evaluating the long-term benefits of disease-modifying treatments. We examined associations among changes in Alzheimer's disease (AD) stages and outcomes that are important to patients and their care partners including activities of daily living (ADLs), geriatric depression, neuropsychiatric features, cognitive impairment, and the probabilities of being transitioned to a long-term care facility (i.e., institutionalization). We also estimate

During Israel's recent "Lion's Roar" conflict, health care systems faced an urgent mandate to expand surge capacity under extreme volatility. This narrative explores the rapid transition of a Nursing Research and Innovation Director from strategic oversight to frontline clinical care within the Hospital at Home (HaH) framework. A primary element of this transition was the adaptation of clinical methods and resources to align with established, evidence-based safety standards in nontraditional care settings. The experience demonstrates that nursing leadership in conflict zones is most effective

This article examines how staff and patients worked to reconcile the rhythms of the body with those of gender-normative health care bureaucracy in a U.S. Midwest gender-affirming health clinic. Drawing from observations of clinical appointments and routine bureaucratic practice, as well as debriefing interviews with transgender and gender-expansive patients, this article applies Laura Bear's theory of "time-maps" and a new materialist approach to bodily agency that recognizes the variability of the body's responses to gender-affirming health care. This evidence demonstrates how health care sta

What housing and service interventions work best to reduce homelessness for families in the United States? The Family Options Study randomly assigned 2,282 families recruited in homeless shelters across 12 sites to priority access to one of three active interventions or to usual care in their communities. The interventions were long-term rent subsidies, short-term rent subsidies, and transitional housing in supervised programs with intensive psychosocial services. In two waves of follow-up data collected 20and 37 months later, priority access to long-term rent subsidies reduced homelessness sa

IntroductionAdolescent and young adult (AYA) cancer survivors are at significant risk for late treatment-related effects yet face challenges when transitioning to survivorship-focused care and longitudinal follow-up. Population-level tools in the electronic health record (EHR) offer a pragmatic approach to track patients and optimize care.MethodsThe Seattle Children's Hospital Cancer Survivor Program created a population health management system, using tools embedded within the EHR, based on a validated registry of AYA survivors. We developed a dynamic dashboard to make care gaps visible survi

The transition from jail to community is a high-risk time for individuals experiencing substance use disorders (SUD), with elevated risks of overdose and other substance-related harms, as well as high recidivism rates and re-incarceration. Gains made from successful treatment in prison are often lost in this transition. The current paper evaluates this process for one justice programme, Chesterfield HARP, a Therapeutic Community based in the Chesterfield County jail that continues supporting former inmates through their release by examining engagement and retention rates and changes in recover

Moderate to severe traumatic brain injury (msTBI) results in physical, cognitive, behavioral, and psychosocial difficulties. Those who receive inpatient rehabilitation following a msTBI need assistance after discharge. Patients and their families often struggle to find information, manage symptoms, and identify and access relevant services. Inadequate transition services from hospital-based care to the community can perpetuate and amplify the consequences of msTBI. There is a critical need for enhanced transitional care following hospital discharge. The goal of the current study was to compare…
