Bedside dysphagia screens in patients with traumatic cervical injuries: An ideal tool for an under-recognized problem.

BACKGROUND: We initiated a prospective interventional study using a nurse-driven bedside dysphagia screen (BDS) in patients with cervical spine injury (CI) to address three objectives: (1) determine the incidence of dysphagia, (2) determine the utility of the new BDS as a screening tool, and (3) compare patient outcomes, specifically dysphagia-related complications, in the study period with a retrospective cohort. METHODS: All patients with CI admitted to a Level I trauma center were enrolled in a prospective 12-month study (June 2016-June 2017) and then were compared with a previous 18-month

[Dissemination of psychotherapy modules for traumatized refugees : Experience gained from trauma work in crisis and conflict regions].

With each additional accumulative exposure to severe and traumatic stressors, the likelihood of developing mental health problems and physical diseases increases. Displaced individuals have usually experienced a number of serious threats to health due to organized violence in their home country or attacks during the flight. Frequently, domestic violence adds additional strain to the stressors experienced. The resulting impairments in psychosocial functioning reduce the resources needed for social adjustment and integration. Social exclusion then in turn often further aggravates the existing me

Managing a surgical exsanguination emergency in the operating room through simulation: an interdisciplinary approach.

OBJECTIVE: Operating room (OR) emergencies, such as fire, anaphylaxis, cardiac arrest, and exsanguination, are infrequent, but high-risk situations that can result in significant morbidity and mortality. An exsanguination scenario involving a pregnant trauma patient in the OR was developed for surgery residents with the objectives of improving overall team performance when activating an emergency response system, identifying a team leader, initiating an exsanguination protocol, following advanced cardiac life support guidelines, and recognizing the mother as the first patient. STUDY DESIGN: D

Spine and spinal cord injuries--causes and complications.

BACKGROUND: The spine as a motor organ is very often exposed to the action of forces released by an injury. The most frequent cause of spine and spinal cord injuries are traumas which are the result of accidents, and untreated osteoporosis or neoplasms as well. OBJECTIVES: Aim of the study is an analysis of the causes of spine and spinal cord injuries as well as of the complications of these injuries. MATERIAL AND METHODS: The material covers 130 patients hospitalized due to spine injury in 2008-2010 at the Clinical Department of Neurosurgery of Wroclaw Medical University. The data has been

Restoring healthy developmental trajectories for sexually exploited young runaway girls: fostering protective factors and reducing risk behaviors.

PURPOSE: To examine effects of the Runaway Intervention Program (RIP), a strengths-based home visiting, case management, and group support program for sexually assaulted or exploited young runaway girls; staffed by advanced-practice nurses, RIP aims to restore healthy developmental trajectories by reestablishing protective factors, reducing trauma responses, and lowering risk behaviors that are common sequelae of sexual violence. METHODS: Quasi-experimental study of RIP girls (N = 68) ages 12 to 15 (mean, 13.75 years), assessed at baseline, 6 and 12 months for changes in family and school con

From Consensus to Practice: Implementation and Impact of the Emergency Medicine Simulation Curriculum for Pediatrics (EM ReSCu Peds).

Background Exposure to critically ill children during emergency medicine (EM) residency is limited and variable. General EDs care for over 85% of the 36 million annual pediatric ED visits in the U.S., highlighting a training gap. To address this, the Emergency Medicine Resident Simulation Curriculum for Pediatrics (EM ReSCu Peds) was developed by more than 50 pediatric simulation experts as a free, open-access curriculum of 16 peer‑reviewed cases. Representatives from 10 national EM, pediatric EM (PEM), and simulation organizations, along with contributors from 44 institutions, created and ite

A case-control study of surgical site infection following operative fixation of fractures of the ankle in a large U.K. trauma unit.

Most of the literature on surgical site infections following the surgical treatment of fractures of the ankle is based on small series of patients, focusing on diabetics or the elderly. None have described post-operative functional scores in those patients who develop an infection. We performed an age- and gender-matched case-control study to identify patient- and surgery-related risk factors for surgical site infection following open reduction and internal fixation of a fracture of the ankle. Logistic regression analysis was used to identify significant risk factors for infection and to calcu

Refractory increased intracranial pressure in severe traumatic brain injury: barbiturate coma and bispectral index monitoring.

Patients with severe traumatic brain injury resulting in increased intracranial pressure refractory to first-tier interventions challenge the critical care team. After exhausting these initial interventions, critical care practitioners may utilize barbiturate-induced coma in an attempt to reduce the intracranial pressure. Titrating appropriate levels of barbiturate is imperative. Underdosing the drug may fail to control the intracranial pressure, whereas overdosing may lead to untoward effects such as hypotension and cardiac compromise. Monitoring for a therapeutic level of barbiturate coma in

The incidence of skin breakdown associated with use of cervical collars.

The most common adverse complication associated with cervical immobilization is skin breakdown. The purpose of this prospective, descriptive study was to assess the incidence of tissue breakdown associated with cervical immobilization. In this convenience sample of 484 patients, skin breakdown was noted in 33 (6.8%) patients. All cases of documented skin breakdown were stage I or II, with only 2 (0.4%) patients having stage III breakdown. Days in the cervical collar is a significant predictor of skin breakdown, along with presence of edema. Results from this study demonstrate that there is a v

Posttraumatic stress disorder and epigenetic signatures of inflammation in middle-aged women.

BACKGROUND: Post-traumatic stress disorder (PTSD) has been linked to elevated serological and plasma concentrations of inflammatory biomarkers in large cross-sectional studies. Methylation signatures for inflammation provide an assessment of inflammatory state beyond the time of blood draw, but such signatures have not been examined in relation to PTSD. In this study, we hypothesized that PTSD would be associated with inflammation-related epigenetic signatures and changes in these levels over time. METHODS: Data are from 831 trauma-exposed women in the Nurses' Health Study II. Lifetime PTSD d

Establishing and Improving Role 2 Documentation Performance: A 2-Part Quality Improvement Initiative.

INTRODUCTION: Accurate and comprehensive medical documentation is essential to safe, effective patient care, particularly in austere environments where patients transition rapidly between teams, locations, and levels of capability. In Role 2 environments, where casualties move from point of injury through damage control resuscitation, surgery, and prolonged casualty care, information loss during transitions poses a persistent threat to patient safety. Despite the existence of Joint Trauma System (JTS) Clinical Practice Guidelines and standardized documentation tools, variability in documentati

'I don't really know where I stand because I don't know if I took something away from her': Moral injury in South African speech-language therapists and audiologists due to patient death and dying.

BACKGROUND: Speech-language therapists and audiologists (SLT&As) may encounter difficulties when confronted with patient death and dying, which may conflict with their moral beliefs and result in moral injury. Furthermore, South African SLT&As practice in a country with a high mortality rate, which may add to the complexity of their experience. Moreover, they may be influenced by African philosophies promoting care, which might conflict with their experiences of patient death and dying. AIMS: To explore the moral injury experienced by South African SLT&As in patient death and dying, and how t

Temporal Patterns of Sexual Assault Presentations in U.S. Emergency Departments: Implications for Staffing and Forensic Readiness.

BACKGROUND: Sexual assault (SA) remains a major public health issue, yet only a fraction of survivors seeks acute medical care in emergency departments (EDs). Although previous studies describe demographic disparities among SA survivors, national patterns of ED arrival timing and their operational implications for emergency care systems remain poorly characterized. STUDY OBJECTIVES: To characterize the demographic and temporal patterns of ED presentations for SA in the United States. METHODS: We conducted a repeated cross-sectional study using the Epic Cosmos database, including U.S. ED enco

The Reconsolidation of Traumatic Memories Protocol's adjustments to the remote treatment of injured Ukrainian military personnel in hospital settings.

Background: Due to the Russian invasion of Ukraine and the intense battlefield combat, many Ukrainian defenders have severe gunshot and explosion injuries, which result in broken bones, spinal damage, limb loss, and more. This physically and emotionally intensive experience often leads to acute stress disorder (ASD) and/or post-traumatic stress disorder (PTSD). During post-surgical recovery, injured military personnel need trauma-centred psychotherapy, which is often unavailable because of the hospital's setting limitations. Objective: The article aims to present adjustments of the Reconsolidat

Workplace Violence in an Urban Community Emergency Department: A Deeper Dive into the Antecedents and Circumstances of Violent Behaviors.

BACKGROUND: Workplace violence (WPV) in emergency departments (EDs) is a growing concern, with significant impacts on staff safety and patient care. While previous studies have largely focused on tertiary care centers, data on WPV in community EDs remain limited. Understanding the characteristics and circumstances surrounding WPV events is critical for developing effective prevention and mitigation strategies. METHODS: We conducted a retrospective observational study of WPV incidents at an urban community teaching hospital ED in Boston, Massachusetts, from January 2021 to April 2023. Using ho

Clinical Outcomes of the First 200 Cases of Single-Operator TEE-Guided Left Atrial Appendage Occlusion (SOLOCLOSE).

SOLOCLOSE is a simplified methodology for left atrial appendage occlusion (LAAO) with the Watchman device consisting of single-operator TEE guidance, nurse-driven conscious sedation, same-day discharge, and deferment of any preprocedural imaging. We aim to investigate the efficacy and safety of LAAO with the SOLOCLOSE methodology. A single-center prospective analysis of 208 consecutive patients undergoing SOLOCLOSE between December 2020 and January 2024 was performed. The primary efficacy outcome was the rate of significant peri‑device (>5 mm) leak at 45-day TEE. The primary safety outcome was

Genetic analysis of cervical cancer with lymph node metastasis.

BACKGROUND: Previous studies on inter-rater reliability of pediatric triage systems have compared triage levels classified by two or more triage providers using the same information about individual patients. This overlooks the fact that the evaluator can decide whether or not to use the information provided. The authors therefore aimed to analyze the differences in the use of vital signs for triage modification in pediatric triage. METHODS: This was an observational cross-sectional study of national registry data collected in real time from all emergency medical services beyond the local eme
