BACKGROUND: To support Tanzania's quest for certification free of human yaws, we tested children with skin ulcers for yaws infection living near areas of reported nonhuman primate (NHP) infection with Treponema pallidum subsp. pertenue (TPE). The study also aimed to test children for infection with Haemophilus ducreyi (HD), the most common differential diagnosis for yaws.
METHODOLOGY/PRINCIPAL FINDINGS: A cross-sectional design was used to study primary school children with skin ulcers living near wildlife areas with TPE infected NHPs. Serum and ulcer swab samples were collected from the chil…

The time course of antibody synthesis during syphilis was studied in experimentally infected rabbits. A rapid antibody response was seen; the rabbits became positive in both the rapid plasma reagin (RPR) test and Treponema pallidum haemagglutination assay (TPHA) by nine days after infection. Treponemal immobilising antibodies were also seen as early as nine days after infection. Antibody inhibition of treponemal attachment to baby rabbit genital organ (BRGO) cells in culture occurred with immune sera taken 30 days after infection but not earlier. When T pallidum was mixed with immune syphiliti…

Syphilis is a systemic sexually transmitted infection caused by the invasive bacterium Treponema pallidum. The diagnosis remains challenging since the disease is labeled as "the great imitator" due to its broad clinical and histopathological appearances. Due to a significant rise in its incidence rates, syphilis should always be considered as a differential diagnosis of any painless, newly developed, oropharyngeal ulcerated lesion. This case demonstrates the rare clinical appearance of a chancre on the tongue as the primary clinical sign of syphilis.

Syphilis is a sexually transmitted, chronic, systemic infection caused by the spirochete Treponema pallidum. If left untreated the disease progresses through 3 stages. The ocular manifestations are highly variable, including interstitial keratitis, anterior, intermediate and posterior uveitis, chorioretinitis, retinitis, retinal vasculitis and optic neuropathy which can occur at all stages. In patients with a suspected diagnosis it is mandatory to carry out serological tests, because an early treatment is essential for preserving visual acuity and ocular function.

We present a case of a patient coinfected with syphilis and the human immunodeficiency virus (HIV) who had unusual and severe cutaneous ulceration. The profound immune defects associated with HIV may lead to an altered clinical presentation and a more aggressive course in patients infected with Treponema pallidum. Despite non-confirmatory histological findings, we feel our patient's cutaneous ulcers probably represent superficial gummata, which have failed to resolve completely following currently accepted high-dose antisyphilis chemotherapy.

OBJECTIVES: Bovine digital dermatitis (BDD) is a globally endemic, painful infectious foot disease of cattle; however, gaps in knowledge regarding treatment efficacy limit our ability to control it. Our objectives were to collect data on minimum inhibitory concentrations and minimum bactericidal concentrations (MICs and MBCs) for conventional and naturally derived therapeutic agents against specific Treponema strains important in BDD pathogenesis in the context of the need for responsible use of antimicrobials. Since antibacterial metals are widely used to control infectious lameness in rumina…

Cases of syphilis are increasing worldwide. Missed diagnosis of syphilis can have serious consequences, particularly in pregnancy. In areas where yaws, a non-sexually transmitted infection due to Treponema pallidum sub-species pertenue (T. pertenue) is endemic, the interpretation of syphilis serology is complex. In Vanuatu, where infection with yaws is prevalent, patients may have positive treponemal serology because of the latter in childhood. We present a case of secondary syphilis with an atypical rash in a post-partum woman in Vanuatu.

Syphilis is a re-emerging disease, and suspicion of syphilis infection should be considered in all cases with atypical skin manifestations, mainly when there is a history of unprotected sexual intercourse. Here, we describe a case of secondary syphilis in a woman aged 42 years, who presented with an atypical, painful and itchy skin rash with vegetative cerebriform lesions, which resolved after the administration of penicillin. Multiplex PCR targeting Treponema pallidum on vaginal and lesion swabs, along with syphilis serology, confirmed the diagnosis.

Blastomycosis is a systemic endemic fungal infection caused by Blastomyces dermatitidis, a dimorphic fungus, that predominantly affects the lungs but can disseminate to other organs, mimicking many diseases, making diagnosis challenging. We present a case of disseminated blastomycosis in a male, initially misdiagnosed as syphilis due to a false positive biopsy result for Treponema pallidum. This case highlights the importance of considering atypical presentations of fungal infections, even in the presence of laboratory findings and outside of the normal geographic regions they may be found.

Syphilis is a multi-system disease caused by Treponema pallidum. Apart from congenital syphilis, it is seen exclusively in sexually active populations, as it has a sexual route of transmission. Children experiencing sexual abuse can acquire syphilis. We report a 12-year-old male child who was a victim of sexual abuse and had presented with a perianal verrucous mass. Immediate surgical resection was opted for to relieve constipation caused by the obstructing mass. The case was managed under the POCSO Act 2012.

Congenital syphilis is an outcome of maternal syphilis that occurs due to the transmission of Treponema pallidum via the placenta of untreated or inadequately treated pregnant women to their newborns. It is now a very rare cause of neurological, developmental, and musculoskeletal disability and death in infants after the advent of penicillin. Here, we report a case of late congenital syphilis presented with classic stigmata of syphilis at the age of 10 years. Reactive serological titer of rapid plasma reagin test and venereal disease research laboratory test confirmed the diagnosis.

Infection with Treponema pallidum is on the rise. In this narrative literature review, we show that the incidence of rare manifestations of syphilis, such as intracerebral gummata, is increasing and should be considered in the differential diagnosis of intracerebral lesions. With the exemplary case that we present here, we aim to raise awareness of the resurgence of this disease, which should be considered in the differential diagnosis of intracerebral lesions, especially for patients who have a risk profile for syphilis, and serological testing for T. pallidum prior to surgery should be discu…

Syphilitic proctitis is a rare disease that usually presents as proctitis, ulcer, and neoplasm but lacks pathognomonic clinical symptoms. It is thus difficult to diagnose and may be treated inappropriately. We report a 31-year-old man who had a hard, ulcerated mass that occupied the rectal and sigmoid colon wall and mimicked a tumor. Fortunately, a biopsy of the mass demonstrated Treponema pallidum organisms consistent with syphilitic proctitis. The patient was successfully treated with intravenous benzyl penicillin, resulting in improvement in his proctitis.

Sexually transmitted infections are a growing public health concern in Canada, with rates of Chlamydia trachomatis infection, gonorrhea and syphilis increasing among adolescents and young adults. The present practice point outlines epidemiology, risk factors, laboratory testing and management for C trachomatis, Neisseria gonorrhoeae and Treponema pallidum, with a lesser focus on HIV. The need for test-of-cure and indications for further investigations are also discussed. The importance of maximizing opportunities to screen for and treat sexually transmitted infections in this age group is high…

Herein a case is described of a 54-years old patient, HIV negative, with cerebro-vascular disease by basilar artery thrombosis secondary to meningovascular neurosyphilis. Neurosyphilis is the impairment at any stage of the central nervous system by Treponema pallidum subspecies pallidum and includes asymptomatic and symptomatic forms of infection. The presentation can take many forms, depending on the location and extent of tissue damage. The currently recommended treatment is crystalline penicillin, 4 million units every 4 hours for 14 days.

Neurosyphilis is the infection of the central nervous system with treponema pallidum species, and true to its reputation as the old mimic, can present with a multitude of clinical scenarios, one of which is stroke in the young. Rare in developed countries, except for an at-risk population of drug abusers and HIV infected patients; it is still common in the rest of the world. We describe the case of a young Indian male, his presentation, diagnosis and treatment, and remarkable recovery on aqueous penicillin G therapy following his presentation with a stroke.

Late congenital syphilis was diagnosed in a 22-year-old woman on the basis of clinical, serologic and epidemiologic features. Clinical examination showed the "saddle-nose" deformity, Hutchinson's teeth, high-arched palate, Dubois little-finger sign, and small maxillae resulting in shallow depression of the central part of the face. Two years after receiving adequate treatment for late congenital syphilis, she presented with a primary syphilitic chancre; thus it was concluded that the woman had no immunity to reinfection with Treponema pallidum.

Hepatitis viruses, enteric pathogens, and anorectal infections may commonly be transmitted by various sexual practices. Because of their larger numbers of sexual partners and sexual practices such as anilingus and anal intercourse, homosexual men are at particularly high risk of acquiring hepatitis B, giardiasis, amebiasis, shigellosis, campylobacteriosis, and anorectal infections with Neisseria gonorrhoeae, Chlamydia trachomatis, Treponema pallidum, herpes simplex virus, and human papilloma viruses. The evidence for sexual transmission of these infections as well as their diagnosis and treatm…
