Primary vaccination and revaccination of young adults with BCG: a study using immunological markers.

Questions have been raised about the effectiveness of Bacille Calmette-Guérin (BCG) vaccination against tuberculosis (TB) in adults. We therefore analysed the immune response after BCG vaccination in primary-vaccinated and revaccinated young adults. 31 tuberculin skin test (TST) negative healthy students were BCG-vaccinated; 15 were primary-vaccinated and 16 revaccinated. Tuberculin-induced lymphocyte transformation (LT) and cytokine production of peripheral blood mononuclear cells were studied before BCG vaccination, as well as after 2 months and 1 y. In the primary-vaccinated as well as the

The influence of BCG immunisation on tuberculin reactivity and booster effect in adults in a country with a high prevalence of tuberculosis.

The relationship of age and previous BCG vaccination with tuberculin skin test (TST) reactivity was investigated to assess the interpretation of TST results in the adult population of Turkey, where there is a high prevalence of tuberculosis and a routine BCG vaccination programme. The influences of age and BCG vaccine status on booster reaction were also evaluated. TST was applied (5 tuberculin units of purified protein derivative intradermally) to two healthy adult groups, namely 98 medical students and 187 elderly people in a retirement home. The TST was considered positive if an induration

Vaccination of guinea pigs with DNA encoding the mycobacterial antigen MPB83 influences pulmonary pathology but not hematogenous spread following aerogenic infection with Mycobacterium bovis.

Protection of cattle against bovine tuberculosis by vaccination could be an important control strategy in countries where there is persistent Mycobacterium bovis infection in wildlife and in developing countries where it is not economical to implement a tuberculin test and slaughter control program. The main aim of such a vaccination strategy would be to reduce transmission of infection by reducing the lung pathology caused by infection and preventing seeding of the organism to organs from which M. bovis could be excreted. Recent reports of successful DNA vaccination against Mycobacterium tube…

Hyperendemic pulmonary tuberculosis in a Peruvian shantytown.

Estimates of the incidence of pulmonary tuberculosis in developing countries are based on case reporting from local health laboratories or the annual risk of tuberculin skin test conversion. Because these methods are problematic, the authors used a multiple case ascertainment method to estimate the incidence of pulmonary tuberculosis from 1989 to 1993 in a Peruvian shantytown of 34,000 inhabitants. Two methods, face-to-face interview of all local inhabitants and examination of local laboratory smear records, were used for case gathering. The number of missed cases was estimated by capture-reca

Immunosuppressive effects of ochratoxin A in young turkeys.

Ochratoxin A (OA), a potent nephrotoxic mycotoxin, was fed to day-old turkey poults at 0 and 4 parts/10(6) dietary levels for 4 and 10 weeks, and its effects, particularly on the immune system, were studied. OA resulted in a significant retardation of growth in both males and females. OA caused a regression of the thymus and bursa of Fabricius and lymphoid depletion in these and other lymphoid organs. Analysis of covariance revealed an enlargement of the kidneys at 10 weeks of age in OA-treated birds; the liver and spleen were enlarged in birds treated with bovine serum albumin (BSA). Cell-med

Vitamin D Concentrations in Infancy and the Risk of Tuberculosis Disease in Childhood: A Prospective Birth Cohort in Cape Town, South Africa.

BACKGROUND: Low vitamin D levels may increase the risk of tuberculosis disease; however, previous observational cohort studies showed variable results. We investigated the relationship between vitamin D levels in infancy and subsequent development of tuberculosis disease throughout childhood. METHODS: We enrolled pregnant women at 20-28 weeks' gestation attending antenatal care in a periurban South African setting in the Drakenstein Child Health Study. Serum 25(OH)D concentrations were measured in newborn infants aged 6-10 weeks. Children were followed prospectively for tuberculosis infection

Molecular signatures distinguishing active from latent tuberculosis in peripheral blood mononuclear cells, after in vitro antigenic stimulation with purified protein derivative of tuberculin (PPD) or Candida: a preliminary report.

Purified protein derivative (PPD) or tuberculin skin testing is used to identify infected individuals with Mycobacterium tuberculosis (Mtb) and to assess cell-mediated immunity to Mtb. In the present study, we compared PBMC cultures in the presence of tuberculin or Candida antigens using cytokine bead arrays and RNA microarrays. Measurements of different cytokines and chemokines in supernatants of PMBC cultures in the presence of PPD showed increased levels of interferon (IFN)-gamma in active tuberculosis infection (ATBI) and latent TB infected (LTBI) compared to controls, and increased levels

Hidden granuloma in plain sight: primary multimucosal tuberculosis (a case report).

Primary mucosal tuberculosis is an exceptionally rare extrapulmonary manifestation, representing a diagnostic challenge due to its marked anatomo-clinical polymorphism and paucity of systemic signs. We report a 62-year-old immunocompetent woman with multifocal mucosal facial plaques showing a fluctuating course. Dermoscopy demonstrated a lupoid pattern. Radiologic evaluation revealed sinonasal extension with osseous lysis. Histopathology confirmed confluent Koester follicles with central caseous necrosis. Systemic evaluation showed no pulmonary or visceral involvement, with only a positive tub

Clinical application and limitations of interferon-gamma release assays for the diagnosis of latent tuberculosis infection.

Interferon-release assays (IGRAs) represent advances in tuberculosis immunology and evolutionary biology. IGRAs were designed to replace tuberculin skin test (TST) for the diagnosis of latent tuberculosis infection because of their logistical advantages and enhanced specificity over TST. Although IGRAs and TST have been useful in epidemiologic studies, they lack the sensitivity and reproducibility normally expected from diagnostic tests in clinical practice. In this review, we present an overview of the current recommendations and knowledge in the field and discuss practical approaches in area

A case of lung tuberculosis in a patient with rheumatoid arthritis treated with infliximab after antituberculosis chemoprophylaxis with isoniazid.

We report a case of a 65-year-old man with rheumatoid arthritis. The patient was treated with methotrexate and prednisolone, but the disease activity remained high. A tuberculin skin test was positive. After antituberculosis (TB) chemoprophylaxis with isoniazid for four weeks, infliximab was administered. Chemoprophylaxis was continued for nine months. Active lung TB was diagnosed at 17 months after the cessation of isoniazid, namely at 27 months after starting infliximab treatment. This case report shows that TB can manifest after chemoprophylaxis in patients treated with antitumor necrosis f

CASE REPORT: A healthy 57 year-old woman with past untreated pulmonary tuberculosis, disclosed a bilateral uveitic syndrome characte rized by iritis, vitreitis, multiple peripheral retinal punched-out lesions, and cystoid macular edema. Systemic evaluation was unremarkable except for a 30 mm tuberculin skin test. Relapses occurred after oral and periocular corticosteroids were interrupted, but the inflammation completely disappeared after a 6 month course of isoniazid, rifampicin and pyrazinamide. DISCUSSION: Intraocular tuberculosis should be considered as a treatable cause of peripheral mul

Effects of dialyzable transfer factor in patients with breast cancer.

Five patients with advanced breast cancer were treated with pooled dialyzable transfer factor from healthy adult donors. The period of treatment ranged from 21 to 310 days, the total dose from 20 to 257 ml. Transfer factor did not elicit inflammatory or hypersensitivity reactions or detectable formation of antibody to itself, nor any hematological or biochemical abnormalities or other side effects. Three patients became responsive (by skin test) to tuberculin and/or streptococcal antigens. Marked partial regression of the breast cancer, lasting 6 months, was observed in one patient.

Preliminary immunological studies in spinal muscular atrophy.

Evidence of impaired cell-mediated immunity in children with spinal muscular atrophy was obtained using lymphocyte transformation with PHA (20 children) and skin tests with tuberculin and DNCB (35 children). Laryngological examination of 16 children demonstrated hypoplasia of lymphatic tissue in Waldeyer's ring and the cervical lymph nodes. An ectodermal defect involving both spinal cord and thymus is suggested.

[Interferon-Gamma Release Assays in the diagnosis of latent tuberculosis infection in clinical situations].

Until recently, the basic test to identify latent tuberculosis infection (LTBI) was the tuberculin skin test, despite its limitations in the form of low sensitivity and specificity. Currently, Interferon Gamma Release Assays from peripheral blood are used for a rapid diagnosis of LTBI and measurement of the interferon gamma (IFN-g) levels secreted by specific T cells stimulated with Mycobacterium tuberculosis antigens. Detection of LTBI is important in the control of people potentially at risk of TB disease, such as people remaining in close contact with BK (+) tb patient and for patients eval

[Miliary tuberculosis and adult respiratory distress syndrome in an infant].

We report on a 7 months old infant, who was admitted to our hospital with the tentative diagnosis of interstitial pneumonia. The infant was treated with erythromycin because of a positive mycoplasma antigen test in the nasopharyngeal mucus and the radiological findings. A tension pneumothorax led to rapid respiratory deterioration and a severe ARDS developed. A sustained respiratory stabilization could not be achieved. The diagnosis miliary tuberculosis was detected on autopsy. A parenchymal focus was found subpleurally in the left lung with a massive hematogenous spread and a tuberculous meni

An outbreak of tuberculosis caused by M. bovis in imported Alpaca's in the Netherlands is reported. A review of the outbreak of the disease is followed by a more detailed discussion of the symptoms, diagnostic testing with the tuberculin skin test and the ELISA test, the gross lesions, the bacteriological examinations, the control measures and the legal aspects. As tuberculosis is not referred to in the Live-Stock Act, the control measures had to be based on the humane Control of Infectious Disease and Tracing the Causes of Disease Act. In the discussion, it is advocated to pay attention to mo

[Changes induced by calf thymus extract on cell-mediated immunity, evaluated by intradermal tests, in the elderly].

A thymic calf extract (LEUCOTROFINA) was injected twice a day for 20 days to 24 elderly patients, in order to establish its efficacy to improve the lymphocytic "memory" against the antigens commonly used in the skin tests (Candidin, Tuberculin, Varidase, Mumps, Trichophytin) and the DNCB test. As far as the recall antigens were concerned, no remarkable improvement was observed in patients over 75 years, while a significant positive response (from 47% to 84,6%- P less than 0,05) was noticed in patients treated with leucotrophina aged between 65 and 75 years, compared to not treated control grou

OCULAR HISTOPLASMOSIS.

Case reports of three residents of Ontario with clinical histoplasmic chorioretinitis are presented. The diagnosis was made on the basis of the clinical appearance, the presence of calcified lesions in the chest, a negative skin test to tuberculin, and a positive skin test to toxoplasmin. All patients were treated with intravenous amphotericin B. Except for transitory elevation of blood urea nitrogen, there were no serious complications from the drug and in all cases the lesions in the eyes were improved. Histologic or cultural proof of the presence of fungus in the eye is not available, but c
