Bladder neck suspension under local anesthesia is a new technique for the treatment of stress incontinence in females which can be done under private clinic conditions. The procedure is a modification of Stamey-Pereyra operation. It has already been applied in 34 patients with a success rate of 80% with low morbidity. A higher success rate was achieved in patients who had no gynaecological or surgical operations previously. The purpose of the technique that we used on these patients is to restore anatomic position without causing urethral obstruction.

We reported a case of spontaneous pneumothorax associated with pulmonary metastasis of urinary bladder carcinoma. Experiments were designed to gain information on the mechanisms leading to diabetic urinary bladder dysfunction. Bladders from control rats, animals subjected to 4-5 wk streptozocin-induced diabetes, and rats subjected to equivalent diuresis produced by 5% sucrose feeding were studied with an in vitro whole-bladder preparation and neurochemical measurements. The diuretic group was used to distinguish alterations produced by metabolic effects on nerve and muscle from those induced by prolonged periods of excessive diuresis. Four young girls with myelodysplasia who remained incontinent despite intermittent catheterization and pharmacological manipulation underwent suprapubic bladder neck suspension along with bladder augmentation. Of the patients 3 underwent augmentation for decreased detrusor compliance and 1 as a part of a planned urinary undiversion. There was a mean increase in continence length of 1 cm. and a mean increase of 49 cm. water in urethral closure pressure in all patients. In two groups composed of 228 patients with bladder tumors and 37 patients with upper urinary tract (UUT) tumors, we compared the transitional cell carcinoma in respect to sex, age, grade of dysplasia, staging, and survival. No differences could be established between the two groups, and we found the same 5-year survival rates of approximately 26% and 29%, respectively, for the invasive carcinomas.

Rapid structural and functional alterations have been noted in several models of partial outlet obstruction. To better characterize the rapid progression of alterations, the partially obstructed urinary bladders of mature NZW male rabbits were studied at 1, 3, 5, 7 and 14 days of outlet obstruction with respect to muscarinic receptor density, DNA, RNA, lipid and hydroxyproline content. We describe a primary mixed adenocarcinoma-neuroendocrine carcinoma of the urinary bladder of probable urachal origin. Neuroendocrine differentiation was confirmed by ultrastructural (neurosecretory granules) and immunohistochemical studies (chromogranin and neuron-specific enolase). Two local recurrences and multiple metastases consisted exclusively of the neuroendocrine component. The patient died 30 months after diagnosis with widely metastatic neuroendocrine carcinoma.

Cutaneous vesicostomy was used for temporary urinary diversion in 12 infants with neurogenic vesical dysfunction. Hydronephrosis, azotemia and clinical urinary infection were resolved in all patients. Postoperative complications consisted of diaper dermatitis in 42 per cent and vesical prolapse in 25 per cent.

The mucopolysaccharide production of urachal and metastatic adenocarcinomas in the urinary bladder has been investigated. It is shown that urachal carcinomas do not produce sulphated acid mucopolysaccharides, whereas the metastases from the large bowel do. Since colonic carcinomas constitute the metastatic adenocarcinomas most common to the bladder, this histochemical difference is regarded as a useful distinguishing feature.

The morphologic response of smooth muscle cells in the urinary bladder to a standardized bladder neck obstruction and the cell changes subsequent to release of that obstruction were studied in both growing and adult rabbits.
