[Characterization of balance function during the interictal period of vertigo attacks in patients with Ménière's disease].

This study aimed to investigate the characteristics of balance function during the interictal period in patients with Ménière's disease (MD) and analyze related influencing factors. A retrospective analysis was conducted on the clinical data of 183 MD patients admitted to the Vertigo Department of Shandong ENT Hospital between July 2021 and December 2023. Balance function was assessed using the sensory organization test (SOT), along with evaluations of mean hearing threshold, semicircular canal function, otolith organ function, and endolymphatic hydrops status. The cohort comprised 89 male and

Evaluation of Vertical Semicircular Canal Function in Patients With Vestibular Schwannoma.

OBJECTIVES:: The aim of this study was to investigate vertical semicircular canal function in patients with vestibular schwannoma (VS) by video head impulse test (vHIT). METHODS:: Fifteen patients with VS who had not received any treatment, including surgery or stereotactic radiotherapy, before vHIT examination were enrolled. Vestibulo-ocular reflex gain and catch-up saccade in vHIT were evaluated. RESULTS:: Dysfunction of anterior and posterior semicircular canals was detected by vHIT in 26.7% and 60.0%, respectively. Six patients (40.0%) demonstrated abnormalities referable to both vestibu

Menière's disease and diphenidol. A critical analysis of symptoms and equilibrium function tests.

In 24 patients with Ménière's disease, chemotherapy using Diphenidol (1,1-diphenyl-4-piperidino-l-butanol hydrochloride), a non-phenothiazinic antiemetic agent, was carried out using the double-blind technique in the cross-over design. The prevalence and intensity of symptoms at the three stages were recorded every three weeks according to fixed scales which were graded: vertigo, unsteadiness, tinnitus, nausea, headache, and shoulder stiffness. The hearing was measured by tone audiometry. Equilibrium function tests were also performed every three weeks and these included the electronystagmogra

Assessing gait variability concurrently with dynamic visual acuity on a treadmill in people with bilateral vestibulopathy.

BackgroundGait variability is increased in people with bilateral vestibulopathy (BVP). Since dedicated gait analysis can be resource-intensive, concurrent assessment with another vestibular function test, dynamic visual acuity (DVA), is worth consideration.ObjectiveTo assess comparability of results from a combined gait and DVA assessment with results from a previous dedicated gait analysis.Methods15 participants (4 women) with BVP were analysed. The DVA test assessed visual acuity during stance and during treadmill walking at 2, 4 and 6 km/h. An 8-camera motion capture system measured spatiot

[Cardinal symptom vertigo from the neurologist's perspective].

In most patients with vertigo, the first and clinically most important question posed to neurologists is whether it is a central or a peripheral syndrome. In more than 90 % of cases, this differentiation is made possible by systematically recording the patient history (asking about the type of vertigo, the duration, triggers and accompanying symptoms) and conducting a physical examination. Particularly in the case of acute vertigo disorders, a five-step procedure has proven useful: 1. A cover test to look for vertical divergence (skew deviation) as a central sign and component of the ocular ti

Two-dimensional analysis of head-shaking nystagmus in patients with Meniere's disease.

The HSN test is a simple examination that can be easily performed by clinicians, however only a few studies have analyzed the 2-dimensional characteristics of HSN in patients with MD. The objective of the study was to characterize different types of 2-dimensional head-shaking nystagmus (HSN) in patients with Meniere's disease (MD). Sixty-five patients with definite MD were enrolled. HSN was considered pathologic, if slow-phase velocity (SPV) was ≥ 4°/s and was classified as paretic or recovery according to the direction, and as monophasic or biphasic according to the presence of direction chan

Vestibular changes after guinea pig cytomegalovirus infection.

A guinea pig model of viral labyrinthitis was established by inoculating guinea pig cytomegalovirus into the inner ear and then determine its effect on vestibular function. Neither spontaneous nor positional nystagmus could be detected postoperatively. Nine out of 18 (50%) animals had caloric hyperfunction in the virus-inoculated ear at 1 to 3 weeks postoperatively. Five of these 9 animals also had caloric hyperfunction in the control ear. A morphological study in animals with caloric hyperfunction showed no change in the peripheral vestibular apparatus by light microscopy. It is thus assumed

Changing response to caloric stimulation with gestational age in infants.

Internuclear ophthalmoplegia was found by caloric testing of the vestibuloocular reflex in normal, very premature infants, indicating that the medial longitudinal fasciculus is functionally immature. At this age, the medial longitudinal fasciculus is more responsive to stimulation by the oculocephalic (doll's-eye) reflex, which resulted in conjugate deviation of the eyes.

Air caloric testing in otitis media. (preliminary studies).

On certain occasions it becomes important to evaluate vestibular function in a patient with otitis media. The potential application of the air caloric test in evaluating such patients was examined. Patients with unilateral otitis media before and after surgery were studied to answer certain clinical questions. More questions were raised than answers provided. The preliminary conclusions from this study are: 1. Patients with tympanostomy tubes or small perforation of one ear may show a caloric response in the perforated ear equal to that of the intact ear. 2. Patients with a large tympanic memb

The nystagmus and the therapeutic effects on early-diagnosed cases of sudden deafness.

We investigated the hearing prognoses of early diagnosed cases of sudden deafness. We tried to find the most reliable factor to determine the prognosis of hearing loss. Of various possibilities that affect the prognosis; the period of onset to first examination, therapeutic manner and vestibular test findings, the findings of caloric test and spontaneous nystagmus showed much better correlation with the hearing prognosis than others. Thus, we presume the vestibular dysfunction in cases of sudden deafness may reflect the extension and severity of inner ear involvement.

Vestibular Evoked Myogenic Potential in Vestibular Migraine: A Systematic Review of Diagnostic Utility.

Background/Objectives: Vestibular migraine (VM) is one of the most prevalent causes of episodic vertigo, yet it remains underdiagnosed due to overlapping features with other vestibular disorders and the absence of definitive diagnostic tests. Vestibular evoked myogenic potentials (VEMPs) assess otolith and vestibular nerve function and may help identify pathophysiological mechanisms in VM. This systematic review aimed to evaluate the usefulness of VEMP in understanding VM, synthesize existing findings, and explore its clinical implications. Method: A systematic search was performed in PubMed,

New onset episodic vertigo as a presentation of vestibular neuritis.

OBJECTIVE: Vestibular neuritis (VN) is a common peripheral cause of acute vestibular syndrome, characterized by sustained vertigo and gait instability, persisting from 1 day to several weeks. With the widespread use of comprehensive vestibular function tests, patients with VN and non-sustained vertigo have drawn attention. In this study, we retrospectively analyzed the clinical presentation of patients with VN and episodic vertigo, aiming to expand the atypical clinical features of VN. METHODS: This retrospective study enrolled 58 patients with VN. Among them, 11 patients with more than 3 rem…
