The transmission of infrasound within the human ear is not well understood. To investigate infrasound propagation through the middle and inner ear, velocities of the stapes and round window membrane were measured to very low frequencies (down to 0.9 Hz from 2000 Hz) in fresh cadaveric human specimens. Results from ear-canal sound stimulation responses show that below 200 Hz, the middle ear impedance is dominated by its stiffness term, limiting sound transmission to the inner ear. During air-conduction, normal ears have approximately equal volume velocities at the oval (stapes) and round window

BACKGROUND: There is a prevailing opinion that spinning sensations signify a peripheral vestibular pathology while non-spinning sensations are not of vestibular origin.
OBJECTIVES: 1) Characterize the subjective sensations reported by patients during caloric testing. 2) Assess if the sensation was correlated with the peak slow phase velocity (SPV).
METHODS: Retrospective chart review at a Canadian adult tertiary-quaternary care balance centre for patients undergoing diagnostic caloric testing between December 2014 and September 2015.
RESULTS: Of 163 patients included, 122 had normal caloric

Sensorineural hearing loss, ataxia, pyramidal signs, and vestibular deficits characterize superficial siderosis of the central nervous system. This study investigated changes in vestibular function, free radical formation, and phosphorylated cJun expression in the vestibular end organs after middle ear treatment with a ferric chloride (FeCl3) solution. A single injection of 70% FeCl3 solution into the unilateral middle ear cavity caused static vestibular symptoms, such as spontaneous nystagmus and head tilt. Asymmetric expression of c-Fos protein was observed in the bilateral vestibular nuclei

In many sensory systems, perception of stimuli is influenced by previous stimulus exposure such that subsequent stimuli may be perceived as more neutral. This phenomenon is known as an aftereffect and has been studied for vision, audition, and some vestibular stimuli including roll and translation. Previous data on yaw rotation perception has focused on low-frequency stimuli on the order of a minute which may not be directly applicable to frequencies during ambulation. The aim of the current study is to look at the influence of yaw rotation on subsequent perception near 1 Hz, the predominant f

BACKGROUND: A recent study investigated the effects of galvanic vestibular stimulation (GVS) on pusher behavior (PB) in post-stroke patients. However, there have been no reports about the effects of multisession GVS on PB.
OBJECTIVE: The purpose of this study was to investigate the feasibility and effects of multisession GVS combined with physical therapy for PB in stroke patients.
METHODS: Two stroke patients who showed PB were enrolled. The ABAB single-case design was used. Each phase lasted 1 wk. In phases A1 and A2, the patients underwent a 60-min-long physical therapy session 5 days a w

BACKGROUND: Inner ear evoked potentials are small amplitude (<1 μVpk) signals that require a low noise signal acquisition protocol for successful extraction; an existing such technique is Electrocochleography (ECOG). A novel variant of ECOG called Electrovestibulography (EVestG) is currently investigated by our group, which captures vestibular responses to a whole body tilt. The objective is to design and implement a bio-signal amplifier optimized for ECOG and EVestG, which will be superior in noise performance compared to low noise, general purpose devices available commercially.

Migraine-associated vertigo has become a well-recognized disease entity diagnosed based on a clinical history of recurrent vertigo attacks unexplained by other central or peripheral otologic abnormalities, which occurs in the patient with a history of migraine headaches. There is no international agreement on what spectrum of symptoms should be covered under this diagnosis, or what terminology should be used. The headaches and vestibular symptoms of migraine-associated vertigo may not be temporally associated, which often obscures the association. Diagnostic tests usually show nonspecific abno

This study investigated the localization and prevalence of hydrops formation in 20 patients with unilateral Ménière's disease using a battery of tests, including audiometry, caloric, ocular vestibular evoked myogenic potential (oVEMP) and cervical VEMP (cVEMP) tests. The latter two tests used air-conducted sound (ACS) and bone-conducted vibration (BCV) as stimuli. Thirteen patients (65%) had abnormal hearing with a 4-tone average >26 dBHL. In the caloric test, 4 patients (20%) had abnormal responses, including canal paresis in 3 and caloric areflexia in 1 patient. The percentages of patients w

Vestibular compensation, or neuronal plasticity in the central vestibular system, is quite an important process in patients with acute unilateral peripheral vestibular disease, allowing them to lead a comfortable daily life when medical treatments fail to cure the peripheral vestibular function. Is the residual unilateral vestibular input from damaged vestibular endo-organs a positive or negative factor for the development of dynamic vestibular compensation in the central nervous system? To elucidate the true mechanism of vestibular compensation, we examined the ENG findings and dizziness hand

One year after neuritis vestibularis, 29% from a sample of 75 patients still complained of vertigo. The objective of this investigation was to study why patients suffer from persisting vertigo. The alternative hypotheses were that the vertigo experienced could be explained either by a persisting vestibular dysfunction or by psychopathological changes. To elucidate this question, patients were examined with dynamic posturography, the symptom check list (SCL 90 R), the state-trait anxiety inventory (STAI), the agoraphobic cognitions questionnaire (ALQ) and the body sensations questionnaire (BSQ)

The current incidence of missile injury to the temporal bone (MITB) is very low in the United Kingdom. However, the increasing frequency of firearm violence in Britain suggests a greater risk of occurrence. This, along with the devastating potential sequelae of MITB (facial palsy, dead ear, intracranial damage, major vascular injury and cosmetic disfigurement), requires otolaryngologists to be conversant with all aspects of their management. The risk of major complications is much higher with MITB than with temporal bone injury following blunt trauma, and surgical management is, therefore, muc

Clinical imaging investigation of vertigo is actually mainly performed using MRI, which offers a detailed analysis of cochieo-vestibular nerve and labyrinth. Computed tomography is reserved to the analysis of bony structures. Several causes of vertigo are described, e.g. tumours, labyrinthitis, malformations, Ménière's disease, otoscierosis, trauma, cholesteatomas and postoperative changes. The interest of imaging is discussed. The necessity to perform paraclinical tests prior to diagnostic imaging to optimise the choice of imaging is emphasized.

We report a case of type I Waardenburg's syndrome that provides insight into the etiopathogenesis of sensorineural hearing loss (SNHL) in this syndrome. The subject, a 76-year-old woman with type I Waardenburg's syndrome (dystopia canthorum, heterochromia irides, and white hair), had congenital low-frequency SNHL in her right ear only, which had remained relatively stable throughout her life. Blood leukocyte DNA studies revealed a PAX-3 mutation with a 1 base pair C-to-A substitution in exon 5 at base 602. Light microscopic studies of the right cochlea showed intact neurosensory structures in

Certain forms of cellular activation lead to immediate early gene (IEGPs) transcription. The biochemical cascade involving IEGPs has been demonstrated to be an important part that cells response to environmental events. The article reviewed the feature of c-Fos expression in vestibular complexes under stimuli. It was considered that c-Fos expression might be a cue parameter in studying the connection of all vestibular complexes, and to reveal the mechanism of motion sickness.
