Influence of neuroactive drugs on corticosteroid feedback regulation of ACTH secretion in man.

Corticosteroid feedback effects on ACTH secretion in man can be manipulated by neuroactive drugs. In patients without endogenous corticosteroids (primary adrenocortical insufficiency) differential and integral feedback effects can be differentiated. When in these patients brain norepinephrine receptor activity was increased by desipramine, the normally negative differential feedback mechanism was converted into a positive one (paradoxical ACTH response). The pre-existing paradoxical ACTH response of patients with Cushing's disease after adrenalectomy was abolished after depletion of norepineph

Autoimmune polyendocrine syndrome type 2 in children: a case report and literature review.

BACKGROUND: Autoimmune polyendocrine syndrome (APS) is a clinical disorder characterized by the loss of immune tolerance, leading to dysfunction in multiple endocrine glands. According to the latest disease classification, APS is categorized into three main subtypes: APS-1, APS-2, and IPEX (Immune Dysregulation, Polyendocrinopathy, Enteropathy, X-linked) syndrome. APS-2 is defined by the presence of at least two autoimmune endocrine disorders, such as type 1 diabetes mellitus, autoimmune thyroiditis, or Addison's disease. APS-2 typically manifests later than APS-1, with onset most commonly occ

Autoimmune Diseases in Patients with Premature Ovarian Insufficiency-Our Current State of Knowledge.

Premature ovarian insufficiency (POI), previously known as premature ovarian failure or premature menopause, is defined as loss of ovarian function before the age of 40 years. The risk of POI before the age of 40 is 1%. Clinical symptoms develop as a result of estrogen deficiency and may include amenorrhea, oligomenorrhea, vasomotor instability (hot flushes, night sweats), sleep disturbances, vulvovaginal atrophy, altered urinary frequency, dyspareunia, low libido, and lack of energy. Most causes of POI remain undefined, however, it is estimated that anywhere from 4-30% of cases are autoimmune

Gastroparesis - a novel cause of persistent thyroid stimulating hormone elevation in hypothyroidism.

Hypothyroidism is easily treated by levothyroxine therapy which has an 80 percent absorption rate, mostly in the jejunum. The replacement dose of daily levothyroxine is usually calculated at 1.6 mcg/kg body weight per day. We report a 77-year-old man who required supraphysiologic thyroxine replacement (>2.7 mcg/ kg/day) to treat his hypothyroidism. The patient was referred for persistent thyroid stimulating hormone (TSH) elevation (40 mcIU/ml) while on 175 mcg of levothyroxine. Patient was compliant with medication. Medical history included diabetes mellitus type 2, cerebrovascular accident, d

New splice site acceptor mutation in AIRE gene in autoimmune polyendocrine syndrome type 1.

Autoimmune polyglandular syndrome type 1 (APS-1, OMIM 240300) is a rare autosomal recessive disorder, characterized by the presence of at least two of three major diseases: hypoparathyroidism, Addison's disease, and chronic mucocutaneous candidiasis. We aim to identify the molecular defects and investigate the clinical and mutational characteristics in an index case and other members of a consanguineous family. We identified a novel homozygous mutation in the splice site acceptor (SSA) of intron 5 (c.653-1G>A) in two siblings with different clinical outcomes of APS-1. Coding DNA sequencing rev

Serum concentration of IL-6, IL-2, TNF-α, and IFNγ in Vitiligo patients.

BACKGROUND: Vitiligo is an acquired depigmenting disorder characterized by the loss of functional melanocytes from the epidermis. Although the etiology of vitiligo is unknown, over the last few years, substantial data from clinical research has greatly supported the 'Autoimmune theory' and this is supported by the frequent association of vitiligo with disorders that have an autoimmune origin, including Hashimoto's thyroiditis, Graves disease, type 1 insulin-dependent diabetes mellitus, and Addison's disease. As cytokines are important mediators of immunity, there is evidence to suggest that th

Autoantibody profile and epitope mapping in latent autoimmune diabetes in adults.

The presence of islet cell autoantibodies in adult diabetic subjects who do not require insulin treatment for at least six months after the initial clinical diagnosis identifies the so-called latent autoimmune diabetes in the adult (LADA). Glutamic acid decarboxylase autoantibodies (GAD65Ab) are the best immune marker to identify LADA patients, while other islet autoantibodies, such as IA-2 autoantibodies, have a very low diagnostic sensitivity. Islet cell antibodies, as detected by indirect immunofluorescence, may improve the diagnostic specificity of the immune analysis when detected in GAD6

The autoimmune disease complex interstitial pneumonia/dermatomyositis in the light of endocrinology and cancer epidemiology.

The autoimmune disease interstitial pneumonia/dermatomyositis complex (ID) is classified as a paramalignancy since it is associated with a variety of malignancies at a high incidence in aged patients. We found a striking resemblance of pathophysiological features between ID and immunoendocrinopathy syndromes (IES) with depressed adrenocortical function. Drip infusion of megadose vitamin C, dehydroepiandrosterone and cortisol (an adrenocortical secretion substitute) was found to be effective for the control of ID. It has also been indicated that ID is a disease of adrenocortical insufficiency.

Perioperative Management of a Patient With Primary Adrenal Insufficiency Undergoing Spinal Cord Stimulation: A Case Report.

BACKGROUND: Spinal cord stimulation (SCS) is an established therapy for the management of chronic neuropathic low back pain (LBP) that is refractory to conventional measures. Although considered minimally invasive, SCS constitutes a surgical intervention that may induce significant physiological stress, placing patients with primary adrenal insufficiency (PAI) at an elevated risk of adrenal crises during perioperative periods. CASE REPORT: The authors present a woman with chronic LBP and PAI who successfully underwent SCS trial and permanent implantation with stress-dose glucocorticoid supple

Follow up of a rare case of adrenal insufficiency due to NNT mutation.

Hypoglycaemia is one of the most common causes of convulsions in neonatal period. Repeated hypoglycaemic convulsions have to be addressed with utmost urgency to prevent its morbid sequelae. Repeated ketotic hypoglycaemia in the infantile period needs detailed endocrine evaluation. Our patient is a boy in the third year of his life, had presented in infancy with hypoglycaemic convulsions and hyperpigmentation of skin and mucous membrane. Investigations revealed ketotic hypoglycaemia, hypocortisolaemia with high adrenocorticotropic hormone (ACTH) and normal aldosterone, 17-hydroxyprogesterone (1

Adrenal insufficiency after curative-intent gastric cancer treatment: a case report.

BACKGROUND: Adrenal insufficiency is a life-threatening condition, and advanced gastric cancer is associated with very poor prognosis. Adrenalectomy and/or metastatic invasion of the adrenal glands can cause primary adrenal insufficiency, which in turn can present with symptoms mimicking advanced cancer. CASE PRESENTATION: Herein we report of a 68-year-old White male with a history of left adrenalectomy in conjunction with distal gastrectomy due to gastric adenocarcinoma. At the 2-year follow-up visit after cancer surgery, the patient presented with fatigue, unintentional weight loss, hyperka

Tyrosine Kinase Inhibitors' Newly Reported Endocrine Side Effect: Pazopanib-Induced Primary Adrenal Insufficiency in a Patient With Metastatic Renal Cell Cancer.

Tyrosine kinase inhibitors (TKIs) have been used in the treatment of multiple types of cancer. Pazopanib is one of the TKIs and is considered a first-line treatment for adult patients with metastatic renal cell carcinoma. Many endocrine-related adverse effects have been noted with the use of TKIs including hypothyroidism, vitamin D deficiency, altered bone density, secondary hyperparathyroidism, abnormal glucose metabolism, gynecomastia, and hypogonadism. Subclinical glucocorticoid deficiency and adrenal insufficiency have been reported with the use of TKIs in only a few cases so far; thus, it
