Cerebral malaria.

Malaria remains of significant public health concern under the tropics, causing millions of deaths annually. The disease is caused by protozoans of the Plasmodium genus, of which harbors several distinct species. Human infection occurs during the blood meal of an infected female mosquito belonging to the Anopheles genus. It is estimated that around 1% of children infected with Plasmodium falciparum develops a more severe form of malaria, which may eventually lead to cerebral complications including cerebral malaria (CM). CM can be positively diagnosed in patients unable to localize a painful s

The safety of deep sedation with propofol controlled by the endoscopist in endoscopic retrograde cholangiopancreatography (ERCP): a prospective study in a tertiary hospital.

INTRODUCTION: propofol administered by an endoscopist with a trained nurse has evolved as an alternative to anesthesia monitoring and is increasingly common in the routine clinical practice, even in advanced endoscopy. OBJECTIVE: to evaluate the safety of deep sedation with endoscopist-controlled propofol in patients undergoing endoscopic retrograde cholangiopancreatography (ERCP). MATERIAL AND METHODS: this was a prospective study in patients undergoing ERCP under deep sedation with propofol. Different patient-related variables were included and the initial and final data on oxygen saturati

The Initial Development of a Checklist for Assessing Thirst in Patients With Advanced Dementia.

BACKGROUND: Patients with advanced dementia (PwAD) often have difficulty expressing their needs verbally because of cognitive impairments. PwAD thus often communicate these needs through behaviors and psychological symptoms. Understanding the behavioral characteristics of certain needs is critical for caregivers and clinicians when caring for PwAD. PURPOSE: The purpose of this study was to develop a checklist that may be used to assess the thirst status of PwAD. METHODS: A mixed method design was used. The checklist was developed using qualitative interviews and surveys. The initial items we

Increase in sharps injuries in surgical settings versus nonsurgical settings after passage of national needlestick legislation.

BACKGROUND: The operating room is a high-risk setting for occupational sharps injuries and bloodborne pathogen exposure. The requirement to provide safety-engineered devices, mandated by the Needlestick Safety and Prevention Act of 2000, has received scant attention in surgical settings. STUDY DESIGN: We analyzed percutaneous injury surveillance data from 87 hospitals in the United States from 1993 through 2006, comparing injury rates in surgical and nonsurgical settings before and after passage of the law. We identified devices and circumstances associated with injuries among surgical team m

Spiraling out of control: one case of pathologic anxiety as a response to a genetic risk of cancer.

The threat of cancer can result in an existential crisis characterized by feelings of uncertainty and fear. Anxiety, the most common response to the threat of cancer, may be expressed in ways as varied as individual personalities and circumstances. It is a normal response to the threat of cancer, but in some it may deteriorate to pathologic anxiety and manifest somatic or avoidant patterns. Members of families diagnosed with genetic mutations that predispose to cancer are unique in that they experience a complex chain of life events. People affected with genetic mutations that increase the ris

Designing for Care, Not Just Function: Lessons From a Non-Clinical Artificial Intelligence Innovation in Person-Centred Fundamental Care.

AIM: To examine how artificial intelligence (AI) influences the definition, enactment and preservation of person-centred fundamental care in healthcare contexts, and to develop a complementary framework that positions care as a design constraint. DESIGN: Theoretically informed case analysis. This approach uses a bounded case as a site for developing theoretical propositions rather than for inductive inference. METHODS: A non-clinical conversational AI companion for individuals affected by breast cancer was used as an empirical case to examine how care is shaped through technological systems.

Communication as a Clinical Intervention: The Role of the Oncology Hospitalist in Primary Palliative Care.

IntroductionHospitalized patients with advanced cancer frequently experience high symptom burden, prognostic uncertainty, and complex decision-making needs. Although specialty palliative care improves patient-centered outcomes, access remains limited in many inpatient settings. Oncology hospitalists may be uniquely positioned to deliver elements of primary palliative care during acute hospitalizations. Communication is a core clinical skill in hospital medicine, yet many oncology hospitalists feel underprepared for breaking bad news, discussing prognosis and leading goals-of-care conversations

Complications and emergency department presentations associated with multidisciplinary team management of patients on home enteral nutrition: A systematic literature review.

BACKGROUND/AIM: Patients receiving home enteral nutrition (HEN) via enteral feeding tubes may have a high risk of complications. International guidelines recommend multidisciplinary team (MDT) approaches to manage these complex nutrition patients. This review aimed to describe the composition of HEN MDTs and investigate the effect of outpatient MDT care in reducing HEN-related emergency department (ED) presentations and/or complications. METHODS: MEDLINE and CINAHL databases were searched between April 9-17th 2024. Screening, extraction and quality assessment (using the Academy of Nutrition a

Adolescent condom negotiation: a concept analysis and conceptual framework.

INTRODUCTION: Adolescents are at risk of sexually transmitted infections and unintended pregnancies. Consistent condom use is effective in curbing the double burden of diseases and unplanned conceptions. Condom negotiation influences consistent condom use. However, a clear operational definition is lacking. This paper analyzes the concept of adolescent condom negotiation and examines its defining attributes, antecedents, and consequences to promote conceptual clarity and enhance sexual health research and practice. METHODS: Norris's (1982) concept clarification method guided this analysis. Th

Caregiver perceptions of β-lactam allergy testing for serum sickness-like reactions.

Background: Beta-lactam allergy (BLA) labels are common in children but often disproven when formally evaluated. This diagnostic discrepancy leads to higher health-care costs and increased adverse outcomes. Beta-lactam antibiotics are associated with a variety of adverse reactions in children. A serum sickness-like reaction (SSLR) is one adverse reaction that is complex in its diagnosis and management due to its heterogeneous clinical presentation and poorly understood mechanism. This unclear presentation and diagnostic criteria can be confusing for patients, families, and health-care professi

Acceptability of supporting lay-carer administration of anticipatory subcutaneous medications at home: a qualitative study using the theoretical framework of acceptability.

BACKGROUND: Globally, evidence indicates that most people prefer to receive care and die at home, provided high-quality care is available. However, systemic and logistical challenges often prevent this outcome. Palliate is a nurse-led intervention aiming to address these barriers, supporting lay-carers in administering end-of-life subcutaneous medications to their loved ones, through training, written guidance, and documentation. AIM: To explore the perceptions and experiences of patients, carers, and healthcare professionals regarding the acceptability of the Palliate intervention, using the

AIM: To develop an eHealth resource to support fibromyalgia patients and explore it for usability and impact on their self-management and self-efficacy. BACKGROUND: Fibromyalgia is a complex, non-progressive chronic condition characterised by a bewildering array of symptoms for patients to self-manage. International guidelines recommend patients receive illness-specific information once diagnosed to promote self-management and improve health-related quality of life. DESIGN: A 3-phase mixed methods exploratory sequential design. METHODS: Qualitative interviews explored the information and se

Interdisciplinary clinicians' attitudes, challenges, and success strategies in providing care to transgender people: a qualitative descriptive study.

BACKGROUND: Access to clinicians competent in transgender health remains a significant barrier and contributor toward health inequity for transgender people. Studies on access and barriers to care have predominantly evaluated transgender patients' perceptions, but scant research has included the perspectives of clinicians. AIMS: We conducted a qualitative study to explore how clinicians (meaning physicians and advanced practice providers, in this paper) in the United States: (1) attain and utilize information, (2) perceive barriers and facilitators, and (3) understood gaps in their profession

Implementation and Postoperative Management of Continuous Adductor Canal Catheters for Total Knee Arthroplasty to Reduce Surgical Backlog Related to the COVID-19 Pandemic: An Acute Pain Service Nursing Perspective and Educational Resource.

PURPOSE: In response to the surgical backlog created by the COVID-19 pandemic and to spare valuable hospital resources, we developed and implemented a continuous adductor canal catheter (CACC) program for total knee arthroplasty (TKA) patients. CACC's offer superior analgesia, decrease opioid use, and increase patient satisfaction while simultaneously promoting a decreased length of hospital stay and even same day discharges. The implementation of analgesia protocols using continuous peripheral nerve catheters and isometric pumps has been described for other surgical procedures and populations

Not just another multi-professional course! Part 1. Rationale for a transformative curriculum.

Undergraduate inter- and multi-professional education has traditionally aimed to develop health professionals who are able to collaborate effectively in comprehensive healthcare delivery. The respective professions learn from and about each other through comparisons of roles, responsibilities, powers, duties and perspectives in order to promote integrated service. Described here is the educational rationale of a multi-professional course with a difference; one that injects value to undergraduate health professional education through the development of critical cross-field knowledge, skills and

Communicating the Geriatrician's Brand in Age-Friendly Care.

The implementation of CMS age-friendly care initiatives has created new opportunities for geriatricians to shape clinical care, quality improvement, education, and health system strategy. A geriatrician's unique contribution extends beyond managing medical complexity; it lies in applying expertise in aging physiology, functional change, psychosocial context, heterogeneity, prognosis, patient priorities, and ageism to clinical and organizational decision-making. Geriatrics expertise must be visible, clearly communicated, and connected to the people and structures that guide organizational chang
