[Alcoholic foetopathy: an update].

INTRODUCTION: In recent years a great deal of attention has been given to the role of prenatal exposure to alcohol in the production of a wide range of disorders known as foetal alcohol spectrum disorders (FASD). Foetal alcohol syndrome represents the classic syndrome and the most serious manifestation caused by the epigenetic changes induced by such exposure. It is considered to be the number one preventable cause of congenital defects and mental deficiency. AIM: To update the body of knowledge on this group of disorders by reviewing the most important aspects in terms of the epidemiology, d

Alcoholic cardiomyopathy: a review.

Alcohol abuse can cause cardiomyopathy indistinguishable from other types of dilated nonischemic cardiomyopathy. Most heavy drinkers remain asymptomatic in the earlier stages of disease progression, and many never develop the familiar clinical manifestations that typify heart failure. We review the current thinking on the pathophysiology, clinical characteristics, and treatments available for alcoholic cardiomyopathy. The relationship of alcohol to heart disease is complicated by the fact that in moderation, alcohol has been shown to afford a certain degree of protection against cardiovascular

Alcohol and gastrointestinal oncology.

Results from several large epidemiological studies have firmly established that alcohol is associated with elevated cancer incidence and mortality. Recently the International Agency for Cancer Research stated that acetaldehyde associated with alcoholic beverages is carcinogenic to humans and confirmed the Group 1 classification of alcohol consumption and of ethanol in alcoholic beverages. Alcohol consumption causes cancers of the oral cavity, pharynx, larynx, oesophagus, colorectum, liver, pancreas and female breast. The frequency of most alcohol-induced diseases increases in a linear fashion

Acamprosate for alcohol dependence.

BACKGROUND: Alcohol dependence is among the main leading health risk factors in most developed and developing countries. Therapeutic success of psychosocial programs for relapse prevention is moderate, but could potentially be increased by an adjuvant treatment with the glutamate antagonist acamprosate. OBJECTIVES: To determine the effectiveness and tolerability of acamprosate in comparison to placebo and other pharmacological agents. SEARCH STRATEGY: We searched the Cochrane Drugs and Alcohol Group (CDAG) Specialized Register, PubMed, EMBASE and CINAHL in January 2009 and inquired manufactu

Cardiodepressive effects of alcohol.

UNLABELLED: The toxic effect of alcohol on the heart was recognized long time ago. The aim of our study was to evaluate the cardiodepressive effect of alcohol in our in-patients and compare the obtained results with those in the literature. MATERIAL AND METHOD: The study included 187 patients (184 men and 3 women) aged 30 to 75 years and followed-up for 8 years. All patients underwent a series of biochemical and hematological test and paraclinical investigations. RESULTS: Our study demonstrates the toxic effects of alcohol on the contractile function of the myocardium, and the deterioration

An alcoholic presenting fits.

A known alcoholic was admitted for a repeated episode of fits abolished by treating the patient's hypocalcaemia. Several mechanisms of hypocalcaemia in alcoholics have been described but in this patient coeliac disease was also diagnosed. Non-compliance with a gluten-free diet led to poor control of the disease, continued hypocalcaemia and an admission with a repeat seizure leading to death.

Alcohol and liver fibrosis.

Alcoholic liver disease involves significant crosstalk among intracellular signaling events in the liver. Overall, inflammatory and innate immune responses in Kupffer cells due to elevated gut-derived plasma endotoxin levels, increased reactive oxygen species-induced damage, and profibrogenic factors such as acetaldehyde or lipid peroxidation products contribute to activation of hepatic stellate cells, the key cell type involved in liver fibrosis. Using in vitro and in vivo approaches, there has been great progress in our understanding of the mechanisms leading to liver fibrosis: potential bio

[Alcoholic liver disease].

A study conducted 15-year ago showed that only 13.5% of chronic alcoholics developed alcohol-induced liver damage, which misled some people to believe a lack of relationship between the amount of alcohol and the occurrence of liver disease. However, it is true that a significant correlation exists between per capita consumption and the prevalence of cirrhosis. Alcoholic fatty liver is observed in most of chronic alcoholics even though the severity is not uniform. Abstinence remains the cornerstone of therapy for alcoholic liver disease (ALD). There is also consensus for the use of corticostero…

Alcohol-induced blackout.

For a long time, alcohol was thought to exert a general depressant effect on the central nervous system (CNS). However, currently the consensus is that specific regions of the brain are selectively vulnerable to the acute effects of alcohol. An alcohol-induced blackout is the classic example; the subject is temporarily unable to form new long-term memories while relatively maintaining other skills such as talking or even driving. A recent study showed that alcohol can cause retrograde memory impairment, that is, blackouts due to retrieval impairments as well as those due to deficits in encodin

Treatment of alcohol dependence.

It is important to manage alcohol withdrawal properly. Mismanagement could lead to permanent brain damage or even death. The principles are simple and easily learnt. New pharmacological agents have come into use which helps in maintaining abstinence. Clinicians should update themselves on their use.

A dilated alcoholic cardiomyopathy.

Dilated forms of cardiomyopathy are characterized by ventricular chamber enlargement and systolic dysfunction with normal LV wall thickness. Among other causes, chronic excessive consumption of alcohol has a very important presence. We present a case of a dilated alcoholic cardiomyopathy in a 65-year-old man. Also this case focuses attention on dilated alcoholic cardiomyopathy.

Alcohol and cardiovascular health.

The substantial medical risks of heavy alcohol drinking as well as the probable existence of a less harmful or safe drinking limit have been evident for centuries. Modern epidemiology studies suggest lowered risk of morbidity and mortality among lighter drinkers. Thus, defining "heavy" drinking as > or =3 standard drinks per day, the alcohol-mortality relationship is a J-curve with risk highest for heavy drinkers, lowest for light drinkers and intermediate for abstainers. A number of non-cardiovascular and cardiovascular problems contribute to the increased mortality risk of heavier drinkers.

The cost of alcohol in California.

BACKGROUND: California is the largest alcohol market in the United States. In 2005 alone, Californians consumed almost 14 billion alcoholic drinks, which contributed to many severe and potentially fatal alcohol-related illnesses and conditions. Alcohol use also causes violent and nonviolent crimes, as well as injuries and traffic collisions. While several studies have estimated the magnitude and cost of these problems nationally and others have analyzed underage drinking costs, no overall cost estimate at the state level currently exists for California. We present the first comprehensive estim

Acute alcohol intoxication.

Acute alcohol intoxication is a clinically harmful condition that usually follows the ingestion of a large amount of alcohol. Clinical manifestations are heterogeneous and involve different organs and apparatuses, with behavioral, cardiac, gastrointestinal, pulmonary, neurological, and metabolic effects. The management of an intoxicated patient occurs mainly in the emergency department and is aimed at stabilizing the clinical condition of the patient, depending on his/her clinical presentation. One specific drug that is useful in the treatment of acute alcohol intoxication is metadoxine, which

How irritant is alcohol?

BACKGROUND: Alcohol-based hand rubs are used worldwide to prevent transmission of nosocomial pathogens. OBJECTIVES: To investigate skin irritation caused by alcohols alone and in combination with detergent washing. METHODS: Single and repetitive patch testing with 60-100% alcohols [ethanol, 1-propanol, 2-propanol (synonyms: isopropyl alcohol, isopropanol)], a positive control [0.5% sodium lauryl sulphate (SLS)] and negative controls (empty chamber and water) were performed. Wash tests were performed with 80% ethanol and 0.5% SLS on the forearms with each agent alone and with both agents in a

Acute alcoholic hepatitis.

Acute alcoholic hepatitis (AAH) is a frequent inflammatory liver disease with high short-term mortality rate. In this review, relationships between alcohol abuse and the epidemiology and the outcomes of AAH are discussed, as well as AAH pathogenesis. The role of endotoxins, tumor necrosis factor alpha, fibroblasts, and immune response to altered hepatocyte proteins is discussed. The need of a careful prognosis, supported by the use of Maddrey score, by the model for end-stage liver disease [Mayo end-stage liver disease (MELD)] score or by the Glasgow alcoholic hepatitis score, is outlined, as

Treatment of alcoholic hepatitis.

Cirrhosis and its sequelae are responsible for close to 2% of all causes of death in the United States. Some studies have suggested that the costs of liver disease may account for as much as 1% of all health care spending, with alcohol-related liver disease (ALD) representing a major portion. It accounts for between 40% to 50% of all deaths due to cirrhosis, with an accompanying rate of progression of up to 60% in patients with pure alcoholic fatty liver over 10 years, and a 5-year survival rate as low as 35% if patients continue to drink. A subset of patients with ALD will develop an acute, v
