Perioperative noninvasive cardiac output monitoring in parturients with singleton and twin pregnancies undergoing cesarean section under spinal anesthesia with prophylactic phenylephrine drip: a prospective observational cohort study.

Purpose: Spinal anesthesia is considered the gold standard anesthetic technique for cesarean deliveries (CDs) but is associated with a high rate of hypotension. The recent international consensus recommends continuous prophylactic phenylephrine infusion (PPI) administered throughout CD to prevent hypotension. However, little information is available on the hemodynamic profiles of women with twin pregnancies as compared to singleton pregnancies perioperatively. Therefore, in this study, we aim to compare maternal hemodynamic changes both intraoperatively and postoperatively with the use of the

10-degree reverse Trendelenburg position on hemodynamic parameters and block characteristics in unilateral spinal anesthesia in below knee orthopedic surgeries - can head up position do the trick?

Unilateral spinal anesthesia (USpA) is a technique used to restrict the effect of the spinal block on the operative side. 10-15 degrees reverse Trendelenburg position has been used to control the height of the spinal block using hyperbaric drugs. We aimed to study the effect of the 10-degree reverse Trendelenburg position on the quality of block and hemodynamic stability in unilateral spinal anesthesia in this hospital-based, double-blind, randomized clinical trial. 60 patients of both sexes between 20-60 years of age, undergoing below-knee orthopedic surgeries, were randomized into 2 groups.

[Case of bladder perforation due to the obturator nerve reflex during transurethral resection (TUR) of bladder tumor using the TUR in saline (Turis) system under spinal anesthesia].

Bladder perforation due to the obturator nerve reflex (ONR) is a serious complication during TUR of bladder tumor using the conventional TUR system; requiring monopolar electrocautery and non-conductive solution as perfusate. Recently, the TURis system, which employs bipolar electrocautery and physiological saline as perfusate, has been developed. Electrical resistance of physiological saline and human tissues are approximately 40 and 500 omega, respectively. Thus, theoretically, electrical current flows between the resection loop and the recovery electrode integrated in the outer sleeve of th

Comparison of fractionated versus bolus administration of different doses of hyperbaric bupivacaine combined with opioids in spinal anesthesia for caesarean section - a prospective, randomized, participant-blind clinical trial.

BACKGROUND: Spinal anesthesia with hyperbaric bupivacaine and neuraxial opioids is the preferred technique for cesarean section, but maternal hypotension remains a common complication. Fractionated administration of local anesthetic may provide hemodynamic stability while maintaining effective anesthesia. This study aimed to compare the efficacy and safety of fractionated versus bolus administration of different doses of hyperbaric bupivacaine combined with opioids in spinal anesthesia for cesarean section. METHODS: This prospective, randomized, participant-blind clinical trial included 99 pre

Perfusion Index and Its Correlation With Intraoperative Hypotension in Lower-Segment Cesarean Section Under Spinal Anesthesia: A Prospective Observational Study in a Tertiary Care Hospital in Eastern India.

Background Hypotension is commonly encountered in patients undergoing lower-segment cesarean section (LSCS) under the subarachnoid block (SAB) owing to decreased vascular resistance caused by the sympathetic blockade and decreased cardiac output because of blood pooling in blocked areas of the body. Perfusion index (PI) is a good indicator of systemic vascular resistance and can foretell hypotension. This study aimed to associate baseline PI with intraoperative hypotension after SAB in LSCS. Methodology This was a prospective observational study with a sample size of 50. The baseline PI was re

A prospective randomized study to evaluate the effect of epidural volume extension with normal saline in combined spinal epidural anesthesia for lower limb surgeries using intrathecal hyperbaric levobupivacaine.

BACKGROUND: Combined spinal epidural anesthesia (CSEA) offers rapid onset and effective postoperative analgesia for lower limb surgeries. Epidural volume extension (EVE), achieved by epidural saline injection following intrathecal administration, influences the cephalad spread of local anesthetic. This study examined the effect of EVE with normal saline on sensory and motor block characteristics following intrathecal hyperbaric levobupivacaine. METHODS: A total of 100 American Society of Anesthesiologists I-II patients aged 18-60 years were assigned to two groups. Group A received 2.5 ml of 0

Comparison of Ultrasound-Guided Femoral Nerve Block and Intravenous Fentanyl For Analgesia During Positioning of Patients With Femur Fracture for Combined Spinal-Epidural Anesthesia: A Randomized Controlled Study.

Background Central neuraxial blockade is the preferred anesthesia technique for femur fracture surgeries. However, positioning patients for neuraxial anesthesia can be challenging due to severe pain. Analgesic strategies, including nonsteroidal anti-inflammatory drugs (NSAIDs), opioids, non-opioid analgesics, and nerve blocks like lumbar plexus, fascia iliaca, three-in-one, and femoral nerve blocks (FNBs), are used to ease positioning. This study compared the analgesic efficacy of intravenous (IV) fentanyl and ultrasound-guided FNB using visual analogue scale (VAS) scores in patients with femu

A randomized comparative study to prevent supine hypotension syndrome in pregnant females undergoing LSCS after giving spinal anesthesia using a wedge and novel 3D printed uterine displacement device.

INTRODUCTION: Pregnancy induces physiological changes, including alterations in cardiovascular dynamics, predisposing pregnant women to supine hypotension syndrome (SHS) during lower-segment cesarean section (LSCS) under spinal anesthesia. Various methods, including manual displacement of the uterus and use of wedges or cushions, have been proposed to prevent SHS, but their effectiveness remains variable. This study aimed to compare the efficacy of a novel 3D-printed uterine displacement device with that of a traditional wedge in preventing SHS during LSCS after spinal anesthesia. METHODOLOGY

Comparison of oral 5-aminolevulinic acid-induced hypotension between general and spinal anesthesia during photodynamic diagnosis-assisted transurethral resection of bladder tumor for non-muscle invasive bladder cancer.

BACKGROUND: Transurethral resection of bladder tumor (TURBT) using photodynamic diagnosis (PDD) with oral 5-aminolevulinic acid hydrochloride (5-ALA) with non-muscle-invasive bladder cancer (NMIBC) frequently results in hypotension being reported as a common serious side effect. Previous papers have reported that oral 5-ALA-induced hypotension is associated with both general and spinal anesthesia However, no study has analyzed oral 5-ALA-induced hypotension by separately evaluating each anesthesia type. METHODS: We retrospectively compared the systolic blood pressure (SBP) values with the fre
