Combined double-needle flanged-haptic intrascleral fixation of an intraocular lens and Descemet-stripping endothelial keratoplasty.

A slight modification to the Yamane transconjunctival double-needle flanged-haptic technique of intrascleral fixation of an intraocular lens … of the IOL position at all times. This combined technique can be a viable option for patients with aphakia and endothelial cell dysfunction.

Transscleral fixation of a toric intraocular lens to correct aphakic keratoplasty with high astigmatism.

A 71-year-old man with penetrating keratoplasty (PKP) and aphakia in the right eye and a preoperative refraction of +20.0 -11.0 x 38 and logMAR visual acuity of 20/60 presented to our hospital. The capsular support was absent because of previous complicated intracapsular cataract extraction. The implanted IOL was a custom-made Rayner 570T with +20.5 diopters (D) sphere and +11.0 D cylinder. The IOL's special haptics allowed it to be safely fixated in the sagittal plane. The postoperative refraction was +1.0 -2.0 x 5 with logMAR acuity of 20/60. Follow-up of 2 years revealed no IOL decentration…

Surgical technique for transscleral fixation of a foldable posterior chamber intraocular lens.

The safety and efficacy of a modified technique for transscleral fixation of a foldable posterior chamber intraocular lens (IOL) for intracapsular aphakia (secondary IOL) or after complicated phacoemulsification was evaluated. All eyes had inadequate or no posterior capsular support. Follow-up was between 4 and 26 months (mean, 12 months). Uncorrected visual acuity improved (> 2 lines) in all patients. During follow-up, the IOL was correctly positioned in all cases. There were no major complications, such as endophthalmitis or suture erosion through the conjunctiva at the site of fixation. No …

Pitx3 is required for motor activity and for survival of a subset of midbrain dopaminergic neurons.

Mesencephalic dopaminergic (MesDA) neurons play crucial roles in motor and behavioral processes; their loss in Parkinson's disease (PD) results in striatal dopamine (DA) deficiency and hypokinetic movement disorder. The Pitx3 homeobox gene is expressed in the MesDA system. We now show that only a subset of MesDA neurons express Pitx3 and that in Pitx3-deficient aphakia mice, this subset is progressively lost by apoptosis during fetal (substantia nigra, SN) and postnatal (ventral tegmental area) development, resulting in very low striatal DA and akinesia. Similar to human PD, dorsal SN neurons …

Foldable posterior chamber intraocular lens implantation in the absence of capsular and zonular support.

PURPOSE: To implant foldable posterior chamber intraocular lenses in the absence of capsular and zonular support. METHODS: Case reports. In two patients, two eyes with aphakia, lack of capsular or zonular support, and contact lens intolerance underwent the implantation of a silicone and an acrylic foldable posterior chamber intraocular lens, respectively. RESULTS: In the two eyes, final visual acuity was 20/25 and 20/50, respectively. No intraoperative vitreal, retinal, or choroidal complications were noted. The postoperative recovery was rapid, and there was minimal induced astigmatism. CO…

Harold Ridley and the invention of the intraocular lens.

On November 29, 1949, Harold Ridley implanted the first intraocular lens (IOL). This marked the beginning of a major change in the practice of ophthalmology and, for Ridley, the beginning of an era of inspiration, reward and challenge, unfortunately marred by the disdain and discourteous actions of many colleagues within the academic establishment in Europe and the United States. By the late 1970s IOLs and implantation procedures had undergone many improvements, and Ridley's invention had become an accepted option for the optical correction of aphakia. Since that time Ridley has been accorded …

Miotics and retinal detachment: upgrading the community standard.

The majority of ophthalmologists who responded to a questionnaire regarding the relationship between miotics and retinal detachment felt that such a relationship does exist, and that myopia and aphakia/pseudophakia predispose to the formation of new retinal breaks or to retinal detachment from pre-existing breaks with miotics. Horseshoe breaks and dialyses are pre-existing lesions that should be treated prophylactically prior to miotic therapy. Patients with no predisposing pathology or whose eyes have lattice degeneration or operculated breaks should be warned of possible retinal detachment p…

Correcting low vision in aphakic children.

There are many children who have subnormal vision in spite of successful cataract surgery and aphakic spectacle lenses. There are children who have lens opacities for whom the state of aphakia would not be an improvement. Careful evaluation of functional vision, careful refraction, and the prescription of high adds or other optical devices are part of complete ophthalmological care for every one of these children. The dioptric strength of the reading add is determined by the functional reading vision and the calculation of the reciprocal of the visual acuity. It is also important to realize th…

Purpose This study aimed to analyze temporal changes in the number of surgeries performed for intraocular lens (IOL) dislocation/decentration and aphakia and to examine the associated patient and ocular background factors across three institutions. Methods We conducted a retrospective review of all cataract-related procedures performed between January 1, 2008, and December 31, 2014, at Nagoya University Hospital, Sugita Eye Hospital, and Miyake Eye Hospital. We quantified (1) the annual number of standard cataract surgeries (routine lens extraction with IOL implantation), (2) the annual number…

Results following secondary intraocular lens implantation in children.

BACKGROUND: Placement of a secondary intraocular lens (IOL) in a child may be considered in children with congenital monocular cataracts who have had complete opacities removed early in life and who later become contact lens intolerant, in eyes that have received trauma which precluded placement of a primary IOL, and in young adults who have bilateral aphakia who become resistant to use of contact lenses or spectacles.
