Wide QRS tachycardia due to AV nodal reentry and a "bystander" bypass tract with slow conduction properties.

An unusual mechanism for recurrent, wide QRS complex supraventricular tachycardia is described in this report. A 25-year-old man with normal PR and QRS intervals during sinus rhythm was shown to have preexcitation with a left bundle branch block pattern during tachycardia and during atrial pacing. Electrophysiologic studies demonstrated slow and decremental conduction properties in an accessory "bystander" AV pathway utilized for antegrade conduction during AV nodal reciprocating tachycardia. The differential diagnosis of this tachycardia is discussed in detail.

Echocardiographic diagnosis and intravenous digoxin management of fetal tachyarrhythmias and congestive heart failure.

Fetal echocardiography now affords an accurate clinical diagnosis of nonimmune fetal hydrops secondary to fetal tachyarrhythmias and/or certain types of congenital heart disease. Individual case reports of the treatment of tachyarrhythmias have been reported that use various drugs, including digoxin, propranolol hydrochloride, procainamide hydrochloride, and digoxin plus verapamil hydrochloride. We found no report of intrauterine treatment of congestive heart failure due to congenital heart disease with sinus rhythm. The seven cases presented herein include five cases of isolated supraventricu

Heart rate at onset and termination of paroxysmal atrial fibrillation in apparently healthy subjects.

Eighteen paroxysms were documented on Holter electrocardiogram both at the onset and the termination of paroxysmal atrial fibrillation in 14 apparently healthy subjects. There were large inter- and intraindividual variations and no definite trend was observed in the time and duration of paroxysms. Heart rate at the onset of paroxysms was consistently higher than that at sinus rhythm immediately before atrial fibrillation started. There were 2 patterns in the sequence of heart rate during paroxysms. One was an almost constant heart rate during paroxysms, and the other was a trend toward a decre

Surgery for Wolff-Parkinson-White syndrome: further experience with an epicardial approach.

We have previously reported the use of an epicardial approach for ablation of left ventricular free wall accessory atrioventricular pathways. The technique involves mobilization of the atrioventricular fat pad and exposure and cryoablation of the atrioventricular junction at the site of the accessory pathway on the normothermic beating heart. Here we describe our further experience with left ventricular free wall accessory pathways and right ventricular free wall accessory pathways. Our experience is based on 53 consecutive patients. There were 35 male and 18 female subjects, 6 to 52 (mean 41.

Paroxysmal vagally mediated AV block with recurrent syncope.

Paroxysmal complete atrioventricular (AV) block without associated electrocardiographic (ECG) abnormality is not a well recognized entity. A mother and her daughter had recurrent syncopal episodes, but a normal ECG. The episodes were preceded by nausea and vomiting. ECG during these episodes revealed complete heart block. In the mother, one episode was promptly reversed by atropine. Electrophysiological evaluation of the sinus and AV nodal function and atrial and ventricular effective refractory periods before and after autonomic blockade was normal. Provocative manoeuvres failed to induce AV

Catheter-mediated electrical ablation of the posterior septum via the coronary sinus: electrophysiologic and histologic observations in dogs.

In a series of 12 dogs, the electrophysiologic and histologic effects of a single damped sine-wave shock delivered via standard electrocatheters to the region of the coronary sinus orifice were investigated. Six dogs received 200 J and six received 360 J of stored energy. The shock was delivered to two consecutive proximal poles of a standard quadripolar catheter positioned at the coronary sinus orifice and connected to the positive output (anode) of a defibrillator. A disc electrode positioned on the anterior chest wall served as the cathode (negative pole). During the shock, voltage and curr

Selective blockade of retrograde fast pathway by intravenous disopyramide in paroxysmal supraventricular tachycardia mediated by dual atrioventricular nodal pathways.

Electrophysiological effects of 2 to 2.5 mg/kg iv disopyramide were studied in 10 patients with dual nodal pathways who used a slow pathway for anterograde and a fast pathway for retrograde conduction during paroxysmal supraventricular tachycardia (mean cycle length 308.5 +/- 37 ms; range 260-370 ms). Disopyramide terminated the tachycardia in six cases by production of ventriculoatrial block in five and by sinus overdrive in one. In the remaining four patients cycle length of the paroxysmal supraventricular tachycardia increased significantly from 270 +/- 8 ms to 377.5 +/- 28 ms. In all 10 pa

Symptomatic sinus node dysfunction in children without structural heart disease.

Symptomatic sinus node dysfunction rarely occurs in children with structurally normal hearts. Three children with syncope and bradycardia are described. Noninvasive and invasive testing revealed isolated sinus node dysfunction and normal cardiac anatomy. Electrophysiologic evaluation showed abnormalities of sinus node automaticity and sinoatrial conduction. All three patients were treated with permanent ventricular pacemakers and have remained asymptomatic. Sinus node dysfunction should be considered in the differential diagnosis whenever a child is seen with unexplained dizziness or syncope.

[Role of accessory atrioventricular pathways in the genesis of permanent junctional tachycardia. Apropos of 2 cases].

Electrophysiological studies were performed in two patients with permanent junctional tachycardias. In both cases, the heart rate was 160/mn and standard ECG showed negative P waves in leads D2, D3 and AVF, isobiphasic or negative P waves in the left precordial leads and a PR interval of 0.12 and 0.13 secs. respectively. The tachycardia started without lengthening of the PR interval or preceding extrasystoles. Shortening of the sinus cycle before the tachycardia was inconstant. The reciprocating mechanism was shown by stimulation studies. Premature ventricular depolarisations not conducted to

Atrial reentry in chronic repetitive supraventricular tachycardia.

Atrial reentrance as a mechanism of the tachycardia was demonstrated in a 28-year-old patient suffering from chronic repetitive supraventricular tachycardia. Criteria for diagnosis included the following: (1) Repetitive supraventricular tachycardia was induced and terminated by properly timed atrial extrastimuli. (2) Return cycles of all atrial extrastimuli not abolishing the tachycardia were fully compensatory. (3) A-H prolongation was not a prerequisite to induce the tachycardia. (4) The contours of P and A waves during tachycardia differed from those in sinus rhythm, but atrial activation r

[Wolff-Parkinson-White syndrome: two accessory pathways concealed conduction accompanied by enhanced AV node conduction (author's transl)].

The case report of a 40 years old female patient with Wolff-Parkinson-White syndrome and concealed conduction of the accessory pathways is presented. Endocardial mapping revealed two re-entry tachycardias with different rates and different sequences of retrograde atrial depolarisation. These accessory bypass tracts were accompanied by enhanced AV node conduction. -- Patients with multiple accessory pathways and enhanced AV node conduction appear to be a special group of WPW-syndrome in respect of life-threatening tachycardias.

Electrophysiologic observations in a patient with bradycardia-dependent atrioventricular block.

In a patient with atrioventricular (A-V) block distal to the His bundle (H), 1:1 A-V conduction with right bundle branch block and H-V interval of 70 msec was established with atrial pacing at rates of 120 to 150/min, suggesting that the A-V block was bradycardia-dependent. Advanced second degree A-V block distal to the H deflection occurred with atrial pacing at 160/min after completion of A-V nodal Wenckebach periodicity proximal to the H deflection because of the long H-H encompassing the blocked P wave. Atrial extrastimulus testing coupled with sinus rhythm (with A-V block) demonstrated th

Evidence suggesting dual A-V nodal pathways in patients without supraventricular tachycardias.

Electrophysiologic evidence for dual pathways of conduction through the A-V node is presented in three patients without history of supraventricular tachycardia. In case 1, abrupt spontaneous changes in the PR interval from 0.17 to 0.42 second were seen. His bundle electrographic studies showed two sets of A-H intervals during sinus rhythm and at several atrial pacing rates, although at rates over 100 per minute only the slow pathway conducted. Using the extrastimulus method, different refractory periods for the fast and slow pathways were documented. Cases 2 and 3 underwent His bundle electrog

Verapamil and the heart: Pharmacological and Therapeutic Considerations.

Verapamil is a new anti-anginal and anti-arrhythmic drug whose pharmacological actions can be related to its specific blocking effect on the transmembrane transport of calcium ions. Intravenous verapamil slows the ventricular rate in atrial fibrillation and flutter with an occasional patient reverting to sinus rhythm. The drug promptly terminates most cases of paroxysmal supraventricular tachycardias. It is likely to become the initial agent of choice in the acute treatment of this arrhythmia. Verapamil is not effective in ventricular tachycardia or in atrial fibrillation complicating the WPW
