Treatment of fear of flying: behavioral, subjective, and cardiovascular effects.

BACKGROUND: Nonaviator fear of flying is a common problem usually managed with behaviorally oriented treatment. The unknown time courses of aspects of anxiety and of physiological response were investigated during a 3-d treatment including flights. METHODS: Ratings, heart rate (HR), and heart rate variability (HRV; pNN10, pNN50) of 15 Moderate-Anxious and 9 High-Anxious subjects at critical epochs of treatment and on 2 actual flights, on which 9 Controls participated, were compared. RESULTS: All subjects took the flights and displayed a remarkably reduced fear when comparing pre- vs. post-tr

An unusual case of ventricular ectopy in a military pilot.

BACKGROUND: Congenital left ventricular diverticula are an uncommon cardiac malformation. Although they may be asymptomatic, their presence is important to note as they are associated with systemic embolization of mural thrombus, valvular regurgitation, heart failure, focal ventricular arrhythmias, and sudden death. CASE REPORT: We describe a case of an asymptomatic pilot with a significant burden of ventricular ectopy. The diagnosis of a large left ventricular diverticulum was made by cardiac CT and confirmed by cardiac magnetic resonance imaging. No specific treatment was warranted; however

A retrospective study of acute mountain sickness on Mt. Kilimanjaro using trekking company data.

BACKGROUND: High altitude illnesses (HAI) are a risk factor for any individual who is exposed to a significant increase in altitude. To learn more about the epidemiology of HAI, we sought to determine if health records from a commercial trekking company could provide novel data on the prevalence of HAI, as well as efficacy data regarding common HAI therapeutics. METHODS: Health parameters from 917 tourists ascending Mt. Kilimanjaro over a 10-yr period were analyzed for meaningful data. RESULTS: Of all subjects, 70% experienced at least one instance of a symptom related to HAI (headache, naus

Acceleration endurance with pressure breathing during G with and without a counterpressure vest.

INTRODUCTION: The purpose of this study was to test whether pressure breathing during G (PBC) without a counterpressure vest negatively influences G endurance or increases breathing fatigue during extended duration high-G exposures. METHODS: While using PBG, 10 subjects underwent 2 trials of +3 Gz exposures: once when wearing a counterpressure vest and once without. The exposures consisted of a relaxed, gradual G onset run until peripheral or central light loss, a straining rapid onset GC run to +6 Gz for 15 s, and a simulated aerial combat maneuver (SACM) G profile consisting of 10-s periods

Electrogastrographic and autonomic responses during oculovestibular recoupling in flight simulation.

INTRODUCTION: Simulator sickness causes vestibulo-autonomic responses that increase sympathetic activity and decrease parasympathetic activity. The purpose of the study was to quantify these responses through electrogastrography and cardiac interbeat intervals during flight simulation. METHODS: There were 29 subjects that were randomly assigned to 2 parallel arms: (1) oculovestibular recoupling, where galvanic vestibular stimulation was synchronous with the visual field; and (2) control. Electrogastrography and interbeat interval data were collected during baseline, simulation, and post-simul

Motion sickness: an evolutionary and genetic basis for the negative reinforcement model.

It has been theorized that motion sickness evolved as a negative reinforcement system which terminates motion involving postural instability and/or sensory conflict. A hypothetical example is provided by a "thought experiment" whereby protohominids are in a tree looking for food. Selection pressure results when the organisms that have an aversion to motion-producing sensory conflict do not venture out too far on the tree limbs and therefore tend to survive. In order to support an evolutionary model for motion sickness there must be evidence for genetic and/or heritable predisposition. The pres

Women's health in spaceflight.

OBJECTIVE: To review the current state of knowledge with regards to clinical challenges related to women's health during spaceflight. METHODS: Articles were reviewed relevant to "women", "sex," and "gender" in "microgravity," "weightlessness," and "spaceflight" in the English and Russian languages. RESULTS: There were 50 papers identified. Studies have shown that crewmembers suffer from space motion sickness, but gender discrepancies have not been explored. Nearly all women experience orthostatic intolerance in space, which may be due to differences in female cardiovascular response. Immunos

Yawning as a behavioral marker of mild motion sickness and sopite syndrome.

INTRODUCTION: Severe motion sickness is easily identifiable with sufferers showing obvious behavioral signs, including emesis (vomiting). Mild motion sickness and sopite syndrome lack such clear and objective behavioral markers. We postulate that yawning may have the potential to be used in operational settings as such a marker. This study assesses the utility of yawning as a behavioral marker for the identification of soporific effects by investigating the association between yawning and mild motion sickness/sopite syndrome in a controlled environment. METHODS: Using a randomized motion-coun

Tolerance of centrifuge-simulated suborbital spaceflight by medical condition.

INTRODUCTION: We examined responses of volunteers with known medical disease to G forces in a centrifuge to evaluate how potential commercial spaceflight participants (SFPs) might tolerate the forces of spaceflight despite significant medical history. METHODS: Volunteers were recruited based upon suitability for each of five disease categories (hypertension, cardiovascular disease, diabetes, lung disease, back or neck problems) or a control group. Subjects underwent seven centrifuge runs over 2 d. Day 1 consisted of two +G(z) runs (peak = +3.5 G(z), Run 2) and two +G(x), runs (peak = +6.0 G(x

Subject anxiety and psychological considerations for centrifuge-simulated suborbital spaceflight.

INTRODUCTION: Anxiety and psychological concerns may pose a challenge to future commercial spaceflight. To help identify potential measures of anxiousness and indicators of flight-related stress, the psychiatric histories and anxiousness responses of volunteers exposed to G forces in centrifuge-simulated spaceflight acceleration profiles were examined. METHODS: Over 2 d, 86 individuals (63 men, 23 women), 20-78 yr old, underwent up to 7 centrifuge runs. Day 1 consisted of two +G(z) runs (peak = +3.5 G(z)) and two +Gx runs (peak = +6.0 G(x)). Day 2 consisted of three runs approximating suborbi

Cardiac indexes in young subjects with and without bicuspid aortic valve.

OBJECTIVES: Bicuspid aortic valve (BAV) is a common congenital anomaly. The aeromedical implications of this condition are unclear. This study attempts to evaluate a possible association between BAV with or without aortic regurgitation (AR) and cardiac and aortic morphology in young healthy subjects. METHODS: Air force academy applicants undergo routine echocardiography as part of the screening process. All echocardiographic examinations performed in the years 2004-2011 were evaluated. Applicants in whom BAV was identified were divided into those with and without aortic regurgitation. Both gr
