Flexion Dislocation After Limb Lengthening: Correction With Distal Femoral Osteotomy, Quadriceps Release, and Physeal-Sparing Medial Patellofemoral Ligament (MPFL) Reconstruction.

BACKGROUND: Patellar instability is a common clinical condition in skeletally immature individuals. Surgical treatment is considered when risk of recurrence is high. INDICATIONS: Distal femoral osteotomy is indicated in the setting of obligate flexion dislocation where femoral valgus contributes to a shortened lateral column, with concurrent quadriceps procedures considered for chronic contracture and medial patellofemoral ligament (MPFL) reconstruction for added stabilization. TECHNIQUE DESCRIPTION: This procedure is performed in a stepwise manner as some components may not be necessary bas

Biomechanical Comparison of Fixed-Loop and Adjustable-Loop Cortical Suspensory Devices for Metaphyseal Femoral-Sided Soft Tissue Graft Fixation in Anatomic Anterior Cruciate Ligament Reconstruction Using a Porcine Model.

PURPOSE: To compare the displacement, stiffness, and ultimate failure load of a fixed-loop cortical suspensory device with 2 adjustable-loop devices when positioned on metaphyseal bone. METHODS: Thirty devices (10 of each device) were positioned on the metaphyseal cortex of 30 porcine femora simulating anatomic anterior cruciate ligament femoral tunnel placement. Bovine tendons were used for soft tissue grafts, and the constructs were then cycled 1,000 times and pulled to failure, measuring displacement, stiffness, and failure load. RESULTS: Initial displacement, cyclic displacement, and tot

The upper limb in children with cerebral palsy. Evaluation and treatment.

Management of the upper limb in children with cerebral palsy is often complex and must be carried out by a team experienced in this field. Several clinical parameters must be taken into consideration, such as higher functions, visual problems, overall upper limb function, motor control, sensitivity, presence of hemineglect or synkinesis, limb position at rest and during walking. And last but not least, a complete analysis of the upper limb is required. It is only after this exhaustive assessment - which often includes occupational therapy, physiotherapy and in some cases, video and electromyog

Limb reconstruction in Ollier's disease.

We present our experience of lengthening and correction of complex deformities in the management of patients with Ollier's dysplasia (multiple enchondromatosis) from 1985 and 2002. All patients were under 18 years with a minimum follow-up time of 2 years (mean 9.6 years, range 2-15 years). There were a total of ten patients of which seven were male and three female. The mean age at presentation was 10.7 years (range 5-17 years; SD 3.7 years). The total length gain was 42.3 mm (range 30-110 mm; SD 28.9 mm). The number of days in external fixation was 164.8 days (range 76-244 days; SD 42.9 days)

[Combined crown lengthening surgery with restorative therapy for inducing papilla growth: a case report].

A young lady with unsatisfied restorations of upper anterior teeth and swollen gum wanted to improve aesthetics. Oral examination showed that 12-22 were provisional crowns with normal occlusion, poorly gingival contour and gummy smile. The gingiva was red, light swollen and bleeding on probing. X-ray showed the roots of 11, 21 were short and the alveolar bone absorbed. After the periodontal initial treatment, an ideal location of gingival margin was determined. Then, an esthetic periodontal surgery was performed to recover the biology width and the gingival margin was fitted with the anterior

Double V-shaped cancellous osteotomy of the distal radius for Kienböck's disease.

Treatments of Kienböck's disease range from conservative measures-usually immobilization during the acute phase-to such procedures as joint leveling by radial shortening or ulnar lengthening, intercarpal fusion, silastic arthroplasty, and vascularized bone grafts. Joint-leveling procedures have many complications such as nonunion, hardware problems, and pain. In this study, 34 patients with Kienböck's disease, stage 1 through 3B in Lichtman's classification, were treated with a new technique: a double V-shaped metaphyseal osteotomy of the distal radius and dorsal tilt of the distal radius. The

Early posterior ankle release in the treatment of congenital clubfoot.

Eighty-one patients (116 clubfeet) underwent posterior ankle release before the age of two years, following unsatisfactory responses to serial corrective casts applied according to the technique of Kite. Seventy-three per cent of these feet showed no or only mild talar flattening at an average follow-up of 7.5 years. Four years, following posterior ankle release there was a none-mild talar flattening rate of 69 per cent in this group compared to a 40 per cent none-mild rate in ankle release reduces the incidence of recurrent equinus deformity and the necessity for subsequent surgery in compari

Challenges with Fassier-Duval rod exchanges in congenital pseudarthrosis of the tibia: explant roadblock and solution.

Congenital pseudarthrosis of the tibia (CPT) is characterized by anterolateral tibial bowing and hamartomatous periosteum that predisposes it to fracture. Fassier-Duval telescopic rods can improve the structural integrity of bone segments after reconstruction. We present our experience treating CPT with the Fassier-Duval rod and a novel technique for Fassier-Duval exchange that was developed after extraction failed in one patient. Patients were identified who underwent treatment with Fassier-Duval rods for CPT between 2007 and 2016 and had undergone their first rod exchange. Medical records we

Impact of Screw Diameter on Pedicle Screw Fatigue Strength-A Biomechanical Evaluation.

OBJECTIVE: Loosening of pedicle screws is a frequently observed complication in spinal surgery. Because additional stabilization procedures such as cement augmentation or lengthening of the instrumentation involve relevant risks, optimal stability of the primarily implanted pedicle screw is of essential importance. The aim of the present study was to investigate the effect of increasing the screw diameter on pedicle screw stability. METHODS: A total of 10 human cadaveric vertebral bodies (L4) were included in the present study. The bone mineral density was evaluated using quantitative compute

Soft Tissue Retraction Maneuver in Cone Beam Computed Tomography Prior to Crown-Lengthening Procedure-A Technical Note.

Background: An accurate determination of the biological width and the relationship of the cemento-enamel junction with the border of the alveolar bone is crucial during a clinical crown-lengthening (CCL) procedure. The aim of this study was to present a technical note about the retraction techniques in cone beam computed tomography (CBCT) prior to CCL, highlighting the significant enhancement in procedural accuracy and predictability that these techniques offer. Methods: Clinical and radiological examinations should be performed before a CCL procedure. It is necessary to determine the length o

Restoration of Limb Length Discrepancy and Alignment With the Ilizarov Device After Management of an Aneurysmal Bone Cyst Crossing the Distal Femoral Physis.

An aneurysmal bone cyst (ABC), when located juxta-physeal, may rarely penetrate the growth plate and extend into the epiphysis. The recurrence rate is considered higher when ABC is in contact with the active growth plate. Treatment methods usually focus on cyst healing and the rate of cyst recurrence. We present the method of treatment used for addressing the lesion of the growth plate following the surgical management and healing of a juxta-physeal ABC. A seven-year-old girl had an aggressive ABC in the juxta-physeal area of the distal femur, penetrating the growth plate and extending in the

[Cleft lip, maxilla and palate treatment by Dr. Talmant's team in Nantes].

Over the last 30 years, our private cleft lip and palate team has developed an increasing activity based on the Victor Veau's concept: "All the structures are present and only deformed". Our goal is to achieve an anatomical and fully functional repair in every fields with the first operation. A few recent refinements have improved our primary procedures: intravelar veloplasty; simultaneous lengthening of the columella and primary lip repair in bilateral clefts; nasal retainer for the 3 or 4 first postoperative months allowing the establishment of a nasal breathing mode at once. Our timing has

[Treatment of severe concave clubfoot in neural muscular atrophy].

The deformity with which this neurological condition first shows itself is the pes cavus. During the period of growth a severely instable cavovarus deformity develops. The author avoids operations on children because the results are usually poor; the basic problem cannot be resolved with surgical measures. He recommends that the affected feet be immobilized by plastic boots until puberty is reached, at which time only minor bone resections for correcting the form of the feet are necessary. In cases of severe cavovarus deformity in early adulthood the author does not use the usual treatment, tr

Interimplant femoral fractures.

The number of hip or knee arthroplasties, and internal fixations of the proximal and distal femur, is increasing in proportion to the growing and ageing population, whose life expectancy is lengthening. Thus, fractures of the femur between proximal and distal implants, although rare, are becoming more frequent. Women over the age of 70, with fragile bones and whose ends of the two implants are close to each other ("kissing implants") are particularly vulnerable to them. Reliable and reproducible fracture classifications exist when it comes to 2 prostheses, but they are less well established in

Orthodontic management of the patient undergoing mandibular distraction osteogenesis.

Distraction osteogenesis is a gradual incremental bone-lengthening technique that requires very precise treatment planning and surgical execution. Subtle variation in the position of the corticotomy/osteotomy or in the position of the distraction device will affect the ultimate position of the tooth-bearing (distal) segment. Other functional and/or anatomic factors (that at this time are not well understood) also have an influence on the direction in which the tooth-bearing segment moves during distraction. These factors combine to create a discrepancy between the planned direction and the act
