Diagnosis and treatment of nail melanoma: a review of the clinicopathologic, dermoscopic, and genetic characteristics.

Nail melanoma (NM) is an important differential diagnosis in patients with longitudinal melanonychia. However, diagnosis is often challenging as it is difficult to differentiate from other pigmented nail disorders. The main challenge for diagnosis is obtaining adequate nail matrix biopsy specimens for histopathological assessment. Furthermore, the histopathological changes in the early stages of NM are subtle and contribute to a delay in diagnosis and care. Therefore, the integration of clinical and histopathological analyses is essential. Clinical and dermoscopic features, such as a broadened

[Pathophysiologic consequences of various osteosynthesis procedures in polytrauma patients. Part I: Experimental studies of intramedullary pressure development in reamed and unreamed intramedullary nailing and plate osteosynthesis of the femur].

The aim of this study was to measure the intramedullary pressure (IP) of a) a reamed intramedullary nail (RN), b) an unreamed intramedullary nail (UN), and c) the AO-plate (PL) for stabilization of an experimental fracture of a sheep femur. The increase in IP whilst reaming or introducing intramedullary nails in long bone fractures is often said to be responsible for fat intravasation and secondary systemic and pulmonary damage after severe blunt trauma. 21 sheep were studied, 7 in each group (RN, UN, PL). The animal study was permitted by the local authorities. Under general anesthesia, the f

[Intramedullary nailing of mid-femoral fractures].

75 consecutive cases of mid-shaft femoral fractures were treated by open intramedullary nailing, using the solid, diamond-shaped Hansen-Street nail. The average age was 32.5 years; road accidents accounted for 43 cases; average operative time was 40 minutes. There were 2 systemic complications but no infections. 56 of the 75 were studied after an average of 5 years and 19 were followed for at least 6 months. At 4 months all fractures were united except 1. Average time to sound bone union was 4 months. There was no residual deformity in varus-valgus or in the sagittal plane. Malrotation was rar

Pullout strength of connected pins in hand surgery: Effect of pin diameter and insertion angle.

OBJECTIVE: The purpose of this study was to examine the effect of insertion angles on the pullout strength of connected pins using a synthetic model simulating a hand bone. MATERIAL AND METHODS: The material consisted of Sawbones® (20 mm×20 mm × 60 mm), fixation pins secured to a connecting rod much like an external fixator, an electric drill (speed 1,290 rpm) and a tensile testing machine. The Sawbones® were drilled with different pin diameters (1.2 mm, 1.5 mm, and 1.8 mm) and insertion angles (100°, 110° and 120°). A vertical displacement of 1 mm/min was applied until the pins were extracte

Septic Dislocation of Hip After Cephalomedullary Nail Fixation of Intertrochanteric Fractures of Femur: A Case Report.

CASE: We describe 3 cases of septic hip arthritis with a dislocation after failed, cephalomedullary nail (CMN) fixation following fragility intertrochanteric (IT) hip fractures that were treated with antibiotic spacer placement. CONCLUSION: Septic hip arthritis and compromised hip abductors can contribute to dislocation of the hip after CMN for IT fractures. This case report presents a treatment plan to address this rare complication, not yet reported after CMN for IT fractures in the literature, to control infection and optimize function.

A Percutaneous Threaded Wire as a Clamp Technique for Avoiding Wedge Deformity While Nailing Intertrochanteric Femur Fractures.

Hip fractures are common injuries managed by the orthopaedic surgeon, and within the umbrella of hip fractures, intertrochanteric femur fractures constitute a significant portion of these injuries. Recent trends have shown an increased usage of cephalomedullary nails for these injuries. A known potential complication of this technique is the generation of a wedge deformity creating a varus alignment, especially when the cranial portion of the intertrochanteric fracture exits at or near the starting point of the nail. Although the biomechanics of this phenomenon are well described, few reports

Bilateral Tibial Fibrous Dysplasia in a Pediatric Patient treated with Intramedullary Nailing.

Fibrous dysplasia is a benign developmental disorder of bone in which fibrous connective tissue containing abnormal bone with irregular trabeculae replaces normal cancellous bone. It may affect 1 (monostotic) or multiple bones (polyostotic). Polyostotic disease is the less common of the 2, occurring in only 20 to 25% of fibrous dysplasia patients and tending to affect those who are younger than 10 years of age; patients having this form tend to experience bone enlargement beyond normal skeletal maturation, which can cause pain, progressive damage, and increased risk of pathological fracture. Nailing proximal femur fractures: how to choose starting point and proximal screw configuration.

Fractures of the proximal femur can be a challenging treatment dilemma for the orthopaedic surgeon. Complex mechanical forces and anatomic variables in this region combine to make treatment of these injuries difficult and can often result in serious complications. The decision to treat this fracture with an intramedullary device requires the surgeon evaluate many variables in the context of the specific fracture pattern. These include the choice of implant, starting portal location, and positioning of the patient. Assessment of the fracture pattern and its 3 dimensional orientation is usually

Complete spontaneous avascular necrosis of the adult navicula associated with Mee's growth arrest lines of the great and second toenails.

Spontaneous total avascular necrosis of the tarsal navicula has been well documented in children (Kohler's disease) but is uncommon in adults where partial necrosis is usually seen after trauma or in Müller-Weiss disease. A case of spontaneous complete navicular osteonecrosis in a 46 year old female is described; she had accompanying Mee's leuchonychial lines in the toenails of the great and second toes only; the lines resolved after 9 months. She has been treated with an excision of the navicula and interpositional iliac crest bone graft talo-cuneiform fusion with resolution of her pain. It i

Treatment strategies of distal radius fractures.

There has been a surge in the operative management of distal radius fractures. Closed reduction, external fixation, and open reduction with internal fixation each have advantages and disadvantages. The purpose of this review is not to provide the clinician with an algorithm for treatment of distal radius fractures. These fractures span an extensive spectrum of severity across age groups and demographics. Fortunately, the surgeon holds a vast array of options to provide care for patients with distal radius fractures. The choice of fixation or conservative care resides in the personality of the

A user-friendly method of pin site management for external fixators.

UNLABELLED: Pin tract inflammation and/or infection are common with the use of external fixation devices. Pin sites need to be managed to help prevent complications. There are a variety of regimens proposed for pin care in the literature. The authors present an efficient and simplified approach for addressing pin care protocol using cord locks with the use of compression sponges for external fixation devices. This method obviates the need for extensive bandaging and makes pin care easy for the patient to carry out. A brief review of the current evidence on pin care is presented, and an evidenc

Nail dystrophy and bony involvement in chronic sarcoidosis.

A patient with a chronic course of sarcoidosis with pulmonary and cutaneous involvement presented with dystrophy involving 20 nails. Histologic analysis revealed noncaseating granulomas consistent with nail sarcoidosis, and radiologic evaluation revealed bony cysts typical of sarcoidosis. Nail dystrophy in sarcoidosis is rare; however, when present, it signifies a chronic course of disease. We review the incidence, histopathologic correlation to clinical symptoms, radiologic findings, differential diagnosis, and treatment of nail dystrophy in sarcoidosis.

[Injuries to femoral vessels in fractures of the hip].

Injury to femoral vessels in trochanteric fractures is a rare but a severe complication. Most often it involves the profunda femoris artery, less often the (superficial) femoral artery. This injury may be caused by different mechanisms; most frequently by pressure of a sharp bone fragment (the lesser trochanter), the tip of protruding cortical screws (extramedullary implant) or the distal locking screw (Gamma nail). Less often it is caused by the drill bit or incorrectly placed elevators. Pressure of a sharp bone fragment or contact with protruding tips of screws results in a gradual erosion o

[Nail-patella syndrome. A case report and review of the literature].

We report on the case of a 36 year-old woman with nail-patella syndrome that was detected following a plain roentgenogram of the abdomen showing iliac horns. The nail-patella syndrome (NPS) is a hereditary disorder of tissues of ectodermal and mesodermal origin transmitted as an autosomal-dominant trait. Complete NPS is characterised by a series of skeletal disorders and specific nephropathy; in the recent years some other diseases associated with NPS were described, whereas many patients exist with incomplete NPS.
