Graft osteolysis and recurrent instability after the Latarjet procedure performed with bioabsorbable screw fixation.

HYPOTHESIS AND BACKGROUND: The Latarjet procedure is a reliable treatment of recurrent anterior shoulder instability. The coracoid process is usually fixed with metallic screws; however, these can lead to irritation and the necessity for hardware removal and also can produce artifacts on imaging studies. The use of resorbable screws could avoid these complications. The purpose of this study was to assess the clinical results of the Latarjet procedure performed with bioabsorbable screws in addition to healing of the graft and resorption of the screws. METHODS: In 2009, we performed a prospecti

Biceps tenodesis: a biomechanical study of fixation methods.

Rupture of the biceps tendon occurs predominantly in the middle-aged and elderly, being predisposed through bicipital tendinitis and rotator cuff lesions. Surgical repair may be an option for those requiring strength in supination. This study compared the initial fixation strength of keyhole tenodesis (n = 7) and interference screw fixation by use of cadaveric specimens. Two interference screws were evaluated (n = 7 x 2): the round-headed cannulated interference screw (RCI) and a bioresorbable screw (Sysorb). All specimens failed at the fixation site but one. This study found that overall ther

Less Invasive Surgery With Autologous Bone Grafting for Proximal Fifth Metatarsal Diaphyseal Stress Fractures.

Surgery with autologous bone grafting for proximal fifth metatarsal diaphyseal stress fracture has a potential to decrease nonunion, but it is not performed widely as the primary surgery because of donor-site morbidity. We have devised and performed a less invasive surgical procedure with autologous bone grafting and aimed to investigate the clinical and radiologic outcomes of this procedure. The data for 73 patients who underwent primary intramedullary screw fixation with autologous bone grafting from the fifth metatarsal base for proximal fifth metatarsal diaphyseal stress fractures were inv

Translaminar Screw Fixation in the Upper Thoracic Spine: Computed Tomography-Based Quantitative Laminar Analysis and Feasibility Study of Translaminar Virtual Screw Placement.

Translaminar screws (TLS) offer an alternative to pedicle screw fixation in the upper thoracic spine. Although some studies have described the anatomy of the laminae at the upper thoracic spine, computed tomography (CT) imaging is the modality of choice for presurgical planning. The characteristics of upper thoracic lamina in healthy Han adults have not been ascertained for the safe placement of TLSs. To determine the measurements (the diameter, maximal screw length, and optimal screw trajectory) required for safe TLSs placement in the T1-3 by CT scan evaluation, forty patients (20 men, 20 wom

One-third tubular hook plate.

It is often the case that screws cannot be firmly anchored in small, marginal joint fragments. This is especially relevant in osteoporotic bone. By redesigning the last plate hole of a one-third tubular plate to function as a hook, these bone fragments can be securely grasped and stabilized. Placing two plates on top of one another further improves mechanical strength.

Evaluation of pullout strength and failure mechanism of posterior instrumentation in normal and osteopenic thoracic vertebrae.

OBJECT: There is limited data on the pullout strength of spinal fixation devices in the thoracic spine among individuals with different bone quality. An in vitro biomechanical study on the thoracic spine was performed to compare the pullout strength and the mechanism of failure of 4 posterior fixation thoracic constructs in relation to bone mineral density (BMD). METHODS: A total of 80 vertebrae from 11 fresh-frozen thoracic spines (T2-12) were used. Based on the results from peripheral quantitative CT, specimens were divided into 2 groups (normal and osteopenic) according to their BMD. They

Non-union of the scaphoid. Treatment with cannulated screws compared with treatment with Herbert screws.

We retrospectively reviewed the results for thirty-four patients in whom a non-union of the scaphoid had been treated with bone-grafting and internal fixation with use of one of two types of screws as well as the temporary placement of Kirschner wires parallel to the screw to prevent rotation. The patients were divided into two groups: Group 1 contained sixteen patients who had been managed with a Herbert screw from 1986 through 1989 and Group 2, eighteen patients who had been managed with a 3.5-millimeter cannulated AO/ASIF screw from 1990 through 1992. There were no clinical or radiographic

Bovine bone graft inlay versus onlay for mandibular ridge augmentation (randomized clinical trial).

INTRODUCTION: Augmentation of the alveolar ridge became the backbone for treating the atrophic alveolar ridge. Several augmentation techniques have been proposed for this to compensate for changes in anatomy and ridge morphology and achieve aesthetic and biomechanical outcomes for the implant treatment plane. Increasing bone volume enhances implant stability and long-term implant survival. OBJECTIVES: To compare the efficacy of bovine block inlay graft vs. Onlay graft for 3D alveolar ridge augmentation in the mandible. MATERIALS AND METHODS: Twenty-four patients were randomly assigned to the

Atrophic Anterior Mandible Treated With Sandwich Osteotomy Without Mini-Screws and Mini-Plates: A Case Report With 7 Years of Follow-Up.

Restoring the anterior mandible may be challenging due to both insufficient height and width of the edentulous alveolar ridge; thus, this case report aimed to treat anterior mandibular atrophy by using the inlay technique without the use of mini-screws or mini-plates to stabilize the augmented bone fragments. A 19-year-old patient who lost his anterior teeth in an accident was treated with a horizontal osteotomy performed 4 mm from the alveolar ridge, with two oblique cuts made using an ultrasonic instrument, and the final phase of the osteotomy was performed with a lever for dental extraction

Technique for removal of symptomatic bone anchors placed during stress incontinence surgery.

INTRODUCTION: To present an uncomplicated, reliable technique for bone anchor removal in patients with anchor-related infections or chronic pain. TECHNICAL CONSIDERATIONS: We removed 17 anchors from 9 patients between 1999 and 2004. The surgical technique used fluoroscopy for localization of the anchors and an orthopedic broken screw removal instrument for resection of the anchor and surrounding bone. Nine patients with bone anchors had been symptomatic with chronic pain and/or wound drainage for a mean of 23.7 months before surgery. After removal, 10 anchors grew positive bacterial cultures.

Biomechanical evaluation of three different fixation methods for treating displaced tibial avulsion fracture of the posterior cruciate ligament: a finite element analysis.

OBJECTIVE: Displaced tibial avulsion fractures of the posterior cruciate ligament (PCL) significantly compromise knee stability; however, existing clinical data regarding treatment and prognosis are limited. There exists a paucity of biomechanical research concerning various surgical methods for tibial avulsion fractures of the PCL, and optimal management remains controversial. Therefore, the objective of this study was to investigate the biomechanical stability of displaced tibial avulsion fracture using suture bridge fixation, screw fixation, and TightRope fixation at varying flexion angles.

No long-term effects of ultrasound therapy on bioabsorbable screw-fixed lateral malleolar fracture.

BACKGROUND AND AIMS: The present study was initiated to evaluate the long-term effects of low-intensity ultrasound therapy on bioabsorbable screw-fixed lateral malleolar fractures, which has not been studied earlier. PATIENTS AND METHODS: The study design was prospective, randomized, double-blinded, and placebo-controlled. Sixteen dislocated lateral malleolar fractures were fixed with one bioabsorbable self-reinforced poly-L-lactide screw. The patients used an ultrasound device 20 minutes daily for six weeks without knowing it was active (eight patients) or inactive (eight patients). The foll

The Use of Titanium 3D Mini-Plates in the Surgical Treatment of Fractures of the Mandibular Condyle: A Systematic Review and Meta-Analysis of Clinical Trials.

INTRODUCTION: Fixing fractures of the base and neck of mandibular condyles is demanding due to the difficulties in surgical access and the various shapes of bone fragments. Classic fixation techniques assume the use of straight mini-plates, utilized for other craniofacial bone fractures. Three dimensional mini-plates may provide a reasonable alternative due to their ease of use and steadily improved mechanical properties. The multitude of different shapes of 3D mini-plates proves the need for their evaluation. AIM: This paper aims to summarize the clinical trials regarding the use of various

[Scaphoid pseudarthrosis of the proximal third--results of treatment with the Herbert screw].

Purpose of this study was to assess the value of the Herbert screw used with iliac crest cancellous bone graft for treatment of scaphoid nonunion in the proximal third. From 1986 to 1996, 75 patients underwent this procedure. We followed-up on 67 of them using the Martini score, which covers pain, grip strength and social activities, range of motion, X-ray results, individual, clinical and overall assessment. We found 57 patients (85.1%) with a good to excellent outcome, eight patients (11.9%) with satisfactory results and two patients with a poor result. We conclude that the Herbert screw wit

[Palmarly comminuted scaphoid fractures].

Two cases of acute scaphoid fractures of the middle third with palmar comminution were treated with cancellous bone transplantation and Herbert screw fixation. Despite 6 weeks of cast immobilization, secondary loss of reduction resulted in primary grade dislocation in one patient. In the other patient scaphoid dislocation led to dorsiflexed intercalated segment instability and the need for screw removal due to secondary joint irritation. As a consequence the authors recommend the use of cortical bone grafting of the iliac crest in cases where palmar defects occur after reduction of the scaphoi

Physeal Growth May Resume Following Removal of Transphyseal Epiphysiodesis Screws.

A 14 +6 year-old (bone age 13 +0 years) genetically 46, XX, phenotypic female patient underwent distal femur and proximal tibia screw epiphysiodesis for a leg length discrepancy related to overgrowth following juvenile ACL reconstruction. The patient was considering, but had not committed to, gender affirmation treatment (internally the patient felt male). The patient later started testosterone, which altered predictions of height and leg length discrepancy at maturity. Screws were removed to minimize overshortening of the previously long leg. Upon screw removal, modest growth of the operative

Novel cervical pedicle screw design to enhance the safety insertion without compromising biomechanical strength.

BACKGROUND: Lateral mass screw (LMS) is a more widely adopted method for posterior cervical spine fixation than the cervical pedicle screw (CPS). Despite its lower pullout strength, the insertions of LMS are more reproducible and have a lower risk. CPS insertion is a technically demanding procedure due to the small pedicle channel. Thus, CPS insertion has a high risk of pedicle wall perforation, resulting in neurovascular injury. For these reasons, surgeons may avoid CPS insertion despite its benefit of greater biomechanical strength. Therefore, an improvement in the CPS design is needed to av
