BACKGROUND: Amyloid transthyretin cardiomyopathy (ATTR-CM) is a progressive infiltrative cardiomyopathy that has seen significant advances in diagnosis and treatment over the last two decades. Sex differences in phenotype, treatment response, and long-term outcomes remain poorly understood. Our study aims to describe the factors that contribute to differences in phenotype and long-term outcomes between male and female patients.
METHODS: This was a single-center retrospective analysis of 334 patients with ATTR-CM seen at Northwestern Memorial Hospital, Chicago, IL, USA. The study included adul

Despite the fact that dietary salt restriction is the most logical measure to prevent accumulation of salt and water in patients without renal function, it is not applied in most dialysis centers. In this review, the reasons for this unlucky development are analyzed. First, it appears that many dialysis patients are slightly overhydrated, but this is often not noticed and, if so, the deleterious effects in the long run are not appreciated. These consist not only of 'drug-resistant' hypertension, but also dilatation of the cardiac compartments leading to preventable cardiovascular events. Secon

Doxorubicin (DOX), a cornerstone anthracycline antineoplastic agent, induces cumulative, dose-limiting cardiotoxicity that remains a major barrier to its clinical use. Accumulating evidence suggests that maladaptive metabolic remodeling-particularly loss of glycolytic competence-contributes centrally to DOX-induced cardiac dysfunction. Here we identify Sestrin 2 (SESN2) as a cardiomyocyte-intrinsic metabolic safeguard that maintains glycolytic integrity under anthracycline stress. Integrative transcriptomic analyses reveal that SESN2 is suppressed in DOX-injured myocardium and is closely align

BACKGROUND: Morbid obesity is a growing global health crisis that significantly increases cardiovascular morbidity and mortality. Patients with extreme obesity present unique diagnostic and therapeutic challenges, particularly in the management of heart failure with reduced ejection fraction (HFrEF).
CASE PRESENTATION: We report the case of a 29-year-old African American male with extreme morbid obesity (body mass index 84.62 kg/m2; weight 290 kg) who presented with chest pain, progressive dyspnea, orthopnea, and lower extremity edema. Laboratory evaluation revealed mildly elevated troponin l

Cardiac sarcoidosis (CS) is a rare and complex disease that requires a multidisciplinary approach for diagnosis and treatment. The diagnosis of CS can be confirmed by histological examination of non-caseating granulomas in cardiac or extracardiac tissue, along with supportive clinical and imaging findings. Symptoms are often non-specific, and the yield of endomyocardial biopsy is low due to the patchy nature of cardiac involvement-thus, many cases are not straightforward to diagnose, especially when pulmonary or extracardiac features are absent. Modern non-invasive imaging modalities have uniq

Cardiovascular disease (CVD) is a major complication of JAK2V617F-positive myeloproliferative neoplasms (MPNs), yet the mechanisms linking mutant hematopoiesis to CVD remain incompletely understood. Three complementary murine models were developed to delineate the role of hematopoietic-endothelial crosstalk in JAK2V617F-associated CVD. In Model 1, which expresses JAK2V617F in both hematopoietic cells and endothelial cells (ECs), mice developed spontaneous dilated cardiomyopathy, right ventricular and microvascular thrombosis, arteriolar stenosis, and an increased risk of sudden death in the ab

Diabetes mellitus and chronic inflammation are interconnected pathologies that necessitate multi-target therapeutic strategies. Ricinus communis, with a history of traditional use, was investigated for its anti-diabetic and anti-inflammatory potential via an integrated network pharmacology and bioinformatics approach. From 86 initial phytochemicals, seven bioactive compounds-quercetin, apigenin, kaempferol, stigmasterol, β-sitosterol, ellagic acid, and (-)-epicatechin-were screened based on pharmacokinetic and toxicity profiles. These compounds modulated 89 potential targets common to both dis

Anthracyclines and trastuzumab are widely used to treat breast cancer but increase the risk of cardiomyopathy and heart failure. With the use of trastuzumab and anthracycline-containing medications, this study intends to evaluate the effectiveness and security of current treatments against cardiotoxicity. We conducted a systematic review of randomized controlled trials (RCTs), which used at least one angiotensin-converting enzyme inhibitor (ACEI), angiotensin receptor blocker (ARB), or beta-blocker (BB) to prevent cardiotoxicity of antineoplastic agents for breast cancer, in 4 databases (PubMe

Disorders of immune dysregulation following heart transplantation in children have been reported; however, the management of such disorders remains uncertain and challenging. In this case report, we describe a clinical course of a child with severe autoimmune enteropathy after a heart transplant in infancy and detail a treatment approach with abatacept and alemtuzumab. A 21-month-old girl with a medical history of congenital dilated cardiomyopathy and heart transplantation at 2 months was evaluated for chronic hematochezia. The patient underwent an extensive workup, including endoscopic biopsy

Little is known about clinical symptomatology and genetics of juvenile onset Pompe disease (JOPD). The aims of this study were to analyze how these children are diagnosed, what clinical problems they have, and how phenotype is related to genotype. To accomplish this, we analyzed retrospectively data of 34 patients diagnosed after their first and before completion of their 18th birthday. Median age at diagnosis was 3.9 (range 1.1-17) years. Eight patients (23.5%) developed initial symptoms in the first year, 12 (35%) between 1 and 7 years, and 6 (18%) thereafter. Eight (23.5%) had no clinical s

Background In mammals, muscle contraction is controlled by a family of 10 sarcomeric myosin motors. The expression of one of its members, MYH7b, is regulated by alternative splicing, and while the protein is restricted to specialized muscles such as extraocular muscles or muscle spindles, RNA that cannot encode protein is expressed in most skeletal muscles and in the heart. Remarkably, birds and snakes express MYH7b protein in both heart and skeletal muscles. This observation suggests that in the mammalian heart, the motor activity of MYH7b may only be needed during development since its expre

Arrhythmogenic cardiomyopathy (AC) is a hereditary disorder characterized by degeneration of cardiac myocytes and their subsequent replacement by fat and fibrous tissue primarily in the right ventricle. Our study aimed to systematically evaluate the impact of significant demographic, clinical, electrocardiographic, and echocardiographic factors in arrhythmic events in AC patients. MEDLINE and Cochrane library databases were manually searched without year or language restriction or any other limits until July 31, 2017. A pooled odds ratio with 95% confidence intervals was calculated for each of

Neonatal heart transplantation was developed and established in the 1980's as a durable modality of therapy for complex-uncorrectable heart disease. Patients transplanted in the neonatal period have experienced unparalleled long-term survival, better than for any other form of solid-organ transplantation. However, the limited availability of neonatal and young infant donors has restricted the indications and applicability of heart transplantation among newborns in the current era. Indications for heart transplantation include congenital heart disease not amenable to other forms of surgical pal

OBJECTIVE: To estimate the prevalence of sickle cell disease (SCD) in pregnancy, and to measure risk factors, morbidity, and mortality among women with SCD with and without crisis at the time of birth.
METHODS: We conducted a population-based, retrospective cohort study on all births in the Healthcare Cost and Utilization Project Nationwide Inpatient Sample (HCUP-NIS) from 1999 to 2008. Births to SCD with and without crisis were identified using ICD-9 codes. Adjusted effects of risk factors and outcomes were estimated using logistic regression analyses. Effect of hemoglobin variants among wom

The clinical expression of hypertrophic cardiomyopathy (HC) is undoubtedly influenced by modifying genetic and environmental factors. Lifestyle practices such as tobacco and alcohol use, poor nutritional intake, and physical inactivity are strongly associated with adverse cardiovascular outcomes and increased mortality in the general population. Before addressing the direct effect of such modifiable factors on the natural history of HC, it is critical to define their prevalence in this population. A voluntary survey, drawing questions in part from the 2007 to 2008 National Health and Nutrition

Nonobstructive cardiomyopathies (CMs) may be familial in 20â<euro>"55% of cases. Little is known about the role of fetal echocardiography in such cases. We evaluated the cardiac function serially pre- and postnatally in cases with a family history of nonobstructive CM. The fetal and postnatal studies were performed in the echocardiogarphy laboratory at a tertiary institution. Twenty-six cases from 16 families with a family history of CM were studied. Three fetal echocardiograms were performed at or near 18, 25, and 32 weeks of gestation for complete cardiac functional assessment. Postnatally c

Long-chain 3-hydroxyacyl-CoA dehydrogenase (LCHAD) deficiency is an autosomal recessive disorder of mitochondrial fatty acid oxidation. Apart from life-threatening metabolic derangement with hypoketotic hypoglycemia, patients often show liver disease, cardiomyopathy, and neuropathy. A common mutation (1528G>C) in the gene coding for the alpha-subunit of the mitochondrial trifunctional protein harboring LCHAD activity is found in 87% of the alleles of patients. LCHAD is considered a rare disorder with only 63 patients reported in the literature. Whether this is due to a truly low prevalence of

BACKGROUND: The role of regulatory T cells (Tregs) in Trypanosoma cruzi infection remains unresolved, with conflicting evidence suggesting either protection from immunopathology or impairment of parasite control.
METHODS: We performed the first systematic review integrating data on Treg phenotype and function in human and experimental Chagas disease. A comprehensive search of PubMed, Embase, Scopus, and LILACS (2003-2025) identified studies assessing Tregs during T. cruzi infection. Study selection, data extraction, and risk-of-bias assessment followed PRISMA 2020 and Cochrane guidelines.
