Unusual presentations of craniovertebral junction tuberculosis: a report of 2 cases and literature review.

BACKGROUND: CVJ tuberculosis is a described entity requiring challenging ways of management. Severe neck pain, causing restricted neck movements and torticollis, is a characteristic presentation of neurologically asymptomatic suboccipital Pott's disease. CASE DESCRIPTION: Two patients with unusual CVJ tuberculosis form the basis for the present communication. The first patient presented with tubercular otitis media, causing progressive erosion of the petrous part of temporal bone, and destruction of the occipital condyle, along with the lateral mass of atlas, leading to CVJ instability. This

Surgical Anatomy of Vertebral Artery in Relation to Atlantoaxial Instrumentation: A Cadaveric Study.

BACKGROUND: With the advent of pedicle screws and advanced instrumentation techniques, internal fixation and stabilization of upper cervical vertebrae are possible in fractures of an axis. However, the proximity of vertebral arteries (VAs) poses a unique challenge to surgeons during these procedures and can result in profound physical impairment to patients. Cadaveric studies contributing to fine anatomical details necessitate conducting such studies. METHODS: After receiving due ethical permission, this descriptive cross-sectional study was carried out on 10 cadavers in the department of Ana

Multidisciplinary Exploration of Computed-Tomographic and Ancient-DNA Data of an Iron Age Skull From Latvia With Multiple Lytic Bone Lesions: Differential Diagnosis Between Metastatic Carcinoma, Multiple Myeloma and Skeletal Tuberculosis.

Multiple lytic lesions of the cranial vault are among the most diagnostically ambiguous findings in paleopathology, since at least three pathological processes (metastatic carcinoma, multiple myeloma and disseminated tuberculosis) can produce morphologically convergent "punched-out" foci, while diagenetic erosion can mimic all three. Differential diagnosis based on macroscopic observation alone is therefore frequently inconclusive. The recent debate about whether multiple myeloma should be regarded as an ancient disease at all further reinforces the need for adjunct diagnostic streams. We hypo

3D fat-saturated T1 SPACE sequence for the diagnosis of cervical artery dissection.

INTRODUCTION: This study aims to demonstrate the added value of a 3D fat-saturated (FS) T1 sampling perfection with application-optimised contrast using different flip angle evolutions (SPACE) sequence compared to 2D FS T1 spin echo (SE) for the diagnosis of cervical artery dissection. METHODS: Thirty-one patients were prospectively evaluated on a 1.5-T MR system for a clinical suspicion of acute or subacute cervical artery dissection with 3D T1 SPACE sequence. In 23 cases, the axial 2D FS T1 SE sequence was also used; only these cases were subsequently analysed. Two neuroradiologists indepen

[Application of spiral CT reconstruction in the forensic identification of atlantoaxial injuries].

OBJECTIVE: To evaluate diagnostic value of spiral CT reconstruction in atlantoaxial injuries. METHODS: The images of 25 cases of spiral CT reconstruction were analyzed and compared with images of CT scan and X-ray. RESULTS: In 7 cases of odontoid process fracture, X-ray demonstrated 4 cases and CT demonstrated 5 cases, whereas the spiral CT reconstruction diagnosed 7 cases, which could display the displacement of fracture clearly. The X-ray and CT showed asymmetric space between left and right gaps of atlantoaxial joint in 6 cases, while spiral CT reconstruction showed normal and excluded th

Occipital neurostimulation-induced muscle spasms: implications for lead placement.

BACKGROUND: For many headache types, occipital peripheral nerve stimulation (ONS) provides significant relief of chronic, frequent, and severe headaches. Though rarely reported, ONS may cause painful muscle spasms that make stimulator use impractical. The classic description of the technique advocates placement of the leads transversely at the level of the arch of C1 or at C1-2. At that level, the greater occipital nerve (GON) infrequently pierces the superficial fascia of the neck muscles to become superficial. However, important anatomic variability exists. OBJECTIVE: To report placement of

Bilateral persistence of type 1 proatlantal arteries: report of a case and review of the literature.

Persistent proatlantal artery type 1 is one of the four anastomotic vessels between the carotid and vertebrobasilar arterial systems. Persistence of this embryonic anastomosis is extremely rare. We present such a case with bilateral persistent proatlantal arteries that arose from internal carotid arteries, entered the skull via the foramen magnum and united with the horizontal portions of vertebral arteries. We also mention its embryology, potential clinical implications, and differentiating features between two types of proatlantal arteries.

Association of Spinal Cord Radiomic Features and Disability in Multiple Sclerosis.

BACKGROUND AND PURPOSE: Spinal cord pathology underpins disability accumulation in people with multiple sclerosis (pwMS). Visual inspection of spinal cord magentic resonance imaging (MRI) often fails to reliably detect injury. Radiomics analyzes signal intensities in images to identify pathological changes that may be imperceptible to the human eye. This study evaluated the application of radiomics to spinal cord MRI to distinguish subgroups of pwMS and disability correlations. METHODS: Radiomic features were extracted from upper cervical cord coverage on cross-sectional 3.0T brain noncontras

Temporomandibular disorders in ankylosing spondylitis: a cross-sectional, monocentric study.

There are contradictory results in the relevant literature about the relationship between objective determinants of craniocervical posture and temporomandibular disorder (TMD), whereas no study has worked on ankylosing spondylitis (AS) and TMD relationship. We conducted this study to test the predictors of TMD in AS patients and its relationship with craniocervical posture. AS patients aged between 18 and 50 years consecutively admitted to our outpatient clinics were recruited. TMD was diagnosed by 'Diagnostic Criteria for Temporomandibular Disorders (DC/TMD)'. Spinal mobility was assessed by

Post-mortem computed tomography findings of spinal column injuries in comparison to autopsy: A systematic review.

Accurate identification and evaluation of spinal column injuries in post-mortem settings are crucial for forensic investigations. While traditional autopsies have long been the gold standard, post-mortem computed tomography (PMCT) has emerged as a promising non-invasive and beneficial tool. This systematic review aims to compare the findings of PMCT scans with autopsies in cases of spinal column injuries. Out of 428 articles screened, 11 studies matched the inclusion criteria. PMCT was superior in visualizing fractures in the atlas, axis, and upper cervical spine compared to autopsies. Moreove

On the paranasal and paratympanic sinuses of perinatal Gavialis gangeticus, with notes on the endocast, ossification observations in the braincase, and cervical vertebrae.

Gavialis gangeticus is a critically endangered, enigmatic, long-snouted crocodylian with uncertain phylogenetic affinities. Molecular and morphological data return disparate results for its placement within the larger diversity of Crocodylia. Because of its conservational status, specimens of Gavialis are rarer in museum collections, especially ones of younger ontogenetic age. Here, we use computed tomography (CT) to image a perinatal specimen to investigate cranial morphology, paranasal, and paratympanic pneumaticity. We provide digital reconstructions of the cranial morphology as an open acc

Atlantoaxial Rotatory Subluxation in a 10-Year-Old Boy.

Atlantoaxial rotatory subluxation (AARS) is the loss of normal alignment and stability of the first (atlas) and second (axis) cervical vertebrae with respect to each other. We describe the clinical challenges of managing a 10-year-old boy who presented with repetitive episodes of torticollis. Open-mouth odontoid radiograph and computed tomographic (CT) scan gave a diagnosis in ARRS, based on its characteristic imaging findings. The child was admitted multiple times for continuous halter traction in the first 6 months after symptom onset. He also experienced a temporary complication from an ove

Scintigraphic Characteristics of the Occipitoatlantoaxial Region in Clinically Sound Horses.

The equine occipitoatlantoaxial (OAA) region is biomechanically complex, yet normative scintigraphic reference data for this area are lacking. This retrospective study characterized the physiological radiopharmaceutical uptake pattern in the OAA region of 60 clinically sound horses, assigned equally into four age groups: under 6 years (Group A), 6-10 years (Group B), 11-15 years (Group C), and over 15 years (Group D). A standardized, semiquantitative analysis of lateral bone-phase scintigrams was performed using region of interest (ROI) ratios to determine uptake patterns. A highly consistent

A consensus spinal cord cell type atlas across mouse, macaque, and human.

The spinal cord contains evolutionarily conserved cell types critical for motor function, sensory processing, and autonomic regulation, many of which are implicated in diverse neurological diseases and injuries. Yet the field lacks a comprehensive molecular characterization of cellular diversity in human, macaque, and mouse spinal cord. Here, we present a unified, cross-species cell type atlas based on the integration of single-nucleus gene expression, chromatin accessibility, and spatial transcriptomic data from segments within cervical, thoracic, lumbar, and sacral regions, including motor n
