Optimizing Care in Primary Biliary Cholangitis: Current Treatments and the Second-Line Decision.

Primary biliary cholangitis (PBC) is a chronic, autoimmune, cholestatic liver disease characterized by small bile duct injury that can lead to advanced liver disease including cirrhosis and liver failure. Early diagnosis of PBC is crucial to mitigate the onset of advanced liver disease. However, even after diagnosis, some patients with PBC remain untreated or continue to progress to advanced disease despite initiation of treatment. Further, extrahepatic symptoms such as pruritus and fatigue substantially affect quality of life yet may not be addressed with standard treatment. Ursodeoxycholic a

How Neonatologists Use Genetic Information.

OBJECTIVE: To delineate specific ways in which neonatologists integrate genetic information into their clinical decision making. STUDY DESIGN: We employed chart-stimulated recall, in which neonatologists described how they used genetic tests in specific patient cases, as well as semi-structured questioning about genetic information. RESULTS: Based on 28 interviews with neonatologists, we document 6 uses of genetic information: making a diagnosis, categorizing/stereotyping as "genetic," informing prognosis, influencing treatment, informing goals of care, and supporting accountability. Both sp

Loop gain and central chemosensitivity assessment as a valuable tool in guiding treatment decisions for central sleep apnea in children.

CITATION: This study presents 2 cases of central sleep apnea syndrome in children, highlighting the utility of assessing ventilatory control stability, particularly loop gain and central chemosensitivity in treatment decision-making. In the first case, elevated loop gain for oxygen correlated with periodic breathing, leading to successful treatment with supplemental oxygen in a 13-year-old male with Prader-Willi-like syndrome. Conversely, in the second case, dealing with a 10-year-old female with tumor in the brainstem-spinal cord junction, reduced loop gain prompted treatment with nocturnal n

A Surgical Decision-Making Algorithm for Reconstruction of Charcot Neuroarthropathy.

Surgical reconstruction of the foot and ankle following degenerative changes secondary to Charcot neuroarthropathy poses a challenge due to both soft-tissue and osseous deformity. As a limb salvage procedure, this article aims to address such deformity with the goal of returning to a braceable limb without subsequent ulceration and infection. As this disease process affects both bone and soft tissue, surgical reconstruction should be directed to address osseous and ligamentous deformities that may contribute to postoperative failure. We present an algorithm to eliminate deforming forces, ident

Integrating Patient Preferences with Guideline-Based Care in Neurogenic Lower Urinary Tract Dysfunction After Spinal Cord Injury.

Individual and social factors are important for clinical decision-making in patients with neurogenic bladder secondary to spinal cord injury (SCI). These factors include the availability of caregivers, social infrastructure, and personal preferences, which all can drive bladder management decisions. These elements can be overlooked in clinical decision-making; therefore, there is a need to elicit and prioritize patient preferences and values into neurogenic bladder care to facilitate personalized bladder management choices. For the purposes of this article, we review the role of guideline-base…

Gradient boosting approaches can outperform logistic regression for risk prediction in cutaneous allergy.

BACKGROUND: Contact allergy is a major clinical and public health challenge. It is important to identify individuals who are at risk and perform patch testing to identify relevant allergens. Predicting clinical risk on the basis of input parameters is common in clinical medicine and traditionally has been achieved with linear models. OBJECTIVES: We hypothesized that the risk of a clinically relevant positive patch test could be predicted according to clinical and demographic parameters. METHODS: We compared the predictive accuracy of logistic regression with more sophisticated machine learni
