Modeling the transmission mitigation impact of testing for infectious diseases.

A fundamental question of any program focused on the testing and timely diagnosis of a communicable disease is its effectiveness in reducing transmission. Here, we introduce testing effectiveness (TE)-the fraction by which testing and post-diagnosis isolation reduce transmission at the population scale-and a model that incorporates test specifications and usage, within-host pathogen dynamics, and human behaviors to estimate TE. Using TE to guide recommendations, we show that today's rapid diagnostics should be used immediately upon symptom onset to control influenza A and respiratory syncytial

The changing face of rabies in Canada.

Rabies prevention and control programs in Canada have proven highly successful in past decades and have significantly reduced both terrestrial animal and human rabies cases. Successful management and prevention of rabies to date have not, however, eliminated our need for ongoing rabies prevention and control programs. This issue of the Canadian Communicable Disease Report (CCDR) provides an overview of recent and emerging rabies trends and challenges in Canada and examines the rationale to maintain our rabies programs and further supplement them with new and innovative approaches. The articles

Clinical progress note: Rotavirus.

Rotavirus is a vaccine-preventable cause of acute, viral gastroenteritis typically characterized by diarrhea, vomiting, and fever. Prior to rotavirus vaccine introduction, nearly all children worldwide had been infected by 5 years of age. Severe rotavirus gastroenteritis causes dehydration and electrolyte derangements, and infants and young children are particularly vulnerable to seasonal outbreaks due to high communicability and initial infection severity. While rotavirus vaccination has significantly decreased hospitalization of infants with rotavirus infections, recognition and management o

Protective population behavior change in outbreaks of emerging infectious disease.

BACKGROUND: During outbreaks of emerging and re-emerging infections, the lack of effective drugs and vaccines increases reliance on non-pharmacologic public health interventions and behavior change to limit human-to-human transmission. Interventions that increase the speed with which infected individuals remove themselves from the susceptible population are paramount, particularly isolation and hospitalization. Ebola virus disease (EVD), Severe Acute Respiratory Syndrome (SARS), and Middle East Respiratory Syndrome (MERS) are zoonotic viruses that have caused significant recent outbreaks with

Towards a theoretically informed policy against a rakghoul plague outbreak.

A long time ago in a galaxy far, far away, the Sith Lord Karness Muur engineered the rakghoul plague, a disease that transformed infected humans into near-mindless predatory rakghouls. At its peak, the disease infected millions of individuals, giving rise to armies of rakghouls on a number of planets. Whether rakghoul populations have persisted until this day is not known, making a rakghoul invasion on Earth not completely improbable. Further, a strategy for defence against an outbreak of the disease on Earth has not yet been proposed. To fill this glaring gap, we developed the first mathemati

Neglected diseases, civil conflicts, and the right to health.

Neglected diseases remain one of the largest causes of disease and mortality. In addition to the difficulties in provision of appropriate drugs for specific diseases, many other factors contribute to the prevalence of such diseases and the difficulties in reducing their burden. We address the role that poor governance and politically motivated oppression have on the epidemiology of neglected diseases. We give case examples including filariasis in eastern Burma and vector-borne diseases (Chagas' disease, leishmaniasis, and yellow fever) in Colombia, we show the links between systematic human ri

Enterobius Vermicularis Infections in Iraq.

Enterobiasis continues to be among the highest parasitic infections affecting the human population worldwide. A study was conducted between 2011 - 2015 in Iraq to evaluate the enterobiasis reported by the Communicable Diseases Control Center (n=220,607 cases) in relation to demographic (age, sex, rural population and family size) and spatial variables (local and regional sources). Females were more parasitized than males, as well as children and youth ages 4 to 15. Approximately 40 % of cases are from the South region provinces (Thiqar, Miasan, Basrah and Wassit). However, most cases occurred

Quantitative analyses and modelling to support achievement of the 2020 goals for nine neglected tropical diseases.

Quantitative analysis and mathematical models are useful tools in informing strategies to control or eliminate disease. Currently, there is an urgent need to develop these tools to inform policy to achieve the 2020 goals for neglected tropical diseases (NTDs). In this paper we give an overview of a collection of novel model-based analyses which aim to address key questions on the dynamics of transmission and control of nine NTDs: Chagas disease, visceral leishmaniasis, human African trypanosomiasis, leprosy, soil-transmitted helminths, schistosomiasis, lymphatic filariasis, onchocerciasis and

Shifting priorities in vector biology to improve control of vector-borne disease.

Vector control remains the primary measure available to prevent pathogen transmission for the most devastating vector-borne diseases (VBDs): malaria, dengue, trypanosomiasis, filariasis, leishmaniasis, and Chagas disease. Current control strategies, however, are proving insufficient and the remarkable advances in the molecular biology of disease vectors over the last two decades have yet to result in tangible tools that effectively reduce VBD incidence. Here we argue that vector biologists must fundamentally shift their approach to VBD research. We propose an agenda highlighting the most criti

Almond oil for patients with hyperlipidemia: A randomized open-label controlled clinical trial.

BACKGROUND: Cardiovascular diseases currently account for nearly half of non-communicable diseases. It was shown that enjoying a handful of nuts every day can significantly reduce the risk of developing heart diseases as they contain a variety of nutrients and other bioactive substances contributing to lowering the risk of heart diseases and controlling the cholesterol. The aim of this study was to determine the effect of almond oil on the lipid profile of patients with hyperlipidemia. METHODS: Ninety-seven patients were divided into the intervention (n = 49) and control (n = 48) groups. The

Using spatio-temporal surveillance data to test the infectious environment of children before type 1 diabetes diagnosis.

The "hygiene hypothesis" postulates that reduced exposure to infections favours the development of autoimmunity and childhood type 1 diabetes (T1D). But on the other side, viruses, notably enteroviruses, are suspected to trigger T1D. The assessment of the possible relationships between infections and T1D still defies the classical tools of epidemiology. We report the methods and results of a geographical approach that maps the addresses of patients to a communicable diseases surveillance database. We mapped the addresses of patients at birth, infancy and T1D diagnosis to the weekly estimates o

Taenia ovis: an emerging threat to the Chinese sheep industry?

BACKGROUND: Taenia ovis is a tapeworm that is mainly transmitted between dogs and sheep. Although T. ovis infection is not a public health issue, it causes a great financial loss due to condemnation of carcasses.  The first outbreak of T. ovis infection in China occurred in 2015. Reassessment of adverse effects of T. ovis infection on Chinese sheep industry in future is necessary. DISCUSSION: The first T. ovis outbreak in China suggests that the epidemic situation across the country is underestimated. For the transmission of T. ovis, many factors, including eggs, dogs and wild canids, human b

Emerging and resurging vector-borne diseases.

Over the last four decades, a number of arthropod-borne infections have been recognized for the first time. Some have become of considerable public health importance, such as dengue hemorrhagic fever (DHF), and others are spreading geographically and their incidence is increasing. There has been an important recrudescence of several long-known vector-borne diseases. Malaria, leishmaniasis, dengue, and plague have resurged in numerous foci, in some cases where they were thought to be under effective control. In most instances, the appearance of new diseases and syndromes and the resurgence of o

Tuberculosis control in the New York State Department of Correctional Services: a case management approach.

In the New York State Department of Correctional Services (DOCS), the incidence of tuberculosis (TB) increased from 43 per 100,000 persons in 1985 to 225 per 100,000 in 1991. To combat this increase in cases, the NYSDOCS enhanced its collaborative efforts with the New York State Department of Health and the Division of Parole, developing a comprehensive TB control program. This control program focuses on prevention and containment of disease. Policies and directives were developed on the basis of epidemiologic principles, which include surveillance and detection. To assist the NYSDOCS in the i

Building Global Epidemiology and Response Capacity with Field Epidemiology Training Programs.

More than ever, competent field epidemiologists are needed worldwide. As known, new, and resurgent communicable diseases increase their global impact, the International Health Regulations and the Global Health Security Agenda call for sufficient field epidemiologic capacity in every country to rapidly detect, respond to, and contain public health emergencies, thereby ensuring global health security. To build this capacity, for >35 years the US Centers for Disease Control and Prevention has worked with countries around the globe to develop Field Epidemiology Training Programs (FETPs). FETP trai

Outbreak of Escherichia coli O157 phage type 2 infection associated with eating precooked meats.

Fourteen cases of Escherichia coli O157 infection were reported to the consultant in communicable disease control in Sunderland Health Authority in August 1995. E. coli O157 phage type 2, Vero cytotoxin 2 was isolated from the faeces of ten cases. Nine of the isolates were indistinguishable by Vero cytotoxin subtyping and analysis of chromosomal DNA. Two cases with haemolytic uraemic syndrome were confirmed serologically and two cases remained unconfirmed. A case control study showed infection with E. coli O157 to be associated with having eaten precooked meats from a single shop. Environmenta…
