Confronting complexity and unmet need: A qualitative study to inform palliative care service design for adults with multiple long-term conditions.

BACKGROUND: Most people live with and die from multiple long-term conditions. In the last year of life, many receive burdensome care of limited benefit, while access to palliative care remains inconsistent. Improving care for this population would help address inequity because disadvantaged groups are disproportionately affected. AIM: To examine professionals' perspectives on palliative care needs and end-of-life challenges for people with multiple long-term conditions and their families, and to identify implications for service design and organisation. DESIGN: Semi-structured interviews wer

Developing a professional pathway in health equity to facilitate curricular transformation at the University of Michigan Medical School.

PROBLEM: Medical schools are challenged to realign curricula to address society's needs in a rapidly changing environment, and to support new instruction and assessment methods that require substantial faculty time. APPROACH: In 2010, the University of Michigan Medical school began planning the Global Health and Disparities Path of Excellence (GHD Path), an optional co-curriculum for students interested in health disparities, with explicit goals to (1) draw attention to the school's social mission; (2) test new, faculty-intensive methods of learning and assessment for all students; and (3) se

The role of genetics in the provision of essential public health services.

States include genetics services among their public health programs, but budget shortfalls raise the question, is genetics an essential part of public health? We used the Essential Services of Public Health consensus statement and data from state genetics plans to analyze states' public health genetics programs. Public health genetics programs fulfill public health obligations: birth defects surveillance and prevention programs protect against environmental hazards, newborn screening programs prevent injuries, and clinical genetics programs ensure the quality and accessibility of health servic

Transforming family planning services in the Latin American and Caribbean region.

The 1994 International Conference on Population and Development held in Cairo has generated widespread commitment to changing family planning programs from categorical and medically focused service organizations to reproductive health initiatives that embrace a wide range of social and human services. This article uses qualitative data analysis to review the experience of nine family planning association projects in the Latin American and Caribbean region that have made a successful organizational transition from services focused on family planning to a gender-based and sexual health approach.

Evaluation Framework for HIV Prevention and Care Activities in the Enhanced Comprehensive HIV Prevention Planning Project, 2010-2013.

OBJECTIVE: The Enhanced Comprehensive HIV Prevention Planning (ECHPP) project was a demonstration project implemented by 12 U.S. health departments (2010-2013) to enhance HIV program planning in cities with high AIDS prevalence, in support of National HIV/AIDS Strategy goals. Grantees were required to improve their planning and implementation of HIV prevention and care programs to increase their impact on local HIV epidemics. A multilevel evaluation using multiple data sources, spanning multiple years (2008-2015), will be conducted to assess the effect of ECHPP on client outcomes (e.g., HIV ri

Patient-centered care for patients requiring dialysis during disasters and public health emergencies: a scoping review.

BACKGROUND: Disasters and public health emergencies disrupt health systems, threaten continuity of care, and disproportionately affect medically vulnerable populations. Patients requiring dialysis are especially at risk because their survival depends on regular access to complex, resource-intensive treatment. While disaster nephrology literature increasingly addresses preparedness and resilience, it remains unclear how explicitly this literature addresses patient-centered care. OBJECTIVE: To map the literature on patient-centered care for patients requiring dialysis during disasters and publi

Bigiswun Kid Project: a longitudinal study of adolescents living with high rates of prenatal alcohol exposure, fetal alcohol spectrum disorder and early life trauma in remote Australian Aboriginal communities.

INTRODUCTION: The Lililwan Project was the first Australian population-based prevalence study of fetal alcohol spectrum disorder (FASD) using active case ascertainment. Conducted in 2010-2011, the study included 95% of all eligible children aged 7-9 years living in the very remote Aboriginal communities of the Fitzroy Valley, Western Australia. Women from Marninwarntikura Women's Resource Centre, a local Aboriginal-led organisation, are concerned that some participants from the study are struggling in adolescence so partnered with researchers from the University of Sydney to follow up the Lili

Access to Affordable Housing Promotes Health and Well-Being and Reduces Hospital Visits.

Clinical interventions can only partially mitigate homelessness and housing insecurity, which are threats to health and well-being. Clinicians have several opportunities to address these problems: They can refer patients who are homeless or housing insecure to support services, advocate for their employer or care group to commit resources to end homelessness and housing insecurity, and/or work with government and private sector community organizations to address and eliminate these problems. Citing examples from around the US, we will illustrate how clinics, hospitals, health plans, and public

Committee Opinion no. 512: health care for transgender individuals.

Transgender individuals face harassment, discrimination, and rejection within our society. Lack of awareness, knowledge, and sensitivity in health care communities eventually leads to inadequate access to, underutilization of, and disparities within the health care system for this population. Although the care for these patients is often managed by a specialty team, obstetrician–gynecologists should be prepared to assist or refer transgender individuals with routine treatment and screening as well as hormonal and surgical therapies. The American College of Obstetricians and Gynecologists oppos

'Back to better': amplifying health equity, and determinants of health perspectives during the COVID-19 pandemic.

INTRODUCTION: Equity and social justice have long been key tenets of health promotion practice, policy and research. Health promotion foregrounds the pertinence of social, economic, cultural, political and spiritual life in creating and maintaining health. This necessitates a critical structural determinants of health perspective that actively engages with the experiences of health and wellbeing among diverse peoples. The inequitable impacts of pandemics are well documented, as are calls for improved pandemic responses. Yet, current pandemic and emergency preparedness plans do not adequately a

Role of digital media campaigns in improving TB health literacy at the community level.

Tuberculosis (TB) is a significant health issue in the world, and lack of health literacy has resulted in delayed care-seeking practices, non-compliance with treatment regimens, and subsequent stigma in communities with the infection. The objective of the study was to determine whether multi-platform digital media campaigns enhance health literacy of the community on TB. The study involved a cluster-randomized controlled trial with sixteen communities in four districts in twelve months with intervention communities (n = 8) having targeted digital media campaigns and control communities (n = 8)

Design of an implementation research study for a digital omnichannel community engagement and risk communication intervention for the prevention and control of vector-borne diseases in India: the OMNIVEC-India study protocol.

INTRODUCTION: Vector-borne diseases (VBDs), such as malaria, dengue, and lymphatic filariasis, pose significant public health challenges in India, contributing significantly to the nation's morbidity and mortality. In this scenario, digital technology promises scalable, contextual, and user-centric interventions for behavior change. However, systematic and comprehensive community-based digital approaches for VBD prevention in India are limited. This study, therefore, aims to co-design a digital omnichannel Community Engagement and Risk Communication (CERC) intervention for VBDs in collaboratio

[National action plan for the emergence of invasive meningococcal disease in Chile, 2012-2013].

Invasive meningococcal disease is challenging for public health, mainly when it manifests with sudden changes in incidence, serogroups and hypervirulent clones that spread in the population, causing great alarm due to its sequelae and often fatal course, a situation that occurred in Chile, starting at week 26 of the year 2012. To face this scenario, an organization of multidisciplinary teams was required, called W-135 Action Plan in Chile, which included sanitary alerts, education, reinforcement of the epidemiological surveillance of suspicious cases, immediate diagnosis through state-of-the-a

A G-CHAM program and policy development curriculum using simulated executive review in undergraduate public health nursing education.

BACKGROUND: Public health nurses in Japan commonly work in administrative settings and are expected to translate community health assessment findings into feasible programs and policies through organizational decision making, negotiation, and budgeting. However, undergraduate public health nursing education provides limited opportunities for students to experience this administrative program and policy development process. This study describes a G-CHAM program and policy development curriculum using simulated executive review and presents an initial assessment of changes in students' self asse

Aeromedical Transport Operations Using Helicopters during the 2016 Kumamoto Earthquake in Japan.

More than 6,000 people died in the Great Hanshin (Kobe) Earthquake in 1995, and it was later reported that there were around 500 preventable trauma deaths. In response, the Japanese government developed the helicopter emergency medical service in 2001, known in Japan as the "Doctor-Heli" (DH), which had 46 DHs and 2 private medical helicopters as of April 2016. DHs transport physicians and nurses to provide pre-hospital medical care at the scene of medical emergencies. Following lessons learned in the Great East Japan Earthquake in 2011, a research group in the Ministry of Health, Labour and W
