BACKGROUND: Lateral extra-articular tenodesis (LET) is a surgical technique used to decrease the risk of anterior cruciate ligament reconstruction (ACLR) failure by supplementing internal rotational stability and reducing pivot shift.
INDICATIONS: A growing body of literature indicates that LET should be performed for patients under 25 years, those with increased posterior tibial slope and ligamentous laxity, and elite athletes in cutting and/or pivoting sports. Additional indications include patients with grade 2 pivot shift or greater and those with a history of anterior cruciate ligament g

INTRODUCTION: Outcome after surgery depends on both patient-related as well as procedure-related risks. Complications after surgery are a significant burden to patients and to the health system. A vast amount of often unstructured data from different sources are generated during surgery, which contain valuable information associated with outcome. Advances in computer hardware and machine learning now increasingly facilitate the development of prediction models in standardised, parametric, information-rich areas such as the perioperative setting. For the development and validation of risk score

BACKGROUND: Deep medial collateral ligament (dMCL) injuries are a known cause of medial knee pain, which can result from either trauma or degenerative conditions. While conservative management, including bracing and physical therapy, remains the first-line treatment, persistent symptoms may necessitate additional interventions. Platelet-rich plasma (PRP) therapy has emerged as a promising treatment modality due to its ability to enhance ligament healing by promoting collagen synthesis and tissue regeneration. Recent studies highlight PRP's potential to improve pain and function in patients wit

BACKGROUND: Although rare, athletic head and cervical spine injuries can be devastating. All health care professionals involved in the on-field care of athletes with these injuries should be aware of updated national guidelines as well as individual institutional practices regarding head and neck immobilization after on-field injuries. Appropriate prehospital management-including evaluation, equipment removal, spine board immobilization, and patient transfer-can mitigate further risk for these injured athletes.
INDICATIONS: The goal of spinal-motion restriction is to reduce spinal instability

BACKGROUND: Open lateral retinaculum (LR) lengthening has shown strong outcomes when combined with additional patellofemoral stabilization procedures, such as the tibial tubercle osteotomy (TTO) and medial patellofemoral ligament complex reconstruction (MPFLR). Here, we present an optimized technique for concurrently performing these 3 procedures to comprehensively stabilize the patellofemoral joint.
INDICATIONS: Patients with a previous patellar dislocation and evidence of an MPFL tear on magnetic resonance imaging are indicated for reconstruction. Patella alta alongside previous patellar di

BACKGROUND: Female infertility is a global health issue however, its link with genital infections is often overlooked due to asymptomatic nature of infections. Delayed diagnosis and treatment due to absence of reliable point-of-care (POC) tools result in long-term pathological consequences and infertility. This pilot-scale study aims to identify the most noteworthy prognostic symptoms of genital infections that exhibit a significant correlation with reproductive tract disorders and infertility.
METHODS: We designed a detailed questionnaire and conducted a case-control, observational study wit

BACKGROUND: Sudden cardiac arrest (SCA) is the leading cause of student-athlete mortality, often described interchangeably as sudden cardiac death (SCD). For persons aged ≤35 years, structural heart disease, such as hypertrophic cardiomyopathy, is historically the most common etiology. Regarding individuals aged >35 years, coronary artery disease is the main contributor to SCD during exercise.
INDICATIONS: Though some athletes may endure prodromal symptoms prior to a SCA, approximately 25% to 50% do not. Up to 23,000 people aged <18 years die from SCA annually. SCA athlete deaths are reported

BACKGROUND: Early assessment of patients with suspected transient ischaemic attack (TIA) is crucial to provision of effective care, including initiation of preventive therapies and identification of stroke mimics. Many patients with TIA present to emergency medical services (EMS) but may not require hospitalisation. Paramedics could identify and refer patients with low-risk TIA, without conveyance to the ED. Safety and effectiveness of this model is unknown.
AIM: To assess the feasibility of undertaking a fully powered randomised controlled trial (RCT) to evaluate clinical and cost-effectiven

BACKGROUND: Quadriceps autograft, though well established for anterior cruciate ligament reconstruction, is underutilized in posterior cruciate ligament (PCL) reconstruction largely due to slow adoption. All-inside meniscal ramp repair and quadriceps tendon autograft PCL reconstruction have been described in isolation, but not concomitantly in a video journal.
INDICATIONS: PCL reconstruction is indicated in grade 3 isolated tears with instability that have not improved with nonoperative management and in instances with associated injuries such as meniscal ramp tears, as observed in this 18-ye

BACKGROUND: Anatomically, native posterior tibial slope (PTS) ranges from 6° to 10° and have significant effects on cruciate ligament stability. PTS <6° is correlated with increased posterior tibial translation (PTT) and force on the posterior cruciate ligament (PCL), predisposing individuals to PCL injuries and an increased risk of PCL graft attenuation. In rare cases, a reverse tibial slope can occur (<0°) as a result of trauma, physeal arrest, or abnormal development. This results in increased PTT and can lead to posterior tibial subluxation. Reverse tibial slopes in patients can be treated

BACKGROUND: Isolated triceps tendon injuries are infrequent, and their combination with a medial ligament avulsion is even rarer. Eccentric force on the triceps tendon following a fall on the outstretched hand, combined with a valgus torque at the elbow joint, exposes the medial side to stretching forces, potentially disrupting the ulnar collateral ligament (UCL). Furthermore, compressive forces on the lateral side could result in injury to the radial head. Currently, there is no standardized surgical approach for managing such combined injuries.
INDICATIONS: The complex triad injury presente

BACKGROUND: Global policy and guidelines for low back pain (LBP) management promote physical activity and self-management yet adherence is poor and a decline in outcomes is common following discharge from treatment. Health coaching is effective at improving exercise adherence, self-efficacy, and social support in individuals with chronic conditions, and may be an acceptable, cost-effective way to support people in the community following discharge from treatment for LBP.
AIM: This qualitative study aimed to understand which aspects of a community over-the-phone health-coaching program, were l

BACKGROUND: The ischial tuberosity apophysis serves as an attachment site for the hamstring muscle complex in the pediatric pelvis. Once the apophysis begins to ossify around age 13 to 15 years, decreasing elasticity makes the physis the weakest part of the hamstring attachment to the pelvis. An avulsion injury of the hamstring muscle group in the adolescent therefore results in a fracture in the adolescent and is the functional equivalent to a 3-tendon proximal hamstring injury in an adult. Ischial tuberosity fractures have a higher rate of non-union than other pelvic apophyseal injuries. End

BACKGROUND: Patellar instability with symptomatic cartilage lesions in young patients is a challenging problem to treat. The use of osteotomies to correct malalignment and fresh osteochondral allograft transplantation (OCA) to address cartilage lesions and patellofemoral dysplasia is a viable treatment option for carefully selected patients.
INDICATIONS: The patient is a 22-year-old woman with medial patellar instability caused by iatrogenic distal femoral external rotation and uncontained grade IV patellofemoral lesions. The patient was indicated for OCA because the lesions were too large fo

BACKGROUND: Anterior glenohumeral instability is common in the young and athletic population and can develop from a dislocation or subluxation event. Avulsion of the anterior inferior glenoid labrum (Bankart lesion) occurs in over 90% of these events. In patients who have unsuccessful conservative management or present with a high risk for redislocation, surgical intervention is indicated. This video presents our technique for arthroscopic anterior and inferior labral repair.
INDICATIONS: In addition to those patients who have unsuccessful conservative management, surgical management of anter

BACKGROUND: Fibular collateral ligament (FCL) injuries commonly present in a multiligament knee injury pattern. These injuries are associated with significant instability leading to altered tibiofemoral biomechanics and therefore require surgical intervention. Similarly, grade 3 posterior cruciate ligament (PCL) injuries may disrupt normal tibiofemoral and patellofemoral biomechanics and increase the risk of secondary osteoarthritis. Therefore, concomitant reconstruction of the FCL and PCL should be performed to decrease knee laxity and optimize functional outcomes.
INDICATIONS: Early operati

BACKGROUND: There are several techniques for the combined reconstruction of the anterior cruciate ligament (ACL) and the anterolateral ligament (ALL), but none have shown superiority. This study aims to present a surgical technique that theoretically reduces some of the risks reported in these procedures.
INDICATIONS: The main indications for the ALL reconstruction include chronic ACL injuries, revision ACL reconstruction, acute ACL injuries in patients with high-grade pivot-shift (grades 2 or 3), patients involved in pivoting sports, and generalized ligament hyperlaxity or knee hyperextensio

BACKGROUND: Medial patellofemoral ligament (MPFL) injury occurs in up 96% of lateral patellar dislocations, commonly affecting those less than 20 years old. The MPFL can be reconstructed using either a double-limb or single-limb technique. While both techniques achieve similar patient outcomes, a single-limb technique may require less operative time.
INDICATIONS: The MPFL reconstruction is indicated in patients with recurrent patellar subluxations, dislocations, or unresolved apprehension. Care should be exercised in identifying whether patients require additional procedures in addition to MP

BACKGROUND: The incidence of anterior cruciate ligament (ACL) injuries, reconstructions, and re-ruptures has rapidly increased. Patients with failed ACL reconstructions have been reported to suffer from far worse outcomes as compared with those with primary reconstructions, prompting the advancement of surgical and biologic techniques. Effective treatment of re-tears has been shown to be achieved either utilizing a 1-stage or 2-staged approach, with the latter preferred if the patient presents with significant bone loss, previously malpositioned tunnels, or unacceptable tunnel expansion.
