Autorosette inhibition factor: a positive acute phase reactant in interstitial cystitis.

Autorosette inhibition factor (AIF), complement C3d and eosinophil cationic protein (ECP) in urine were determined in 28 patients with painful bladder disease. In patients with interstitial cystitis (IC), diagnosed by the demonstration of detrusor mastocytosis, a positive correlation (r = 0.73, p less than 0.01) between AIF and C3d was found, whereas no reliable correlation was found in the remaining patients. The median concentration of urinary ECP was significantly elevated in the group of patients with IC whereas the median concentration of C3d was significantly elevated in both groups. AIF…

Acute hemorrhagic cystitis caused by adenovirus type 11 after renal transplantation.

A case of acute hemorrhagic cystitis caused by adenovirus type 11, which occurred in an allograft recipient 6 months after a living-related renal transplantation, is described. The patient lacked a neutralizing antibody to adenovirus type 11 before transplantation. Adenovirus type 11 was isolated from his urine and he developed a neutralizing and complement-fixing antibody to this virus. Although adenovirus type 11 isolates had been obtained from 2 of 18 renal allograft recipients, only 1 patient suffered acute hemorrhagic cystitis. Adenovirus type 11 may play a role in acute hemorrhagic cysti…

Glycosaminoglycan Replacement Therapy with Intravesical Instillations of Combined Hyaluronic Acid and Chondroitin Sulfate in Patients with Recurrent Cystitis, Post-radiation Cystitis and Bladder Pain Syndrome: A Narrative Review.

Defects in the glycosaminoglycan layer (GAG) of the bladder mucosa have been identified as a significant contributor to the pathogenesis and clinical progression of chronic inflammatory diseases of the bladder, such as post-radiation cystitis, bladder pain syndrome and recurrent urinary tract infections. This narrative review aims to explore the contemporary evidence on the role of GAG reconstitution with intravesical installations of hyaluronic acid and chondroitin sulfate in the management of those patients, with a goal to provide valuable insights for clinical practice. The reviewed studies…

Protocol for inducing cystitis via direct instillation of acrolein into the bladder of mice.

Mouse models of bladder inflammation, termed cystitis, can be used to study urinary dysfunction and visceral pain in both sexes. Here, we present a protocol for a mouse model of cystitis that is induced by instilling acrolein directly into the urinary bladder using a transurethral catheter. We provide instructions for preparing the acrolein solution and catheterizing female and male mice. We then detail procedures for confirming the voiding dysfunction phenotype using the void spot assay.

A rare case of incidental asymptomatic emphysematous cystitis in a patient taking empagliflozin.

Emphysematous cystitis (EC) is an uncommon and potentially serious urinary tract infection (UTI) characterized by gas formation within the bladder wall or lumen, frequently observed in patients with diabetes mellitus (DM). We report the case of a 76-year-old female with a history of atrial fibrillation (aFib) and heart failure (HF), who was incidentally diagnosed with EC on imaging during the evaluation of transient gross hematuria. She was clinically asymptomatic, with negative urine and blood cultures, and no systemic signs of infection. Notably, she was taking empagliflozin, a sodium-glucos…

Ferroptosis and interstitial cystitis: a systematic review of preclinical evidence and molecular mechanisms.

BACKGROUND: Interstitial cystitis/bladder pain syndrome (IC/BPS) is a chronic inflammatory bladder disorder with a poorly understood etiology and limited therapeutic options. Ferroptosis, an iron-dependent form of regulated cell death driven by lipid peroxidation, has recently been implicated in bladder epithelial damage, suggesting a potential role in IC/BPS pathogenesis. OBJECTIVE: To systematically review available preclinical studies examining the potential role of ferroptosis in experimental models relevant to interstitial cystitis, with a focus on molecular mechanisms and ferroptosis-re…

[Obstetrician-gynecologists and urologists perspective on the treatment of cystitis: the need for a multidisciplinary approach].

Cystitis in women is one of the most common urological diseases and requires a multidisciplinary approach. Depending on the stage of a womans life, her gynecological status, including hormonal levels, the condition of the vaginal microflora, and anatomical features, has a significant impact on the course of cystitis and its treatment. Underestimation of gynecological factors is sometimes a cause of treatment failure. The clinical features of cystitis in women of reproductive age and in perimenopause are discussed in the article. The need to assess estrogen status and vaginal flora are substant…

Emphysematous cystitis treated with targeted antibiotic therapies: a report of two cases.

Emphysematous cystitis (EC) is a rare, complicated urinary tract infection characterized by gas formation in and around the bladder wall, often occurring in older adults with diabetes mellitus, urinary tract obstruction, or long-term indwelling urinary catheters. EC can be easily misdiagnosed due to atypical symptoms and signs. Delayed treatment causes life-threatening complications like emphysematous pyelonephritis, bladder perforation, or septic shock. Here, we retrospectively analyzed two cases of EC who had been effectively treated and discharged following antibiotics, strict blood sugar c…

Outcomes of Long-Term Antibiotic Therapy in Women with Chronic Recalcitrant Cystitis.

INTRODUCTION AND HYPOTHESIS: Lower urinary tract symptoms (LUTS) may persist despite negative conventional urine cultures, potentially due to intracellular bacterial invasion, a condition described as chronic recalcitrant cystitis (CRC). Evidence evaluating CRC and the effectiveness of long-term antibiotic therapy for CRC remains limited. We aimed to evaluate patient-reported outcomes following long-term antibiotic treatment in women with CRC. METHODS: We conducted a retrospective review of women diagnosed with CRC who were treated with long-term antibiotics. Patient-reported outcome measures…

Interstitial cystitis as the initial presentation of primary Sjogren's syndrome: A case report.

Interstitial Cystitis, mostly affecting middle-aged women, has been rarely associated with Sjögren's syndrome. We report a 51-year-old woman who presented with painful micturition, pollakiuria, lower abdominal pain, and urinary urgency. In addition, the patient exhibited xerostomia, keratoconjunctivitis sicca, and leukopenia. The cystoscopy and pathological examinations confirmed the diagnosis of interstitial cystitis. Subsequent testing revealed a positive antinuclear antibody, leading to referral to the department of Rheumatology and Immunology for a labial gland biopsy, which resulted in a …

Immunotherapy-related cystitis induced by nivolumab: A case report and review of the literature.

Immune checkpoint inhibitors (ICIs) like nivolumab have revolutionized cancer treatment but can cause immune-related adverse events (irAEs), including rare cystitis. This case report describes a gastric cancer patient who developed urinary symptoms, such as hematuria and frequent urination, after nivolumab treatment. Despite initial antibiotics, symptoms worsened, leading to a diagnosis of immune-related cystitis. Intravenous methylprednisolone provided rapid relief and restored renal function. Early recognition and corticosteroid treatment are crucial for managing ICI-induced cystitis and pre…

Optimal Duration of Hydrodistension for Symptomatic Treatment of Interstitial Cystitis: A Systematic Review.

INTRODUCTION: Interstitial cystitis (IC) is a chronic pelvic condition in the absence of other pathology leading to significant morbidity with limited available treatment options. Cystoscopic hydrodistension (HD) remains an offered intervention for diagnosis and management, providing temporary relief in some patients. There is no overall consensus regarding the optimal duration of HD. This systematic review aims to comprehensively assess the existing literature to identify if there is a relationship between HD time and treatment efficacy. MATERIALS AND METHODS: A systematic search in MEDLINE,…

An Atypical Case of Concurrent Emphysematous Cystitis and Osteomyelitis in a Patient With Metastatic Cancer.

Emphysematous cystitis (EC) is a rare, gas-forming bladder wall infection, most commonly caused by Escherichia coli (E. coli) and typically seen in diabetic or immunocompromised patients. We report a case of a 50-year-old female with breast cancer undergoing neoadjuvant chemotherapy who presented with hematuria, nausea, and vomiting. Imaging and laboratory workup revealed EC and concurrent osteomyelitis caused by Pseudomonas aeruginosa (P. aeruginosa). She was treated with extended antibiotic therapy, leading to stabilization of the osteomyelitis. This report highlights the unusual role of P. …

Emphysematous cystitis: A hidden danger of bladder rupture and sepsis - Case report.

INTRODUCTION: Emphysematous cystitis (EC) is a rare anaerobic bacterial infection, often linked to diabetes, neurogenic bladder, or radiotherapy. Severe cases can weaken the bladder wall, causing spontaneous rupture, which may be intra- or extraperitoneal. While conservative treatment with drainage and antibiotics is possible, intraperitoneal rupture requires surgery to prevent septic shock. PRESENTATION OF CASE: A 61-year-old dialysis-dependent patient with a history of lumbotomy for renal calculus and type 2 diabetes was admitted for confusional syndrome and shock. Workup showed inflammator…

Potential Role of Macrophage Polarization in the Progression of Hunner-Type Interstitial Cystitis.

BACKGROUND: Hunner-type interstitial cystitis (HIC) is a chronic inflammatory condition of the bladder. However, it remains unclear whether there is a causal relationship between the presence of Hunner lesions and seemingly normal-appearing areas in the bladder (non-Hunner lesions). This study aimed to investigate the fundamental aspects of HIC by examining potential genetic differences between Hunner and non-Hunner lesions and elucidate their role as potential markers in the progression and suppression of the disease. METHODS: This cross-sectional study enrolled patients with HIC (n = 10) wh…
