[Formation of intraabdominal abscess after performance of intestinal surgery].

Experience of surgical treatment of 69 patients with intraabdominal abscess, formed after various intestinal operations performation, was summarized. In 28 patients the abscesses puncture was applied as well as their opening using traditional method. Complications had occurred in 15.9% of patients, postoperative mortality was 5.8%. Application of modern technologies of diagnosis and treatment constitutes perspective trend of the treatment results improvement in such patients.

Lower limb acute compartment syndrome after colorectal surgery in prolonged lithotomy position.

PURPOSE: Acute compartment syndrome in patients undergoing prolonged colorectal procedures is uncommon but can have catastrophic consequences for the patient with the development of metabolic acidosis, myoglobinuric renal failure, Volkmann's contracture, limb loss, and death. The potential to produce long-term disability in a patient has important medicolegal implications, particularly if the complication is avoidable. Why only some patients develop acute compartment syndrome is not fully understood. The purpose of this study was to highlight current knowledge and suggested prevention strategi

[Method of the surgical treatment of nonspeciphic extrasphincteric rectal fistula].

The method of treatment of nonspeciphic extrasphincteric rectal fistula was proposed, which consists of the fistula channel excision en bloc with external and internal (the anal sinus, which bears the fistula) apertures on perineum, subsequent layer by layer restoration of the rectal wall, as well as the sphincters and the anal levators muscles integrity (if necessary) and the formed cavity drainage. The operation was performed in 25 patients. Its performance have guaranteed the recurrences rate lowering as well as the postoperative incontinence occurrence.

A flexible approach to achieve a true primary repair for all infants with esophageal atresia.

Our purpose is to present our results using a flexible surgical approach to achieve a true primary repair for all infants with esophageal atresia (EA). The proposed methods are designed to reach this goal, even when most of the intrathoracic esophagus is missing. What has made this goal attainable is the ability to rapidly induce esophageal growth. We reviewed the results of 63 consecutive patients who presented between 1984 and 2004 with an esophageal gap of greater than 2.5 cm, a distance where increasingly difficult repairs begin. Gaps of 4 cm or longer (46), 6.0 cm or longer (14), and over

[Technique of surgical ampullectomy].

We describe the technique of surgical ampullectomy, which consists of complete resection of the papilla of Vater, including the sphincter, the distal part of common bile duct and Wirsung duct, and the duodenal wall around the papilla. Limits of resection are assessed by frozen section, particularly on both biliary and pancreatic ducts which are sutured together and reinserted on the duodenal wall. Surgical ampullectomy, combined with frozen section, is associated with a low morbidity, and represents a valid alternative to pancreaticoduodenectomy and endoscopic ampullectomy for presumed-benign

Wilkie's syndrome.

A superior mesenteric artery syndrome (SMAS) was diagnosed in two young women with, respectively, a 2- and 1-year history of postprandial vomiting and epigastric pain. The patients underwent a laparoscopic duodenojejunal bypass, and resumed a normal diet on the fifth postoperative day. The patients are still symptom-free with patent anastomosis on gastrointestinal radiographic control at 24 and 6 months, respectively, following their operation. Herein we also describe the varying clinical presentation of this rare syndrome, as well as treatment options. We conclude that laparoscopic duodenojej

Surgical treatment of respiratory complications associated with gastroesophageal reflux disease.

Although the association between gastroesophageal reflux (GER) and typical esophageal symptoms, such as heartburn and regurgitation, is straightforward, this is not the case for the relation between GER and respiratory symptoms. The strongest link is the finding of abnormal reflux in the proximal esophagus or pharynx. This finding, in conjunction with evidence of airway injury such as seen on laryngoscopy, strongly supports the relation between GER and airway disease. Medical therapy provides relief to some patients, but with less consistency than for those with typical symptoms of GER disease

Surgery and chronic pancreatitis.

It is hoped that, in this millennium, chronic pancreatitis will be diagnosed earlier in the course of the disease process. Improved axial imaging of the pancreatic duct and pancreatic parenchyma will diminish the need for other invasive tests. Surgical procedures are directed at pancreatic duct decompression or resection of the pancreas (head, body or tail) or, infrequently, total pancreatectomy. Pain relief in 75% to 90% is the general rule, with diabetes developing subsequently in as many as 33% of patients. Surgery for chronic pancreatitis is effective in correcting sequelae of pancreatic f

[Nineteen years experience with posterior sagittal anorectoplasty as a treatment of anorectal malformation].

UNLABELLED: Posterior sagittal anorectoplasty (PSARP) is considered today the best surgical technique for the treatment of the anorectal malformations. With the aim of evaluating the results of the PSARP in our patients, the charts of 39 children (27 male, 12 female) were reviewed. Mean age was 11 years (3 to 20 years) and the mean follow-up period from the closure of the colostomy was 9.4 years (1 to 18 years). Patients with any alteration of the extrinsic innervation or those with rare malformations were excluded of the study. According to the Wingspread classification, 14 patients had a hig

[Diagnosis and surgical treatment of secondary gastroesophageal reflux disease].

Examination and treatment was performed in 69 patients with secondary gastroesophageal reflux disease (GERD), 61 (88.4%) of whom were primarily operated on for achalasia of cardia and GERD. The choice of the operation method was based on complex examination of the patients. Due to conduction of preoperative preparation it was possible to perform the most physiological antireflux fundoplication operations in the patients.

Anal fissures.

BACKGROUND: Anal fissures are common conditions, presenting with bleeding, itching, and pain of varying severity. Pain and bleeding is frequently attributed to haemorrhoids, which may delay commencement of appropriate therapy. Other causes for bleeding must also be excluded, with investigations taking into account the clinical findings and the age of the patient. OBJECTIVE: To discuss the pathogenesis and management of anal fissures. DISCUSSION: Recent studies have changed our understanding of the pathophysiology of anal fissures. It is now known that the majority of fissures are caused by i

Small bowel volvulus: a review.

Small bowel volvulus is a rare but life-threatening surgical emergency. The aetiology may be primary, as is often seen in Africa and Asia, while in Western countries other predisposing conditions usually initiate the volvulus. Early preoperative investigation and expedient surgical treatment is required if bowel infarction is to be prevented. Central abdominal pain resistant to narcotic analgesia should heighten the suspicion of the diagnosis. The diagnostic value of computerised tomography (CT) scanning in such situations has been emphasised. If the bowel is infarcted resection is required, b

Rectal sleeve advancement: repair of rectovaginal fistula associated with anorectal stricture in Crohn's disease.

Successful repair of rectovaginal fistula in patients with Crohn's disease has been reported when these patients have a normal appearing rectum. We report the performance of stricturectomy in conjunction with circumferential rectal sleeve advancement for patients who have a rectovaginal fistula arising from an anorectal stricture secondary to Crohn's disease. This technique provides for repair of rectovaginal fistula and correction of the anal stricture and maintains continence.
