Peribiliary cysts are common in patients with chronic liver disease. Ambiguous imaging features and association with cirrhosis-induced hyperbilirubinemia may lead to misdiagnose an obstructive jaundice. Magnetic resonance cholangiopancreatography (MRCP) may be a useful sequence by showing small cystic structures with a specific periportal distribution on both sides of the portal veins, which do not communicate with the biliary ducts. These abnormalities may be recognized in order to avoid unnecessary endoscopic retrograde cholangiography.

The report presents a rare case of suffocation due to upper airways obstruction by a large mucolaryngocoele. The mechanism of death by suffocation was confirmed by histopathological examination of the lungs which showed acute lungs ectasy. Location of the laryngocoele hindered standard intubation by emergency doctor. Maybe the victim's life could have been saved by immediate conio- or tracheotomy, which in this case was not performed.

OBJECTIVE: Dilatation of the intercellular space (DIS) of the esophageal epithelium is recognized as one of the earliest histological changes in gastroesophageal reflux disease patients. At the human gastroesophageal junction, reactive nitrogen oxide species (RNOS) are generated luminally through the entero-salivary re-circulation of dietary nitrate. In cases with gastroesophageal reflux, the site of luminal RNOS generation may shift to the distal esophagus. The aim of this study was to investigate whether luminal RNOS exposure could be involved in the pathogenesis of DIS.

OBJECTIVE: To establish a classification of bicuspid aortic valve (BAV) that includes both leaflet morphology and aortic shape.
SETTING: Two academic medical centres of the University of Washington, Seattle.
PATIENTS: 191 adult patients with BAV.
INTERVENTIONS: Review of clinical data and transthoracic echocardiograms.
MAIN OUTCOME MEASURES: Assessment of leaflet morphology; valve function; aortic shape and dimensions.
RESULTS: We identified three morphologies: type 1, fusion of right and left coronary cusp (n = 152); type 2, right and non-coronary fusion (n = 39); and type 3, left and no

Symptoms of impaired consciousness (syncope and pre-syncope) occur in 15-25% of patients with hypertrophic cardiomyopathy (HCM).1 In young patients a history of recurrent syncope is associated with an increased risk of sudden death.2(-)5 Syncope usually occurs without warning or symptoms suggestive of the cause. Detailed investigations identify a probable mechanism in a minority, usually paroxysmal atrial fibrillation or ventricular tachycardia. In the majority however no likely mechanism is found despite repeated 24-hour ambulatory echocardiography (ECG) or patient-activated monitoring, exerc

Sphincter of Oddi dysfunction and pancreas divisum are very distinct anatomic abnormalities, yet are diagnosed in similar clinical situations. While both entities are uncommon, they are most often discovered during the evaluation of postcholecystectomy syndrome, recurrent idiopathic pancreatitis, and biliary or pancreatic pain when first line studies are normal. Treatment consists of surgical sphincteroplasty or endoscopic sphincterotomy for both diagnoses, which result in reliable relief of symptoms for most sphincter of Oddi dysfunction patients but less predictable response in pancreas divi

We describe here three cases of pancreatitis after congenital choledochal cyst excision. In these three cases, the choledochal remnant in the pancreas head was markedly dilated, probably because of an incomplete resection of the cyst at the primary operation, and an increase in intraluminal pressure of the pancreatic duct caused by a dynamic obstruction by a protein plug or a pancreatic calculus. Complete cyst excision, including the choledochal wall in the pancreas, is therefore strongly recommended.

PURPOSE: To find out if "tubular ectasia of the epididymis" is suggestive of postvasectomy status.
METHODS: Tubular ectasia of the epididymis is defined as enlargement of the epididymis with multiple interfaces (i.e., a speckled appearance). We found 24 cases exhibiting tubular ectasia of the epididymis over a period of six years. We reviewed the sonographic findings of tubular ectasia of the epididymis and evaluated the prevalence of associated findings.
RESULTS: In 24 patients with tubular ectasia of the epididymis, 16 cases were bilateral, five were right-sided, and three were left-sided.

An unusual case of choledocholithiasis followed by gallstone ileus documented by serial computed tomography is reported. A 91-year-old woman underwent gastrostomy because she repeatedly developed aspiration pneumonia, and a common bile duct stone was detected. She and her family refused surgery once symptoms resolved. One year later, she presented with increasing, intermittent abdominal pain and nausea. Abdominal computed tomography revealed a common bile duct stone with inflammatory changes, but the patient still refused surgery. Three months later, she was admitted with abdominal pain and vo

OBJECTIVE: We report the anesthesic and obstetrical management of two pregnant patients with Marfan's syndrome. An important dilatation of the root of aorta was established at the beginning of the pregnancy. Based on a review of the literature and our experience, we searched for clues to identify the ideal term and the best mode of delivery, and which type of anesthesia may be the more appropriate in patients with aortic dilatation.
RESULTS: No consensus can be found in the literature as far as anesthesia and obstetric management of these patients is concerned. Pregnancy must be continued as

We describe a neonate presenting with a cholestatic jaundice due to Alagille syndrome. On echocardiography as part of the diagnostic work-up, we noted a slight dilation of the normally functioning left ventricle at the initial examination in the third week of life. Over the next 2 weeks, serial echocardiograms showed slowly progressive dilation and dysfunction of the left ventricle, together with a persistent mild elevation of levels of cardiac Troponin-I in the serum. These findings, unrelated to the underlying Alagille syndrome, prompted cardiac catheterisation, which confirmed that the main

BACKGROUND: Patients with bicuspid aortic valves tend to develop dilatation of the ascending aorta. The aim of this study was to analyze whether or not there is any histologic difference in the aortic media of patients with a bicuspid aortic valve or a tricuspid aortic valve.
METHODS: A morphometric analysis of the wall of the ascending aorta was performed in 107 patients with bicuspid aortic valves undergoing aortic valve operations. The thickness of the elastic lamellae of the aortic media and the distances between the elastic lamellae were measured with the use of an image analysis system.

We report on a 69-year-old man presenting with a giant scrotal hernia, who failed to tolerate a pre-operative pneumoperitoneum applied with the intention to increase his intra-abdominal capacity. After enlarging the hernial orifice, the hernia contents - comprising the entire small bowel, the right colon and the greater omentum - were replaced into the abdominal cavity. Closure of the abdominal wound with mesh support was possible only after extensive resection of the small bowel, together with the voluminous mesentery and greater omentum. In a second operation performed later on revision of t

The spontaneous gastric rupture without a preexisting gastric ulcer is an extremely rare event. In the case presented we report the occurrence of a gastric rupture in context with a giant left scrotal hernia. The entire intestine was transposed into the hernial sack and this obviously led to a chronic distension of the stomach and finally to the spontaneous gastric rupture. Other reasons of a spontaneous gastric rupture as well as the surgical therapy are discussed. Important are a fast diagnosis and surgical revision in order to prevent patients from a massive diffuse peritonitis.

UNLABELLED: Wiskott-Aldrich syndrome is a rare X-linked disorder, comprising the triad eczema, thrombocytopenia and progressive immunodeficiency. The prognosis has been poor in the past but is now improving with therapeutic options including splenectomy and bone marrow transplantation. We report the case of a 21-year-old male with an established diagnosis of Wiskott-Aldrich syndrome, who developed aortic root dilatation with severe aortic regurgitation requiring aortic valve and root replacement. Histology confirmed a destructive, full-thickness, chronic aortitis.
CONCLUSION: As treatment and
