We sought to determine the lateral soft-tissue injury pattern in a consecutive series of patients with elbow dislocation (10 cases) or fracture-dislocation (52 cases) that required open operative repair. Patients who were seen more than 3 months after injury or those in whom previous operative intervention had obscured the anatomy were excluded. There were 42 men and 19 women (mean age, 43 years; range, 13-82 years). One patient had bilateral injuries. The mean time to surgery was 15 days after injury, with a range from 1 to 76 days. BACKGROUND: Posterior dislocation of the elbow with associated fractures of the radial head and the coronoid process of the ulna has been referred to as the "terrible triad of the elbow" because of the difficulties encountered in its management. A Steindler flexorplasty at the elbow was performed in 26 patients with an obstetric brachial plexus lesion because of deficient elbow flexion. Follow-up after a mean of 2.9 years (range, 1-7 years) showed a good functional result in 23 cases, whereas 3 primary failures were noted. In one case, a recurrent dislocation of the elbow was seen 3 years after the initial flexorplasty. Steindler flexorplasty is a comparatively simple operation and gives very reliable results. It should be preferred above other, more elaborate muscle transfers for loss of elbow flexion.

Between January 1993 and December 1996, 41 patients with fracture dislocation of the elbow joint were treated in our department. In 28 patients (median age 46 years, range 15-77 years; 16 male, 12 female), a clinical and radiological follow-up was obtained after median 34 months (range 12-59 months). Vibrotactilometry with testing of the thresholds of the vibration sense at seven frequencies between 8-500 Hz in different intensities has been correlated with nerve conduction studies of the ulnar nerve to evaluate its diagnostic power in neuropathies at the elbow. We operated on 16 patients for ulnar neuropathy associated with osteoarthritis of the elbow. They were all male manual workers, with an average age of 51 years at the time of surgery. Fractures of the radial head are frequently associated with elbow injuries in children. Thirty-seven patients with this association treated at the Nancy Children's Hospital were analyzed to ascertain the mechanism and the unfavorable factors of prognosis. A novel test for localizing ulnar mononeuropathies (UM), the electromyographic (EMG) motor Tinel's sign, has been developed. We reviewed the cases of 13 children with supracondylar elbow fractures with impaction of the medial wall. The displacement was underestimated in two patients and treated as a minimally displaced fracture by simple immobilization without reduction, resulting in a cubitus varus deformity. In one patient, the correct diagnosis was made, but treatment by reduction was refused by the parents. A loss of carrying angle of 10 degrees resulted from this.
