The triceps-preserving approach for semiconstrained total elbow arthroplasty.

Total elbow arthroplasty (TEA) is a useful tool to relieve pain and provide return to function for many conditions affecting the elbow. For conditions ranging from inflammatory arthropathies to comminuted intra-articular distal humeral fractures in the elderly, TEA is an excellent treatment alternative. Numerous surgical approaches for TEA have been described. Most surgeons use either a direct posterior or posterior-lateral incision. TEA is not without its complications. One such complication is insufficiency of the triceps. Many surgical approaches have been described to try to decrease the p

Electromyographic analysis of elbow function in tennis players.

Muscle activity about the elbow during tennis strokes in nine professional and collegiate level players was studied using indwelling EMG and high speed photography. Eight muscles were evaluated for the serve, forehand, and backhand strokes. The serve was divided into six stages and the ground strokes into four stages. EMG tracings were subjected to analog-to-digital conversion and a relative measure of quantity was obtained. Analysis of variance and Turkey tests were then done to assess statistical significance (P less than 0.05). The ground strokes showed low activity in all muscles tested du

Danger of loss of reduction of supracondylar elbow fracture during radiography.

All too often, the labors of manipulation and anatomical reduction of a supracondylar humeral fracture of the elbow requiring, perhaps, an hour or two, are noticed to be lost once the patients' follow-up radiographs are seen. It is our feeling that the reduction is lost when the forearm is externally rotated for the lateral view of the elbow. To prevent this, we recommend placing the X-ray cassette in the axilla to obtain the true lateral view, thus preventing the unsupervised rotation of the fracture.

Imaging of injuries in rock climbers.

Rock climbing has seen rapid growth in participation worldwide, resulting in an increasing frequency of sport-related injuries encountered in routine clinical practice. Many of these injuries arise from biomechanical demands unique to climbing and demonstrate characteristic imaging features. This review highlights the spectrum of acute and chronic rock-climbing injuries with emphasis on anatomy, injury patterns, and key imaging findings. Upper-extremity injuries predominate, particularly involving the fingers, where annular pulley injuries represent the most common and sport-specific pathology

The pediatric vs. the adolescent elbow. Some insight into age-specific treatment.

Almost 20 % of all long bone fractures in childhood and adolescents involve the elbow region. Physicians dealing with pediatric trauma cases on a regular basis must be familiar with the specific radiologic features of the elbow at every developmental stage. This includes the shape and the appearance of elbow ossification centers, and knowledge of age-specific injury patterns. In young children, lateral condyle and supracondylar fractures of the distal humerus are most common. Radial neck fractures, Monteggia's lesion and olecranon fractures appear in every age during growth. Bicondylar fractur

Sprains, Strains, and Partial Tears of the Medial Ulnar Collateral Ligament of the Elbow.

Medial ulnar collateral ligament (MUCL) insufficiency is becoming common in younger, nonprofessional athletes. In contrast to elite athletes who develop valgus extension overload syndrome and associated chronic pathologic changes in the MUCL, younger patients present with sprains and partial tears that can often be managed non-operatively with successful outcome and rapid return to play. In the younger throwing athlete with medial-sided elbow pain, a hinged elbow brace and rehabilitation of dysfunctional muscles often lead to successful recovery and return to play within 1-2 months. Complex Case of Open Fracture-Dislocation of the Elbow.

Complex elbow instability is difficult to surgically address. Careful consideration of the fractures and soft tissue injuries is required. We present the case of a patient who sustained an open fracture-dislocation of the elbow with significant loss of the external humeral condyle and partial loss of the olecranon. He was surgically treated with an iliac crest tricortical autograft fixed with a buttress plate and a lag screw. His lateral ulnar collateral ligament was reconstructed with tendinous autograft collected from his third and fourth extensor digitorum longus tendons. While the procedur

Coronal Shear Fractures of the Distal Humerus: A Review of Diagnosis, Treatment, and Outcomes.

Fractures of the capitellum and trochlea are uncommon fractures of the elbow and can be challenging to treat due to their size, location, and articular nature. Because of their intra-articular nature and predilection toward displacement, these fractures are typically treated operatively. Furthermore, capitellum fractures have high rates of associated injuries, including radial head fractures or lateral collateral ligament injury in ~30% to 60% of patients. In addition to open reduction internal fixation, operative options include fragment excision, arthroscopic assisted reduction and fixation,

Chronic triceps insufficiency managed with extensor carpi radialis longus and palmaris longus tendon grafts.

Chronic triceps insufficiency, causing prolonged disability, occurs due to a missed diagnosis of an acute rupture. We report a 25 year old male with history of a significant fall sustaining multiple injuries. Since then, he had inability in extending his right elbow for which he sought intervention after a year. Diagnosis of triceps rupture was made clinicoradiologically and surgery was planned. Intraoperative findings revealed a deficient triceps with a fleck of avulsed bone from olecranon. Ipsilateral double tendon graft including extensor carpi radialis longus and palmaris longus were ancho

Management of distal humerus fractures.

Fractures of the distal humerus are complex injuries that can be effectively treated with open reduction and internal fixation (ORiF). Exposure of a complex intra-articular fracture may best be achieved through a posterior approach with osteotomy of the olecranon process. The ulnar nerve must be identified and protected, the articular surface must be reduced anatomically, and rigid fixation must be applied to both the medial and lateral columns of the distal humerus. Range of motion should be initiated as soon as possible postoperatively. Complications such as ulnar neuropathy, elbow stiffness

Dislocations of the upper extremity in children.

Traumatic dislocations and subluxations of the upper extremity in children have been surveyed. The elbow joint is most commonly dislocated and is dislocated more frequently in children than in adults. Severe neurovascular injury may be associated with dislocation of this joint. The most common subluxation is that of the radial head, secondary to a "pulled elbow." Otherwise dislocations and subluxations of the upper extremity are uncommon in children, apparently because of the relative weakness of the epiphyseal plate. Most of these injuries are the result of predictable indirect forces. Princi

Congenital bilateral absence of the elbow flexors.

In this case report, a patient with bilateral congenital absence of the elbow flexors is described, which has not been described previously. The usual methods of tendon transfers after brachial plexus injury were not possible in this case because there were no biceps muscle tendons to which to attach. The method of moving the origin of the common extensor tendon proximally to have it function as an elbow flexor was successful and the patient obtained functional elbow flexion strength.

Current Fixation Options for Elbow, Forearm, Wrist, and Hand Fractures.

The universal goals of upper limb fracture management are to restore anatomic alignment, establish stable fracture fixation (while preserving blood supply), and allow for early mobilization of the elbow, wrist, and digits. However, the indications for operative management and fixation constructs are specific to each fracture type. This article systematically reviews current classifications and treatment options for adult fractures of the distal humerus, radius, ulna, scaphoid, metacarpals, and phalanges. For each anatomic location, we discuss the salient imaging features to guide management de

[Arthroscopy of smaller joints: hand, elbow and ankle arthroscopy].

These joints are accessible, though technically demanding, by arthroscope. Above all, the evaluation of the lesions is not simple. Arthroscopy always represents the last diagnostical alternative. The extraction of loose fragments, the debridement, the shaving and the treatment of infections are excellent indications. Postoperative rehabilitation is favoured and shortened with the arthroscopic approach. Elbow arthroscopy did not greatly, develop during the last years. Posttraumatic situations, corpora libera and cartilage lesions represent the main indications. The arthroscopies of the ankle an

Prevalence, burden and risk markers of health problems in HYROX athletes: a 12-week prospective cohort study.

INTRODUCTION: HYROX is a rapidly growing fitness competition combining eight 1-km running intervals with eight functional fitness stations. This study investigated the epidemiology and markers of health problems in HYROX athletes. METHOD: Eighty-nine HYROX athletes (56 female) completed the 'Oslo Sports Trauma Research Center Questionnaire on Health Problems' (OSTRC-H2) weekly for 12 weeks, alongside exposure reporting and a baseline questionnaire. In an exploratory analysis, potential risk markers (age, sex, previous injury and HYROX experience) for cumulative injury severity were examined u

Bruise Location in Supracondylar Humerus Fractures Predicts Specific Neurovascular Injuries.

PURPOSE: The presence of soft tissue injury in pediatric supracondylar humerus fractures (SCHFs) has been shown to be an independent predictor of any neurovascular injury. Potentially expanding this concept, the specific neurovascular structure injured around the elbow is thought to be dependent upon the direction and magnitude of fracture displacement and subsequent soft tissue injury. Therefore, it was hypothesized that the bruise location following SCHF is indicative of the anatomic location of maximal soft tissue injury and therefore is a specific prognosticator of which neurovascular stru

Isolated coronoid fracture: Assessment by magnetic resonance imaging for concomitant injuries.

BACKGROUND: Ligamentous injury associated with isolated coronoid fracture had been sparingly reported. Concealed or unclear fractures and ligamentous or articular cartilage lesions are promptly acknowledged by magnetic resonance imaging (MRI) but cannot be entirely pictured in regular radiological assessments. In isolated coronoid fracture, the fragment size is very small and due to the complex anatomy surrounding the coronoid radiographic imaging may not be sufficient. The purpose of this study was to evaluate the incidence of combined osteochondral and ligamentous injuries by magnetic resona
