Stroke and cardiac papillary fibroelastoma: mechanical thrombectomy after thrombolytic therapy.

We describe a case of a 34-year-old man with a sudden development of right hemiparesis and aphasia because of infarction of the left middle cerebral artery that was submitted to intravenous (IV) recombinant tissue plasminogen activator and mechanical thrombectomy. Transesophageal echocardiogram showed a small mass on the anterior leaflet of the mitral valve. Cardiac surgery was performed, and histological examination of the removed material was consistent with cardiac papillary fibroelastoma (CPF). Experience in using IV thrombolysis for the treatment of embolic stroke because of CPF is limite…

Case report: Death associated with stroke following intracarotid amobarbital testing.

OBJECTIVE: One previous accidental death during intracarotid amobarbital testing (IAT) associated with cerebral angiography (CA) has been re … edicare and Medicaid Services criteria or invasive procedures; neither did he have risk factors for embolic stroke. CONCLUSIONS: A problem noted is that IAT procedures vary from center to center and that IAT may increase the risk for individual patients because of differences between IAT and other CA interventions.

MR-imaging of thrombus in extra- and intracranial arteries employing balanced fast-field echo MRI.

Causes of stroke in young adults include a variety of disorders that are less frequently seen in older patients. We report the case of a young patient, who suffered recurrent mild embolic infarctions from a free-floating thrombus formation of the internal carotid artery bulb. During preoperative preparation, the thrombus dislodged and led to middle cerebral artery occlusion and major stroke. Both during preoperative work-up and after thrombus displacement balanced fast field echo sequences were able to demonstrate thrombus with a high signal-to-noise ratio and a superior delineation of thrombu

Stroke in Atrial Fibrillation and Other Atrial Dysrhythmias.

PURPOSE OF REVIEW: Atrial fibrillation (AF) is a major risk factor for systemic embolism and ischaemic stroke. Furthermore, AF-related strokes are associated with higher mortality, greater disability, longer hospital stays and lower rates of hospital discharge than strokes caused by other reasons. The aim of this review to summarise the existing evidence on the association of AF with ischemic stroke and provide insights on the pathophysiological mechanisms and the clinical management of patients with AF in order to reduce the burden of ischemic stroke. RECENT FINDINGS: Beyond Virchow's triad,

Carotid Subclavian Bypass for the Treatment of Coronary Subclavian Steal Syndrome.

Coronary subclavian steal syndrome is an uncommon cause of angina in patients with a previous coronary artery bypass graft procedure. The patient had chest pain with the exertion of the left upper limb, difference in blood pressure between the left and right arm, occlusion at the ostium of the left subclavian artery. He underwent carotid subclavian bypass surgery that was successful in relieving symptoms. On the other hand, the patient had an embolic stroke related to the procedure and further assessment may be necessary.

Metastatic lesions of atrial myxoma. A pathologist can clinch them all.

Atrial myxomas are the most common primary benign cardiac tumors. The embolization of tumor particles is not infrequent, and in nearly half of them, the cerebral arteries are affected, usually leading to embolic ischemic stroke. Formation of intracranial aneurysms, development of parenchymal brain metastasis, and intracerebral hemorrhage due to ruptured aneurysms are rarer. Diagnosis of such lesions in a previously undiagnosed case of myxoma may be challenging for a pathologist. Herein, we present two patients of cardiac myxoma with varied neurological manifestations and their pathological fin

Unreported neurological complications of Gemella bergeriae infective endocarditis.

We report the first case of native aortic and mitral valve endocarditis due to Gemella bergeriae from the Middle East in a young patient with rheumatic heart disease. Our case illustrates a fulminant course of infection with G. bergeriae endocarditis that was complicated by embolic stroke, as well as intracerebral and subarachnoid haemorrhage secondary to rupture of a mycotic aneurysm in the right middle cerebral artery. This case highlights the dire, unreported neurological complications of infective endocarditis due to a rare causative organism-G. bergeriae.

Recurrent ischemic stroke in a patient with idiopathic thrombocytopenic purpura.

Recurrent cerebral infarctions developed in a patient with idiopathic thrombocytopenic purpura (ITP). At the time of the first stroke, there were large thrombi in the right proximal internal carotid artery (ICA) and an occlusion of the right terminal ICA. The occlusion was recanalized by intra-arterial infusion of urokinase. After 20 months, he suffered another ischemic attack. The autoantibodies in ITP directed against antigens presented on both platelets and endothelial cells might induce the endothelial damage, thrombus formation, and embolic occlusion of the artery.

Central retinal artery occlusion as the initial sign of aortic valve papillary fibroelastoma.

PURPOSE: We describe a central retinal artery occlusion in a young man who was subsequently found to have an aortic valve papillary fibroelastoma. METHODS: Case report with clinical, echocardiographic, and histopathologic observations. RESULTS: The aortic valve papillary fibroelastoma was successfully treated with tumor resection. CONCLUSION: Appropriate investigations in young patients with central retinal artery occlusion should therefore be conducted, including transthoracic and transesophageal echocardiography, to diagnose and treat this tumor, because failure to diagnose this tumor may

A Case Series of Late Myocardial Infarction Following Self-Expanding Transcatheter Aortic Valve Replacement.

Embolic stroke is a recognized complication of transcatheter aortic valve replacement (TAVR); however, coronary embolism is rarely reported, particularly when occurring late after valve implantation. We described three patients presenting with ST-segment elevation myocardial infarction (STEMI) between 10 and 50 months after self-expanding TAVR. All patients had angiographically normal coronary arteries prior to TAVR, severe native aortic valve calcification, and well-controlled cardiovascular risk factors, and were maintained on guideline-directed single antiplatelet therapy. At presentation, …

[Early recurrence of spectacular shrinking deficit as a form of presentation in a cardio-embolic infarct].

INTRODUCTION: Spectacular shrinking deficit (SSD) is a little known neurovascular syndrome with a monophasic clinical course. It was observed in 4.8% (11 of 231 patients) with cardio-embolic cerebral infarcts in the records of patients with cerebrovascular disease at our hospital. CLINICAL CASE: We present the only patient in our records (0.04% of the cardio-embolic infarcts and 0.06% of the patients in the series, which consists of 1,500 consecutive patients with stroke studied over a period of 8 years) with recurrent SSD, which was the form of presentation of rheumatic mitral valve disease

An improved version of embolic model of brain ischemic injury in the rat.

A modified version of focal embolic stroke model has been developed in rats. Ischemic injury was induced by injection of a pre-formed clot into the middle cerebral artery (MCA). In the first series of experiments, the model was validated. Embolizing a pre-formed clot resulted in an infarction in the territory irrigated by the MCA. At 48 h after embolization, 2,3,5-triphenyltetrazolium chloride (TTC) staining showed that infarction volume was 42.1+/-15.6% (mean+/-S.D.) when 5 microl clot was injected (n=8) and 28.4+/-8.6% in the animals received 3.5 microl clot (n=8). The infarction volume betw…

Practical "1-2-3-4-Day" Rule for Starting Direct Oral Anticoagulants After Ischemic Stroke With Atrial Fibrillation: Combined Hospital-Based Cohort Study.

BACKGROUND: The "1-3-6-12-day rule" for starting direct oral anticoagulants (DOACs) in patients with nonvalvular atrial fibrillation after acute ischemic stroke or transient ischemic attack recommends timings that may be later than used in clinical practice. We investigated more practical optimal timing of DOAC initiation according to stroke severity. METHODS: The combined data of prospective registries in Japan, Stroke Acute Management with Urgent Risk-factor Assessment and Improvement-nonvalvular atrial fibrillation (September 2011 to March 2014) and RELAXED (February 2014 to April 2016) we

Cardioembolic stroke: an update.

Cardioembolic stroke accounts for approximately 15% of all strokes and is thought to be one of the more preventable types of strokes. Features that have been reported to support cardioembolism as a mechanism for ischemic stroke have included documented cardiac source of embolism, maximal neurologic deficit at onset, multiple cerebrovascular territories involved, enhanced tendency toward hemorrhagic transformation, enhanced risk of syncope or seizure associated with presentation, and lower likelihood of premonitory transient ischemic attacks. Features that tend to make cardioembolic stroke less…

High-Value Care in the Evaluation of Stroke.

Value-based care emphasizes achieving the greatest overall health benefit for every dollar spent. We present an interesting case of stroke, which made us consider how frequently health care providers are utilizing value-based care. A 73-year-old Caucasian, who was initially admitted for a hypertensive emergency, was transferred to our facility for worsening slurring of speech and left-sided weakness. The patient had an extensive chronic cerebrovascular disease, including multiple embolic type strokes, mainly in the distribution of the right temporal-occipital cerebral artery and transient isch

Clinical Impact of Switching From or Persisting With Rivaroxaban or Apixaban After a Bleeding Event: A Real-World Study in Patients With Nonvalvular Atrial Fibrillation.

BACKGROUND: Real-world research has identified differences in effectiveness and safety between direct oral anticoagulants (DOACs) when used to reduce risks of stroke and systemic embolism in patients with atrial fibrillation. However, treatment decisions and subsequent outcomes are poorly understood in patients with atrial fibrillation who experience bleeding when receiving a DOAC. METHODS: A retrospective observational study was conducted using US claims data from Optum's Clinformatics Data Mart Database (January 2012 through March 2024).
