Fibropolycystic liver disease in children.

Fibropolycystic liver diseases are a group of associated congenital disorders that present most often in childhood. These disorders include congenital hepatic fibrosis, biliary hamartomas, autosomal dominant polycystic liver disease, choledochal cysts and Caroli disease. We present a discussion and illustrations of the embryology, genetics, anatomy, pathology, imaging approach and key imaging features that distinguish fibropolycystic liver disease in children. The pathogenesis of these disorders is believed to be abnormal development of the embryonic ductal plates, which ultimately form the li…

[Patent foramen ovale: current state of the art].

Patent foramen ovale is a congenital cardiac lesion that persists into adulthood and which is present in over 25% of the adult population. Its diagnosis, evaluation and treatment have attracted increasing interest as it has been suggested that it may be associated with various pathologic conditions, such as cryptogenic stroke, platypnea-orthodeoxia syndrome, decompression sickness and migraine. However, data on these associations are contradictory. Similarly, the optimum treatment of patients with patent foramen ovale is still debated. This article contains a review of the anatomy, embryology …

Esophageal atresia. Lessons I have learned in a 40-year experience.

A brief resume of the highlights in the history of oesophageal atresia is presented. This is followed by research into the etiology, ontogeny and embryology, and microbiological studies. A revised classification of risk factors with consequent survival statistics is presented. Lessons learned in the management of the condition over a 40-year period are reported with particular emphasis on the management of the preterm infant with associated severe respiratory distress, right-side aortic arch, upper pouch fistula, 'long-gap' atresia, and the use of gastrostomy and intercostals drains. The incid…

Agenesis of the gallbladder: difficulties in management.

Gallbladder agenesis is a rare congenital biliary anomaly that may be associated with other biliary and extrabiliary congenital anomalies. Awareness of this entity by clinicians and radiologists is essential because many of these patients present with biliary symptoms and have unnecessary operations. In the present article, the relative epidemiological, etiological (embryology and development), pathophysiological, diagnostic tools and pitfalls and management aspects of this rare anatomic anomaly are briefly discussed through review of the literature. Particular reference to the difficulty in p…

Covered exstrophy with visceral sequestration: case report and review of literature.

We report a case of a male newborn with asymmetrical epispadic diphallia and congenital urethral fistula, and exomphalos minor. A segment of bowel was attached in the region of the pubis; with no communication to the underlying bowel. This was excised; histological examination revealed this to be a colonic remnant. The excision of the examphalos minor and approximation of the pubis and the rectus sheath over the bladder was also done; urethroplasty is planned at a later date. A comprehensive review of the available literature revealed only eight such cases before this; the possible embryology …

CT findings in congenital anomalies of the spleen.

There is a wide range of congenital anomalies of the spleen. Some are common, such as splenic lobulation and accessory spleen. Other less common conditions, such as wandering spleen and polysplenia, have particular clinical significance. Radiologists need to be aware of the various congenital variants of the spleen in order to recognize clinically important anomalies and to avoid mistaking less significant ones for an abnormality. In this pictorial review, the embryology of congenital anomalies of the spleen as well as their appearance on CT are described, diagnostic pitfalls are identified an…

Preduodenal portal vein in the adult.

Preduodenal portal vein (PDPV) is a rare developmental anomaly. We recently encountered this anomaly in a 73-year-old woman who had gastric cancer and colonic cancer with liver metastasis. The PDPV was diagnosed preoperatively by computed tomography and angiography. To the best of our knowledge, there are only 19 adult cases reported in the world literature, with this additional case being reported here. In infants the anomaly has often been associated with duodenal obstruction, but in adults it is often symptomless. We discuss the disease process, embryology, diagnosis, association with other…

Cardiac anomalies restricting blood flow to the left atrium.

This article continues the series describing the presentation and treatment of congenital cardiac disease in the neonatal period. Cardiac anomalies that restrict the return of blood to the left atrium are reviewed. These include total anomalous pulmonary venous connection (with emphasis on TAPVC with obstruction), mitral atresia and mitral stenosis, and pulmonary vein obstruction. Incidence, embryology, pathophysiology and hemodynamics, clinical presentation, differential diagnosis, and medical and nursing management, as well as surgical repair and prognosis, are discussed for each of these de…

Isolated agenesis of the gallbladder. An intraoperative problem.

Agenesis of the gallbladder and cystic duct is a rare congenital malformation. In 40-70% of cases this anomaly is associated with other gastrointestinal, skeletal, cardiovascular and genitourinary malformations. Lithiasis of the common bile duct is present in 25-50% of cases. In the majority of cases patients are asymptomatic or have symptoms compatible with a biliary disorder. A preoperative diagnosis is extremely difficult and the absence of the gallbladder is often an intraoperative finding. The authors report a case of isolated agenesis of the gallbladder. The relative embryology, developm…

Nasoalveolar cysts: a review of two cases.

Nasoalveolar cysts, also referred to as Klestadt's or nasolabial cysts, though well-recognized, are very rare. Patients with these nonodontogenic cystic masses may present to the otolaryngologist-head and neck surgeon with either intranasal, gingival, or perinasal soft-tissue complaints. The congenital nature of these soft-tissue cysts necessitates that the clinician be aware of the embryology of these lesions when planning a surgical approach to their removal. The authors present two such cases, both in middle-aged females, with left-sided nasoalveolar cysts, who presented to the senior autho…

Avulsion of the anomalous facial nerve at stapedectomy.

Injury to the facial nerve in an anomalous location overlying the oval window at the time of stapedectomy has rarely been reported. The authors recently have encountered three patients with such injuries. The facial nerve was found at the time of reexploration to run directly over the oval window. A fourth patient was explored primarily for a conductive hearing loss and the same anomalous course was discovered. The purpose of this report is to alert the otologic surgeon to this rare but potentially catastrophic anomaly and to review the pertinent embryology and management of these malformation…

Congenital nasolacrimal mucoceles: CT evaluation.

A congenital nasolacrimal mucocele, a lacrimal sac mucocele with intranasal extension, is an uncommon mass arising in the medial canthal region of the orbit. The authors describe four infants who presented with medial canthal masses and nasal airway obstruction. All underwent computed tomographic (CT) evaluation, which revealed the triad of cystic dilatation of the lacrimal sac, dilatation of the nasolacrimal duct, and an intranasal cystic mass. Appropriate therapy was determined following identification of the intranasal cystic component. Relevant discussions of the embryology, clinical prese…

Exstrophy of the cloacal membrane. A pathologic study of four cases.

Exstrophy of the cloaca is a rare congenital anomaly. The authors add the pathologic findings of four distinct cases reported in this article to those reported in the literature. In all cases, common anomalies were recognized. In addition, there were rare anomalies, such as single umbilical artery, vestige of the left superior vena cava, common mesenterium, calcification of the cerebellum, incomplete segmentation of the left lung, abnormal shape of the liver, and knock-knee. The embryology of this complex anomaly is difficult. It is considered that this anomalous condition results from breakdo…

[Hydrocele in an infant].

Proper knowledge of the embryologic development of the male genitalia is paramount for correct and successful treatment. The fatal outcome of the following pedriatric case is unfortunate due to a disregard of the basic principles in urology.
