Study for Improving Maternal Pregnancy And Child ouTcomes (IMPACT): a study protocol for a Swedish prospective multicentre cohort study.

INTRODUCTION: First-trimester pregnancy risk evaluation facilitates individualised antenatal care, as well as application of preventive strategies for pre-eclampsia or birth of a small for gestational age infant. A range of early intervention strategies in pregnancies identified as high risk at the end of the first trimester has been shown to decrease the risk of preterm pre-eclampsia (<37 gestational weeks). The aim of this project is to create the Improving Maternal Pregnancy And Child ouTcomes (IMPACT) database; a nationwide database with individual patient data, including predictors record

Ethical aspects of a predictive test for Huntington's Disease: A long term perspective.

BACKGROUND: A predictive genetic test for Huntington's disease can be used before any symptoms are apparent, but there is only sparse knowledge about the long-term consequences of a positive test result. Such knowledge is important in order to gain a deeper understanding of families' experiences. OBJECTIVES: The aim of the study was to describe a young couple's long-term experiences and the consequences of a predictive test for Huntington's disease. RESEARCH DESIGN: A descriptive case study design was used with a longitudinal narrative life history approach. PARTICIPANTS AND RESEARCH CONTEX

Neuropsychiatric and cardiometabolic comorbidities in patients with previously diagnosed Cushing's disease: a longitudinal observational study.

INTRODUCTION: Only few studies have systematically investigated neuropsychiatric aspects in patients with Cushing's disease (CD). Pain syndromes have been described in patients with pituitary adenomas, but so far no systematical investigation has been conducted in patients with CD. Additionally, CD has an association with cardiometabolic comorbidities which ultimately leads to increased morbidity and mortality. Long-term treatment of the hypercortisolic state cannot prevent the persistence of an unfavourable cardiometabolic risk profile. Finally, chronic hypercortisolism is known to impact the

Systematic scoping review of cluster randomised trials conducted exclusively in low-income and middle-income countries between 2017 and 2022.

OBJECTIVE: Cluster randomised trials (CRTs) are used for evaluating health-related interventions in low-income and middle-income countries (LMICs) but raise complex ethical issues. To inform the development of future ethics guidance, we aim to characterise CRTs conducted exclusively in LMICs by examining the types of clusters, settings, author affiliations and primary clinical focus and to evaluate adherence to trial registration and ethics reporting requirements over time. DESIGN: A systematic scoping review using the Preferred Reporting Items for Systematic Review and Meta-Analyses Extensio

CORE-ALI: protocol for a pan-European mixed-methods study to develop a core outcome set for acute lower limb ischaemia.

BACKGROUND: Acute lower limb ischaemia (ALI) is a life- and limb-threatening vascular emergency requiring urgent intervention. Despite advancements in therapeutic strategies, outcome reporting for ALI remains inconsistent, limiting evidence synthesis and guideline development. The CORE-ALI study aims to develop a Core Outcome Set (COS) to standardise outcome reporting and ensure the inclusion of both clinical and patient-centred metrics. METHODS: CORE-ALI will use a structured, multi-phase methodology guided by the Core Outcome Measures in Effectiveness Trials (COMET) initiative and the Core

Protocol for a multi-country retrospective observational paediatric sepsis epidemiological study (SENTINEL International).

INTRODUCTION: Improving outcomes from sepsis in children is a WHO Global Health Priority, yet mortality from sepsis remains high, particularly in low- and middle-income countries (LMICs). This database from children with community-acquired childhood sepsis in LMICs and some high-income countries will allow analysis of the burden of disease, including incidence, severity and outcomes. Understanding these aspects of sepsis care is fundamental for the design and conduct of future international interventional trials to improve childhood sepsis outcomes. METHODS AND ANALYSIS: This multicountry ret

Participatory longitudinal qualitative interview study to understand Autistic gynaecological and obstetric health: the Autism from menstruation to menopause study protocol.

INTRODUCTION: Autism is a lifelong minority neurotype present from birth. There is a dearth of credible evidence to suggest gender variation in Autism prevalence, despite historical under-diagnosis of women. Autistic people Assigned Female At Birth (AFAB) have worse physical and mental health compared with non-Autistic peers. To date, the reproductive health experiences of Autistic AFAB people have been under-investigated. METHODS AND ANALYSIS: This study aims to co-develop a quality improvement intervention to improve the reproductive health of Autistic people. The study uses Community Partn

Understanding living with tracheostomy ventilation for motor neuron disease and the implications for quality of life: a qualitative study protocol.

INTRODUCTION: Home mechanical ventilation can be used to manage symptoms of breathlessness and sustain life for people living with motor neuron disease (plwMND). In the UK, less than 1% of plwMND use tracheostomy ventilation (TV). This contrasts with some other countries, where rates are much higher. Due to a lack of evidence about its feasibility, cost-effectiveness or outcomes, TV is not covered in the UK National Institute for Health and Care Excellence guidance. Most plwMND receiving TV in the UK do so as an unplanned crisis intervention, which can lead to a prolonged hospital stay while a

COVID-19 in Pregnancy and Early Childhood (COPE): study protocol for a prospective, multicentre biobank, survey and database cohort study.

INTRODUCTION: There is limited knowledge on how the SARS-CoV-2 affects pregnancy outcomes. Studies investigating the impact of COVID-19 in early pregnancy are scarce and information on long-term follow-up is lacking. The purpose of this project is to study the impact of COVID-19 on pregnancy outcomes and long-term maternal and child health by: (1) establishing a database and biobank from pregnant women with COVID-19 and presumably non-infected women and their infants and (2) examining how women and their partners experience pregnancy, childbirth and early parenthood in the COVID-19 pandemic. Effect of tart cherry juice on risk of gout attacks: protocol for a randomised controlled trial.

INTRODUCTION: Gout is a painful form of inflammatory arthritis associated with several comorbidities, particularly cardiovascular disease. Cherries, which are rich in anti-inflammatory and antioxidative bioactive compounds, are proposed to be efficacious in preventing and treating gout, but recommendations to patients are conflicting. Cherry consumption has been demonstrated to lower serum urate levels and inflammation in several small studies. One observational case cross-over study reported that cherry consumption was associated with reduced risk of recurrent gout attacks. This preliminary e

The Imperial Prostate 9 - Approaches To Long-term Active Surveillance: Regular MRI scans versus standard of care (IP9-ATLAS) randomised controlled trial.

INTRODUCTION AND OBJECTIVES: Active surveillance is heterogenous using combinations of prostate specific antigen (PSA), digital rectal exam (DRE), magnetic resonance imaging (MRI) and biopsy. Using serial prostate MRI scans during active surveillance to inform when to conduct repeat biopsies might be effective to balance the burden of testing and detecting cancer progression early. IP9-ATLAS aims to determine whether regular MRI scans during active surveillance, compared to current standard of care (clinically-indicated MRI scans), will improve detection of cancer progression over 5 years. Optimising graft selection in ACL reconstruction (GRASE ACL): a study protocol for a randomised controlled clinical trial investigating quadriceps, hamstring and patella tendon for reconstruction in patients with primary ACL rupture.

INTRODUCTION: Anterior cruciate ligament (ACL) reconstruction (ACL-R) is recommended for active patients aiming to return to demanding sports or occupations after ACL rupture. Primary graft options are bone-patellar tendon-bone (BPTB), hamstring tendons (HT) and quadriceps tendon (QT) autografts. However, consensus regarding the optimal graft remains lacking. This trial compares graft selection effects on patient-reported knee function and neuromuscular control after ACL-R. METHODS AND ANALYSIS: The GRASE ACL trial is a single-centre assessor-blinded randomised controlled trial with a three-g

Impact of COVID-19 on neoadjuvant chemotherapy efficacy in patients with breast cancer: An ambidirectional cohort study and mendelian randomization analysis.

INTRODUCTION: Patients with breast cancer are susceptible to coronavirus disease (COVID-19), which affects cancer treatment efficacy and prognosis. However, its effects on neoadjuvant chemotherapy (NAC) response and its genetic correlation with breast cancer remain unclear. We aimed to clarify these effects and investigate potential genetic associations and shared pathogenic mechanisms. MATERIALS AND METHODS: In this ambidirectional (retrospective and prospective) cohort study involving 856 breast cancer patients receiving NAC, with and without COVID-19, we evaluated the impact of COVID-19 on

Virtual reality-based cognitive rehabilitation programme to support employment in patients with breast cancer: protocol for the Cog-RV pilot study.

INTRODUCTION: Cancer-related cognitive impairment is frequently reported by patients with breast cancer after chemotherapy. These difficulties can hinder return to work. It is therefore particularly important to assess and manage these impairments, especially to facilitate employment. We propose the Cog-VR pilot study to assess patient adherence to a virtual reality (VR)-based cognitive rehabilitation programme to support employment. METHODS AND ANALYSIS: This prospective interventional pilot study aims to assess adherence to a VR-based cognitive rehabilitation programme in patients with brea

Effect of music listening on delirium after hip fracture operations (MLDHFO) in a regional hospital in Taiwan: a randomised controlled trial protocol.

INTRODUCTION: Postoperative delirium is a serious complication occurring in 10.09%-51.28% of geriatric patients undergoing surgery for hip fractures. Delirium has resulted in poorer functional recovery, increased readmission rates, repeat surgeries and elevated mortality. Perioperative music listening is a promising non-pharmacological intervention with beneficial effects on delirium. This trial aims to evaluate the effect of perioperative music listening on postoperative delirium in patients with femur fracture undergoing surgery. METHODS AND ANALYSIS: The music listening on clinical outcome

Tebipenem pivoxil as an alternative to ceftriaxone for clinically non-responding children with shigellosis: a randomised non-inferiority trial protocol.

INTRODUCTION: Shigellosis is the second leading cause of diarrhoeal deaths among children worldwide. Oral azithromycin and intravenous ceftriaxone are the recommended first-line and second-line therapies for shigellosis in Bangladesh, respectively, but growing antibiotic resistance will require new antibiotic options. Tebipenem pivoxil, an orally administered carbapenem antibiotic with activity against many strains of antibiotic-resistant bacteria, may be a viable option. METHODS: A phase IIb randomised controlled trial was planned to determine the efficacy and safety of oral tebipenem pivoxi

Efficacy and long-term effects of the intervention of Zhan Zhuang Qigong on Pisa syndrome in Parkinson's disease: the study protocol for a randomised control trial.

INTRODUCTION: Pisa syndrome (PS) is a postural abnormality that frequently manifests in various neurological disorders. In recent years, the dangers associated with Parkinson's disease (PD) and PS have been increasingly acknowledged, leading to heightened research efforts directed toward the treatment of PS. Nevertheless, the underlying causes of PS remain complex, and no clinically effective treatments have been established. While several studies have demonstrated the significant effects of Qigong in treating PD, there is currently no evidence to support the efficacy of Qigong in the treatmen

Behavioural activation for people in custody with depression: A protocol for a feasibility randomised controlled study.

People in custody are at high risk of developing depression. Accessing psychological treatments in a prison setting is a particular challenge, in part, due to difficulties accessing specialist mental health workers. Behavioural Activation (BA) may be helpful in improving health outcomes for people in custody experiencing depressive symptoms. The aim of this study is to establish the feasibility and acceptability of custodial health nurses delivering BA to improve depressive symptoms of people in custody. We will conduct a pilot randomised controlled trial with process observation examining the
