The Value of a Well-Trained Ear: Incidental Detection of Catathrenia on Home Sleep Apnea Tests in Patients With Low Probability for Obstructive Sleep Apnea.

Catathrenia is a rare sleep-related breathing disorder marked by nocturnal groaning during prolonged exhalation. Home sleep apnea tests are growing in popularity, but no prior case reports exist documenting catathrenia detected by a home sleep apnea test. We report two cases of catathrenia incidentally detected in patients with low to intermediate risk of obstructive sleep apnea, in which diagnostic polysomnography was recommended but denied by insurers. Events were automatically scored as central apneas, leading to diagnostic uncertainty. The key to identification of catathrenia in both patie…

Algebraic approximation of the distributed model for the pressure drop in the respiratory airways.

The complexity of the human respiratory system causes that one of the main methods of analyzing the dynamic pulmonary phenomena and interpreting experimental results are simulations of its computational models. Among the most compound elements of these models, apart from the bronchial tree structure, is the phenomenon of flow limitation in flexible bronchi, which causes them to collapse with increasing flow, thus their properties, such as resistance, compliance and inertance, are highly nonlinear and time-varying. Commonly, this phenomenon is ignored, or a distributed model for the airway pres

A deep look into the rib cage compression technique in mechanically ventilated patients: a narrative review.

Defective management of secretions is one of the most frequent complications in invasive mechanically ventilated patients. Clearance of secretions through chest physiotherapy is a critical aspect of the treatment of these patients. Manual rib cage compression is one of the most practiced chest physiotherapy techniques in ventilated patients; however, its impact on clinical outcomes remains controversial due to methodological issues and poor understanding of its action. In this review, we present a detailed analysis of the physical principles involved in rib cage compression technique performan

Carbon monoxide monitoring to objectively measure smoking status in cardiac rehabilitation.

OBJECTIVE: Participation in cardiac rehabilitation (CR) is associated with reduced morbidity and mortality. However, most programs rely on self-report measures when assessing the critical risk factor of smoking. This study examined smoking status using self-report versus objective measurement using expired carbon monoxide (CO) and compared patient characteristics by CO level. METHOD: Patients were screened for smoking status when entering CR by self-report and by objectively measured CO. Measures of aerobic fitness, educational attainment, depressive symptoms, and self-reported physical funct

Why airborne transmission hasn't been conclusive in case of COVID-19? An atmospheric science perspective.

Airborne transmission is one of the routes for the spread of COVID-19 which is caused by inhalation of smaller droplets1 containing SARS-CoV-2 (i.e., either virus-laden particulate matter: PM and/or droplet nuclei) in an indoor environment. Notably, a significant fraction of the small droplets, along with respiratory droplets, is produced by both symptomatic and asymptomatic individuals during expiratory events such as breathing, sneezing, coughing and speaking. When these small droplets are exposed to the ambient environment, they may interact with PM and may remain suspended in the atmospher

Aerosol Dynamics Model for Estimating the Risk from Short-Range Airborne Transmission and Inhalation of Expiratory Droplets of SARS-CoV-2.

The highly infectious SARS-CoV-2 novel coronavirus has resulted in a global pandemic. More than a hundred million people are already impacted, with infected numbers expected to go up. Coughing, sneezing, and even talking emit respiratory droplets which can carry infectious viruses. It is important to understand how the exhaled particles move through air to an exposed person to better predict the airborne transmission impacts of SARS-CoV-2. There are many studies conducted on the airborne spread of viruses causing diseases such as SARS and measles; however, there are very limited studies that c

European Respiratory Society clinical practice guidelines for the diagnosis of asthma in children aged 5-16 years.

BACKGROUND: Diagnosing asthma in children represents an important clinical challenge. There is no single gold-standard test to confirm the diagnosis. Consequently, over- and under-diagnosis of asthma is frequent in children. METHODS: A task force supported by the European Respiratory Society has developed these evidence-based clinical practice guidelines for the diagnosis of asthma in children aged 5-16 years using nine Population, Intervention, Comparator and Outcome (PICO) questions. The task force conducted systematic literature searches for all PICO questions and screened the outputs from

Transport abnormalities from single-breath dynamics of Ar, CO2 and O2.

A model framework is developed for analyzing non-invasive measurements of Ar, CO2, O2, and lung volume during a single exhalation from healthy and diseased lungs. The objective is to distinguish ventilation inhomogeneity from gas-exchange limitations associated with alveolar-capillary transport and processes within the blood. Several practical, quantitative indices are compared to provide distinction and physiological interpretation in terms of rate-determining transport processes. Some indices arise from a model in which the composition in the alveolar space differs from that of the exhaled g…

Prevalence and risk factors for allergic rhinitis in two resource-limited settings in Peru with disparate degrees of urbanization.

BACKGROUND: Allergic rhinitis is a disease with a high global disease burden, but risk factors that contribute to this condition are not well understood. OBJECTIVE: To assess the prevalence and risk factors of allergic rhinitis in two Peruvian populations with disparate degrees of urbanization. METHODS: We conducted a population-based, cross-sectional study on 1441 children aged 13-15 years at enrollment (mean age 14.9 years, 51% boys) to investigate the prevalence of allergic disease. We used a standardized, Spanish validated questionnaire to determine the prevalence of allergic rhinitis an

Volatilomics for breast cancer diagnosis: A narrative review across biological matrices and clinical applications.

Breast cancer (BC) is the leading cause of cancer-related mortality among women worldwide. Early, non-invasive, and accurate detection remains critical to reducing mortality and improving clinical outcomes. Volatilomics, focused on the characterization of volatile organic compounds (VOCs) emitted by the human body, offers a promising approach for disease diagnosis. VOC-based testing has already been translated into routine clinical practice for a number of illnesses, yet such tests for BC remain unavailable. To accelerate translational progress within this field, a comprehensive synthesis of a

Potential of the electronic nose to diagnose psoriatic arthritis in different settings.

Early detection of psoriatic arthritis (PsA) is challenging because of its varied manifestations and diagnostic complexity. Disease-related factors such as inflammation influence metabolic processes and are detectable in volatile organic compounds (VOCs) in exhaled breath. This study explores the potential of an electronic nose (e-nose) for non-invasive PsA detection through exhaled breath analysis. Breath samples were collected from 57 outpatients with PsA and 180 healthy controls between November 2021 and March 2024. Sensor data were processed using the Aethena software package, and a Random

Breathprint of Pulmonary Sarcoidosis: Development of a Non-Invasive Diagnostic Tool.

BACKGROUND: Sarcoidosis is a systemic inflammatory disease of unknown origin, primarily affecting the lungs and lymphatic system. Diagnosis relies on clinical assessment, histological confirmation of non-necrotizing granulomas, and exclusion of other conditions. However, current diagnostic methods are invasive and costly, making them unsuitable for disease monitoring. Existing biomarkers lack accuracy and clinical utility. OBJECTIVE: To evaluate exhaled breath analysis as a non-invasive, real-time diagnostic and monitoring tool for pulmonary sarcoidosis, by identifying disease-specific metabo
