Biomechanical characteristics of Sanders type II and III calcaneal fractures fixed by open reduction and internal fixation and percutaneous minimally invasive fixation.

BACKGROUND: This work investigated the differences in the biomechanical properties of open reduction and internal fixation (ORIF) and percutaneous minimally invasive fixation (PMIF) for the fixation of calcaneal fractures (Sanders type II and III calcaneal fractures as examples) through finite element analysis. METHODS: Based on CT images of the human foot and ankle, according to the principle of three-point fixation, namely the sustentaculum tali, the anterior process and the calcaneal tuberosity were fixed. Three-dimensional finite element models of Sanders type II and III calcaneal fractur

Long-term outcome of fibula flaps in complex limb reconstruction compared to alternative procedures, a median follow up of 41 months.

INTRODUCTION: Complex open long bone fractures present a multidisciplinary surgical challenge. Various treatment options are discussed with no consensus. Fibula flaps (FF) are frequently used in maxillofacial surgery, however their use in limb injuries is less common. With the tremendous improvement in microsurgery, orthoplastic surgery gained importance. Our retrospective study aims to assess the long-term results and the quality of life of the patients operated by FF for complex traumatic limb reconstruction. PATIENTS AND METHODS: We conducted an observational mono centric retrospective stu

Double-Barreled Vascularized Fibula Free Flap in Pediatric Tibial Bone Defect Reconstruction: A 20-Year Monocentric Experience.

INTRODUCTION: Reconstructing large bone defects for lower limb salvage in the pediatric population remains challenging due to complex oncological or septic issues, limited surgical options, and lengthy procedures prone to complications. The vascularized double-barreled fibula free flap is pivotal for reconstructing large bones. In this article, we report our experience with this technique in the surgical management of pediatric tibial bone defects. MATERIALS AND METHODS: We conducted a retrospective analysis of patients under 18 years of age who underwent tibial reconstruction using a double-

Improving mandibular reconstruction by using topology optimization, patient specific design and additive manufacturing?-A biomechanical comparison against miniplates on human specimen.

In this study, topology optimized, patient specific osteosynthesis plates (TOPOS-implants) are evaluated for the mandibular reconstruction using fibula segments. These shape optimized implants are compared to a standard treatment with miniplates (thickness: 1.0 mm, titanium grade 4) in biomechanical testing using human cadaveric specimen. Mandible and fibula of 21 body donors were used. Geometrical models were created based on automated segmentation of CT-scans of all specimens. All reconstructions, including cutting guides for osteotomy as well as TOPOS-implants, were planned using a custom-m

Determinants of surgical necessity in Danis-Weber a lateral malleolus fractures: Prognostic insights from a nonoperative cohort.

BACKGROUND: Danis-Weber Type A lateral malleolus fractures are conventionally deemed stable and managed nonoperatively, yet prognostic heterogeneity leads to chronic complications in subsets of patients, revealing limitations in uniform treatment paradigms. This study introduces a novel subclassification to identify soft-tissue-driven risks and refine surgical thresholds in a nonoperative cohort. METHODS: In this retrospective cohort of 434 skeletally mature patients with acute Danis-Weber Type A fractures initially treated nonoperatively, we subclassified injuries via 3D-CT into Subtype A1 (

Aseptic forearm nonunions treated by plate and opposite fibular autograft strut.

UNLABELLED: Forearm nonunion frequently changes the relationship between the radius and ulna and may lead to impairment of forearm function. We propose a new surgical technique for aseptic forearm nonunions combining a fibular cortical autograft strut with a metal plate and a fibular intercalary autograft in cases with a segmental bone defect. We retrospectively reviewed 20 patients with a mean age of 31 years (range, 17-48 years) at the time of surgery. Minimum followup was 12 years (mean, 14 years; range, 12-21 years). There were no intraoperative or postoperative complications. At last foll

Metaphyseal fractures mimicking abuse during treatment for clubfoot.

BACKGROUND: Metaphyseal injuries resembling the classic metaphyseal lesion (CML) of abuse may occur as the result of serial casting during treatment of clubfoot deformity. Mentioned in the orthopedic literature in 1972, this iatrogenic fracture has not been described in the radiologic literature nor has the similarity to injuries occurring with abuse been previously recognized. OBJECTIVE: To describe the mechanism and radiographic appearance of metaphyseal injury observed during serial casting of clubfoot. Note similarities to the CML of abuse. MATERIALS AND METHODS: Eight children ranging i

[The fibula-pro-tibia operation--indications, results].

We report on 21 cases with combined soft tissue and far reaching bone defects of the lower leg. The bone defects were bridged by the fibula-pro-tibia-operation. In eleven patients the restoration of the full weightbearing leg was achieved. Five patients have to use a supporting walking splint at the end of the medical treatment. In four cases we found fistulas without secretion. Five amputations of the lower leg were unavoidable.

Chronic osteomyelitis treated with a muscle flap.

The use of local muscle flaps in the treatment of chronic osteomyelitis is outlined. The defect treated must be in an anatomic location that permits rotation of the muscle flap. A recent review of 42 patients treated with this technique showed the local muscle flap to be efficacious in 39 of the 42 patients treated.

Evidence into practice: pediatric orthopaedic surgeon use of removable splints for common pediatric fractures.

OBJECTIVES: Removable splints when compared with circumferential casts in randomized trials have been shown to be a safe and cost-effective method of managing many common minor distal radius and fibular fractures. This study estimated the extent to which this evidence is being implemented in clinical practice, and determined the perceived barriers to the adoption of this evidence. METHODS: A cross-sectional survey of practicing orthopaedic surgeon members of the Pediatric Orthopedic Surgeons of North America (POSNA) was conducted, using a 22-item online questionnaire, and distributed using a

Transepiphyseal (Type VII) Ankle Fracture Versus Os Subfibulare in Pediatric Ankle Injuries.

BACKGROUND: Pediatric ankle injuries are common, giving rise to ∼17% of all physeal injuries. An os subfibulare in a child with an ankle sprain may be confused with a type VII transepiphyseal fracture. Here, we evaluate the clinical and radiographic features of type VII transepiphyseal fractures to those of os subfibulare presenting with acute ankle trauma with the hypothesis that radiographs are necessary for final diagnosis and neither clinical history nor examination would be diagnostic. METHODS: We performed an internal review board-approved, retrospective chart review of patients identif

Fibular strut graft for humeral aneurysmal bone cyst with varus deformity.

PURPOSE: Proximal humerus is a common site for ABC and frequently associated with varus deformity that limits shoulder abduction. A prospective study was conducted to evaluate the use of intramedullary non-vascularised autogenous fibular strut graft for reconstruction without internal fixation. METHODS: A total of 20 patients (12 girls, 8 boys) were managed for proximal humeral ABC with varus deformity by extended curettage, osteotomy, intramedullary fibular graft and composite bone substitute. Their ages ranged from 10 to 17 years (average, 13.3 years). The lesion was juxtaphyseal in 16 pati

Reconstruction of lower limb bone defects after sarcoma resection in children and adolescents using free vascularized fibular transfer.

This study included 11 patients with lower limb tumors who had wide local resection and reconstruction by vascularized fibula osteoseptocutaneous flap and their surgery performed at least 24 months before the end of the study. The average age at operation was 14.4 years. All tumors, except one, were high-grade sarcomas (stage IIA, IIB, and Ewing's sarcoma). The tumor volume averaged 200.6 cm3 (range 41.5-400). The resulting defect after tumor resection averaged 14.8 cm (range 9-20). The fibula was inserted as a single strut in eight patients and as a double-barrel construct in three patients.
