The palmoplantar keratodermas (PPKs) are a heterogeneous group of disorders of cornification affecting the palms and soles. BACKGROUND: Mycetoma is a chronic subcutaneous mycosis caused by exogenous fungi or actinomycetes. This infection has a progressive course and shows a typical clinical characteristic of tumefaction, draining sinuses, and grains. Infection initiation is related to local trauma and can spread to muscle, underlying bone, and adjacent organs. Erysipeloid is an acute, bacterial infection of traumatized skin in an individual who was in direct contact with meat or other animal products contaminated with a gram-positive bacillus Erysipelothrix rhusiopathiae. We present a case of a 50-year-old housewife whose hobby was fishing, with a reddish, tender patch on the fifth finger and dorsum of the left hand, which developed a week after she had sustained an injury while boning the fish. The patient was treated with orally administered penicillin V 1,500,000 IU t.i.d. for 7 days, with complete resolution.

Pimecrolimus is a calcineurin inhibitor developed for the topical therapy of inflammatory skin diseases, particularly atopic dermatitis (AD). Pimecrolimus selectively targets T cells and mast cells. A case of fingernail infection by Fusarium is presented. This nondermatophytic mold is an infrequent cause of onychomycosis, more typically involving the great toenail. Characteristic histologic features including the presence of hyphae and chlamydoconia are helpful in rapid diagnosis and selection of appropriate antifungal therapy. Although Fusarium has shown resistance to most antifungal medications in vitro, intermittent therapy with fluconazole led to improvement in this patient.

We present a case of cutaneous infection due to Cladophialophora bantiana, an agent of phaeohyphomycosis.
