Sutureless aortic valve replacement: catheter-based transapical versus direct transaortic implantation.

BACKGROUND AND AIM OF THE STUDY: Catheter-based aortic valve implantation leaves the calcified native leaflets in the aortic root. Upon valve deployment, the leaflets are pushed to the side, potentially compromising coronary perfusion, exerting pressure on the conduction system, and limiting the maximum effective orifice area. The study aim was to evaluate the perioperative outcome of an alternative self-expanding sutureless valve that is inserted via a conventional aortotomy, allowing for complete debridement of any calcified native aortic leaflets and annulus. METHODS: A total of 56 patient

Feasibility and challenges of single-session percutaneous double valve in valve implantation in the elderly.

The 2021 ESC/EACTS guidelines recommend percutaneous approaches for failing bioprostheses. These methods are less invasive, resulting in shorter recovery times and fewer complications. Valve-in-valve (ViV) procedures offer a promising solution, although simultaneous transcatheter replacement of two valves (double ViV) still needs to be standard and primarily documented through case reports. This case series presents the initial outcomes of two patients who underwent simultaneous double transcatheter heart valve implantation, exploring the feasibility of this approach in the elderly. As the dem

Successful transcatheter aortic valve replacement in a failing HAART 300 aortic valve annuloplasty ring: A case report.

Transcutaneous aortic valve replacement (TAVR) has evolved from a complex procedure meant only for patients at prohibitive risk for surgery to a commonly performed procedure across a wide variety of clinical scenarios including the treatment of failed aortic valve bioprosthesis. Annuloplasty rings in the aortic position such as HAART 300 (Biostable Science and Engineering) have been introduced in the management of native aortic regurgitation. Percutaneous management of failed bioprosthesis rings in the aortic position has not been widely described. We present a case of a 69-year-old man with r

Clinical outcomes of Carbomedics Top Hat valve with a robust follow-up system.

AIM: Late failure of bioprosthetic valves may limit their use in patients < 60 years. The superior hemodynamic performance offered by the Carbomedics Top Hat supraannular valve enables greater effective orifice areas to be achieved. The aim of this study was to assess the clinical outcomes of this valve, using a robust follow-up system. METHODS: Patients who underwent aortic valve replacement with or without coronary artery bypass grafting between July 1997 and January 2010 with Carbomedics supraannular Top Hat valves were identified. Details of readmissions and late deaths were obtained from

Striving for physiologic coaptation: An experimental and innovative approach towards designing and implanting aortic neo-leaflets.

Aortic valve repair has emerged as the treatment of choice for congenital aortic valvular disease, avoiding the need for a reoperation associated with stented prosthesis overgrowth. The introduction of a leaflet implant represents a recent advancement in a field that originated early techniques, such as simple commissurotomies. In our experimental approach, we assessed two established leaflet-sizing techniques by analysing their resultant coaptation areas. Although both techniques produced competent valves, the large coaptation areas differed significantly from the native aortic valve. This ob

Development of transcatheter implantable autologous tissue-engineered pulmonary valves using in-body tissue architecture.

Transcatheter pulmonary valve implantation (TPVI) is a minimally invasive procedure used to treat pulmonary valve dysfunction in congenital heart disease. However, the limited durability of xenogeneic bioprosthetic valves is a significant concern. Tissue-engineered heart valves (TEHVs) have emerged as a promising alternative. We developed an autologous TEHV, Biovalve, using an in-body tissue architecture technology based on tissue encapsulation. In this study, we aimed to develop stent-integrated Biovalves (stent Biovalves) that can be applied to TPVI. We designed an asymmetric hourglass-shape

Transjugular Transcatheter Tricuspid Valve Implantation of LuX-Valve Bioprosthesis in a Preclinical Model.

The purpose of this preclinical study in a sheep model was to confirm the feasibility and safety of the LuX-Valve transjugular tricuspid valve (TV) replacement apparatus and to optimize the implantation procedure before beginning first-in-man study. The LuX-Valve was implanted in a sheep model (n = 8) via transjugular approach. Six of eight sheep underwent successful implantation procedure on beating heart. The first two sheep died during the prostheses deployment. In the remaining 6 sheep that survived, postoperative echocardiography results showed there was no paravalvular leakage (PVL) and

Echocardiographic Assessment of Melody Versus Sapien Valves Following Transcatheter Pulmonary Valve Replacement.

OBJECTIVES: The aim of this study was to compare the immediate and midterm echocardiographic performance of the Melody (Medtronic Inc) and Sapien (Edwards Lifesciences Inc) valves after transcatheter pulmonary valve replacement (TPVR) in native and conduit right ventricular outflow tracts (RVOTs). BACKGROUND: TPVR is now a common procedure, but limited data exist comparing postimplantation echocardiographic findings between Melody and Sapien valves. METHODS: This was a single-institution retrospective cohort study of all patients who underwent successful TPVR from 2011 to 2020. Patient demog

Mitral Valve-In-Valve: Defining the Indication Limits by in vitro Hydrodynamic Tests in a Brazilian Transcatheter Prosthesis.

INTRODUCTION: Reoperations in cardiac surgery represent a clinical challenge, particularly because of the higher rate of perioperative morbidity and mortality. Mitral valve reoperation owing to bioprosthesis dysfunction, transcatheter treatment with a prosthesis implantation over the prosthesis has emerged as an alternative, especially for patients with a previous approach. In this study, we analyzed the hydrodynamic behavior of transcatheter prosthesis implantation in conventional mitral bioprostheses through hydrodynamic tests and produced a recommendation for the size of transcatheter valve

Use of RailwayTM Dilator as a Novel Technique to Cross Prosthetic Aortic and Mitral Paravalvular Leak.

Percutaneous endovascular repair of prosthetic paravalvular leaks (PVLs) has emerged as a safer, and less invasive option preferred for many patients. Crossing the PVL with a guide catheter (GC), or guide sheath is one of the challenging aspects of this technique and at times require arteriovenous (AV) rail creation. AV rail creation requires additional arterial access, snaring, and externalizing of a wire, all of which are associated with unique challenges and complications risk. Despite using these techniques, nearly 8% of the PVL closure procedures are unsuccessful. We report two cases with

Transarterial aortic valve replacement with a self expanding stent in pigs.

OBJECTIVES: To evaluate the feasibility of percutaneous aortic valve replacement without cardiac arrest in animal experiments. METHODS: A self expanding nitinol stent, containing pulmonary valves from pigs in its proximal part, was implanted in six pigs (94-118 kg) by means of a 25 French catheter through the left subclavian artery under guidance of fluoroscopy and transoesophageal echocardiography. During stent deployment the original aortic valve was pushed against the aortic wall by the self expanding force of the stent while the new valve was expanded. RESULTS: It was possible to replace
