MUC4 mutations promote a thrombotic phenotype in patients with paroxysmal nocturnal haemoglobinuria by increasing the deposition of terminal complement.

BACKGROUND: Thrombosis is a common complication in paroxysmal nocturnal haemoglobinuria (PNH) patients, but primary prevention remains controversial. Identifying high-risk individuals could enable risk-stratified prophylactic anticoagulation strategies. METHODS: We analyzed clinical data from PNH patients with or without thrombosis, including MUC4 mutation status and serum complement C5b-9 levels. Complement deposition assays and a murine lower limb deep vein thrombosis model were used to investigate the role of MUC4 mutation in thrombotic risk and explore the underlying mechanism involving t

Bone Marrow Failure Syndromes: Clinical and Genetic Characterization in TERT-associated Dyskeratosis Congenita.

BACKGROUND: Bone marrow failure syndromes (BMFs) comprise a heterogeneous group of genetic disorders characterized by impaired hematopoiesis and multisystem involvement. Dyskeratosis congenita (DC) is a telomere biology disorder caused by defects in telomerase or telomere maintenance, leading to progressive BMF and variable extra-hematopoietic manifestations. OBJECTIVES: To describe the clinical, immunologic, genetic, and telomere biology findings in a patient with DC caused by a novel biallelic telomerase reverse transcriptase (TERT) mutation, and to highlight diagnostic and therapeutic cons

[Severe haemolytic anaemia and hepatitis in the course of Epstein-Barr virus infection].

Epstein-Barr virus (EBV) is globally prevalent and in adolescents mostly observed as infectious mononucleosis. Abnormal liver blood tests are common, whereas more serious hepatitis is less prevalent. Autoimmune haemolytic anaemia may also occur in the course of this infection. We report a case of a 15-year-old girl with cholestatic hepatitis and autoimmune haemolytic anaemia associated with EBV infection. The Donath-Landsteiner test was positive suggesting paroxysmal cold haemoglobinuria. She was treated with supportive care and discharged in recovery after three weeks.

Macroscopic haemoglobinuria associated with Mycoplasma pneumoniae infection successfully treated by clarithromycin.

A 25-year-old man developed macroscopic haemoglobinuria after a persistent dry cough. Although chest radiograph findings were normal, since the serum antibody for Mycoplasma pneumoniae was significantly elevated, a diagnosis infection with this organism was made. Despite the absence of apparent anaemia, a marked increase in serum haemolytic markers and positive result for urine haemoglobin indicated the haemolysis of red blood cells, which was likely to have occurred secondarily to M. pneumoniae infection. Shortly after the initiation of a macrolide antibiotic, clarithromycin, the patient's ha

Haemolytic anaemia--PNH type II cells presenting a diagnostic dilemma.

A 28-year-old man presented with a history of intermittent haematuria over the past 10 years usually following fever episodes and requiring blood transfusions during the episodes. History of any thrombotic complications, chest pain or erectile dysfunction was not forthcoming. Examination revealed severe pallor with mild icterus and mild splenomegaly. His blood picture showed pancytopenia with elevated reticulocytes and indirect hyperbilirubinaemia. Indirect Coombs test was positive but direct was negative, serum lactate dehydrogenase was elevated and agglutinins were found to be of IgG type. B

Childhood paroxysmal nocturnal haemoglobinuria (PNH), a report of 11 cases in the Netherlands.

Paroxysmal nocturnal haemoglobinuria (PNH) is characterized by intravascular haemolysis, nocturnal haemoglobinuria, thrombotic events, serious infections and bone marrow failure. This acquired disease, caused by a deficiency of glycosylphosphatidylinositol (GPI) anchored proteins on the haematopoietic cells, is rare in children. We describe 11 Dutch paediatric PNH patients (median age: 12 years, range 9-17 years) diagnosed since 1983, seven cases associated with aplastic anaemia (AA), four with myelodysplastic syndrome (MDS). Presenting symptoms were haemorrhagic diathesis (n = 10), palor/tire

Sclerotherapy for venous malformations of the glans penis.

OBJECTIVES: To retrospectively evaluate the safety and efficacy of percutaneous sclerotherapy with polidocanol in 9 patients with venous malformations of the glans penis. Vascular malformations of the external genitalia can be aesthetically and functionally disabling. These lesions are rare and their treatment is still controversial. METHODS: Seven patients were asymptomatic, seeking advice for cosmetic reasons, and 2 patients had experienced several bleeding episodes during sexual intercourse. In all patients, Doppler ultrasound scanning was performed before treatment. All patients underwent

Increased urinary haemoglobin in diabetics with microalbuminuria--measured by an ELISA.

A solid-phase sandwich enzyme immunoassay for the determination of urinary haemoglobin is described. A screening study was performed to establish whether occult haematuria/haemoglobinuria is present in diabetic patients with elevated urinary albumin excretion. Non-insulin-dependent diabetic patients (145) aged 66.5 years +/- 5.6 with a known duration of diabetes of 10.4 years +/- 6.8 were studied. They delivered a first morning urine sample at consecutive outpatient visits. Erythrocytes were lysed by freezing of the urine samples. The patients were categorized into three groups according to ur

[Marchiafava-Micheli syndrome and pregnancy].

Paroxysmal nocturnal hemoglobinuria (PNH) is an acquired clonal disorder characterized by sensitive populations of erythrocytes, granulocytes and platelets. PNH is a disease of young adults with a slight female predominance. Several complications of PNH during pregnancy, could be prevented; chronic anemia, folate and iron deficiency, deep vein thrombosis. We report seven pregnancies, six of which were successful in four patients. One pregnancy was terminated after 25 weeks by a fetal death during an acute hemolytic crisis. Diagnosis of PNH was made in the four patients before the pregnancy by …

Bovine post-parturient haemoglobinuria: effect of inorganic phosphate on red cell metabolism.

Washed red blood cells from normal cows were incubated as 10 and 20 per cent suspensions in media containing 0, 2.5 and 25 mM phosphate. The results showed that the rate of glycolysis was dependent on the inorganic phosphate concentration. In the absence of phosphate, the consumption of glucose and the production of 2,3-diphosphoglycerate, lactate and adenosine 5'-triphosphate (ATP) were decreased. Incubation without added phosphate also greatly increased the production of fructose-1,6-diphosphate, glyceraldehyde-3-phosphate and dihydroxyacetone phosphate. Moderate hypophosphataemia was induce

Lymphocyte characteristics and function in paroxysmal nocturnal haemoglobinuria.

In six patients with paroxysmal nocturnal haemoglobinuria (PNH) lymphocyte studies were performed to investigate whether the observed immunological dysfunction could be ascribed to a defect in lymphocytes as result of the PNH characteristics, or to an imbalance between T-cell subsets. The PNH characteristics were studied by means of the effect of serum and acidified serum on Indium111-oxine labelled lymphocytes. No increase in release of Indium111-oxine was found when lymphocytes were exposed to acidified sera. Thus a complement mediated lymphocyte lysis in PNH could not be demonstrated. T-cel

In vitro production of PNH-like red blood cells by 2-mercaptopropionylglycine.

2-Mercaptopropionylglycine (2-MPG) transformed normal red blood cells (RBCs) into paroxysmal nocturnal haemoglobinuria (PNH)-like RBCs in vitro, depending on the concentration, pH and time of incubation. The incorporation of radioactive choline in the presence of acetylcholine was reduced, as in RBCs treated with aminoethylisothiouronium salt (AET). In contrast, the uptake in the presence of choline differed when the RBCs were incubated with the two compounds, being reduced in AET-treated RBCs and increased in 2-MPG-treated ones. As true PNH RBCs incorporated to a higher extent the radioactivi

Paroxysmal nocturnal haemoglobinuria in pregnancy: case report.

Pregnancy in a woman with paroxysmal nocturnal haemoglobinuria was complicated by hepatic venous thrombosis, portal hypertension and haematemesis due to gross oesophageal varices. Labour was induced and a live female infant was born. Dural sinus thrombosis and intracerebral haemorrhage resulted in death on the 46th day post partum. A review of the literature shows that venous thrombosis is common in paroxysmal nocturnal haemoglobinuria.

Nephrotoxicity of low molecular weight serum proteins: physicochemical interactions between myoglobin, hemoglobin, bence-jones proteins and tamm-horsfall mucoprotein.

Three types of low molecular weight serum proteins, myoglobin, hemoglobin and BENCE-JONES proteins, are associated clinically with acute renal failure. All have isoelectric points which render them anionic at blood pH but cationic in the distal nephron under conditions of aciduria. Experiments in which these proteins were mixed with TAMM-HORSFALL mucoprotein in vitro and the pH lowered with lN HCl showed co-precipitation of proteins at pH levels of 5.5 and below. In vivo experiments in which 11 different BENCE-JONES proteins of pl ranging from 5.2 to 6.6 were injected into aciduric, hydropenic
