The authors take into consideration the different phases of the treatment of congenital hip dysplasia (CHD) as it is related to age and natural development. Within the first year of age three objectives must be aimed at: reduction of the epiphysis, its retention and immobilization, maturation of the hip. Careful clinical examination remains the primary and most important way of diagnosing developmental dysplasia of the hip (DDH) in newborn infants. Repeated examinations during the first year are important to diagnose DDH subsequent to the newborn period. Sonography can detect cases of clinically silent DDH. Targeting high-risk infants for supplemental ultrasound screening at 4 to 6 weeks of age increases diagnosis of DDH and at significantly less expense than widespread screening. I describe an anterolateral approach to the hip joint. The intermuscular plane is between the gluteus medius and tensor fascia lata. The anterior one third of gluteus medius and the insertion of gluteus minimus are stripped from the anterior aspect of the greater trochanter to raise a flap in a way that facilitates reattachment. BACKGROUND: Hip fracture is the complication with the greatest medical repercussions in osteoporosis. The aim of this study was to know both the incidence of osteoporotic hip fracture in Asturias, Spain and the immediate evolution of these patients.
METHODS: The clinical histories of all the patients admitted for hip fracture in 1992 in two Asturian public health care areas were reviewed. Hip fractures in patients under the age of 45 years or those occurring because of other diseases (metastasis, serious injury) were excluded.

A retrospective study was undertaken of 24 infants with septic arthritis of one or both hips who presented to the Children's Hospital, Camperdown, during the 15 year period from 1973 to 1989. Staphylococcus aureus was found to be the most common pathogen. Umbilical catheterization was a significant aetiological factor in seven of 10 neonates studied. In most cases congenital dislocation of the hip is detected at birth or during the first weeks of life. Forced abduction by means of breeches or wrappage or with Pavlik harness has replaced continuous traction which is useful after 6 months. These measures must be applied according to strict rules, but they do not prevent the occurrence of complications, including post-reductional osteochondritis. Their pros and cons therefore must be weighed with extreme care.

This report deals with the data gathered for a year concerning the incidence of luxation coxae congenita at the Hospital de Gineco-Obstetricia (Gynecology-Obstetrics Hospital) of the Unidad Médica Oblatos (Oblatos Medical Unit) of the Instituto Mexicano del Seguro Social (Mexican Institute of Social Security) in the city of Guadalajara, Jalisco, México. The results show an inverse correlation between the temperature and the number of cases, both statistically significant, demonstrating seasonality for luxation coxae congenita.

Hip arthroscopic examination of 196 joints was performed in 104 patients treated during the past 4 years. Of these, 11 joints were treated by arthroscopic surgery. We have developed a technique using a two-directional approach that facilitates a global view of joint areas and allows simpler performance of surgical procedures. Removal of loose bodies, joint debridement in osteoarthritis, and synovectomy in rheumatoid arthritis are good indications for arthroscopic surgery of the hip joint. We describe a patient with a Neisseria gonorrhoeae monoarthritis of the hip. Treatment with intravenous ceftriaxone, oral doxycycline, and repeated fluoroscopic needle aspirations resulted in a complete recovery of function without residual deficit. Gonococcal monoarthritis of the hip is rare. Gonococcal hip infections appear to respond well to antibiotics and drainage by arthrocentesis. This differs from hip infections caused by other bacteria where joint damage is common and where the recommended initial treatment is open surgical drainage.

Hip fractures account for fewer than 1% of all fractures in children, and many can be successfully treated nonoperatively. Transepiphyseal, transcervical, and displaced cervicotrochanteric fractures, however, generally require closed reduction or open reduction and internal fixation to avoid complications of coxa vara deformity and nonunion. Avascular necrosis appears to be related to the severity of the initial injury and unaffected by treatment.
