Hyperendemic West African trypanosomiasis in a rural hospital setting.

Over 60 cases of trypanosomiasis presented to Eku Hospital between May 1983 and May 1985. During this time, within this previously unreported hyperendemic focus, 47 patients were documented microscopically. Their clinical characteristics are reported here. Seventy-four per cent were felt to have CNS involvement on presentation. Compliance with the therapeutic regimen was a major problem, requiring adjustments in the usually accepted management of this disorder.

Blast injury with particular reference to recent terrorist bombing incidents.

The aetiology of primary blast lung is discussed with reference to the biodynamics of blast injury, and the clinical and pathological features of the condition are described. An analysis of casualties from bomb blast incidents occurring in Northern Ireland leads to the following conclusions concerning the injuries found in persons exposed to explosions: (1) there is a predominance of head and neck trauma, including fractures, lacerations, burns, and eye and ear injuries; (2) fractures and traumatic amputations are common and often multiple; (3) penetrating trunk wounds carry a grave prognosis;

Operating in recurrent crises: a qualitative study of decision-making and unintended consequences in a peripheral hospital.

BACKGROUND: During crises, decision-makers often operate under uncertainty, relying on limited evidence and intuition, increasing the risk of unintended consequences. In Israel, hospitals have recently faced back-to-back crises: the COVID pandemic and the war following October 7th 2023. OBJECTIVE: This study aimed to explore hospitals' decision-making processes during crises and how managing one crisis influenced responses to subsequent crises. METHODS: We conducted a qualitative study guided by a modified version of the World Health Organization's six building blocks framework, and intervie

A Lean-based Digital Twin model for planning and governance in secondary and tertiary hospital networks.

BACKGROUND: Healthcare systems face growing pressure and recurrent capacity-demand imbalances across hospital networks. While Lean Healthcare improves patient flow, most implementations remain confined to departmental silos and individual facilities. Simultaneously, Digital Twin (DT) applications in healthcare predominantly focus on precision medicine or isolated unit optimization, leaving a gap in system-level resource management. This study aims to propose a Lean-grounded DT model to support integrated activity and capacity planning across multi-provider hospital networks. METHODS: Situated

Prophylactic Inhaled Antibiotics for Ventilator-Associated Pneumonia: A Systematic Review and Meta-Analysis of Incidence and Mortality Outcomes.

PURPOSE: Ventilator-associated pneumonia (VAP) is a common ICU complication linked to high morbidity and mortality. Inhaled antibiotics may offer targeted prophylaxis, but their effectiveness has shown mixed results. This study aims to further evaluate whether inhaled antibiotics reduce VAP incidence and ICU mortality through a systematic review and meta-analysis of the most updated available evidence. METHODS: A systematic review was conducted following PRISMA 2020 guidelines. Multiple databases were searched for studies published up to November 14, 2024. Ten studies including 2876 patients

Investing in Well-being: A Hospital-Based Ethics Fellowship for Nurse Leaders.

Frontline nurse leaders often encounter ethically complex issues but may lack adequate formal training in ethics, communication, and leadership, hindering their ability to navigate these challenges. The COVID-19 pandemic exacerbated moral distress, affecting leaders and staff and compromising nurse well-being and patient care. Ethics and leadership-focused professional development improves leaders' confidence and well-being and increases staff retention and engagement. These authors describe an adaptable, yearlong fellowship designed to increase ethics and leadership skills while reducing mora

A new medication, a new toxidrome - A case report of anticholinergic wipe toxicity due to improper medication use.

We describe a case of a young female who presented to the emergency department with 4 days of progressive myopia, dry mouth, anhidrosis and urinary hesitancy due to overuse of a new topical anticholinergic wipes, glycopyrronium tosylate (GT). In the United States medication misuse accounts for nearly 10% of pediatric emergency visits with 65% of these visits considered to be preventable [1]. Being familiar with new medications and their side effect profiles can prevent unnecessary or harmful interventions.

The first use of artificial intelligence (AI) in the ER: triage not diagnosis.

Predictions related to the impact of AI on radiology as a profession run the gamut from AI putting radiologists out of business to having no effect at all. The use of AI appears to show significant promise in ER triage in the present. We briefly discuss the emerging effectiveness of AI in the ER imaging setting by looking at some of the products approved by the FDA and finding their way into "practice." The FDA approval process to date has focused on applications that affect patient triage and not necessarily ones that have the computer serve as the only or final reader. We describe a select g

Use of Atomized Intranasal Tranexamic Acid as an Adjunctive Therapy in Difficult-to-Treat Epistaxis.

There is a growing body of literature on the safe, effective use of tranexamic acid (TXA) for hemostasis in a variety of clinical settings. We present a case series of three patients with difficult-to-treat epistaxis where standard treatment methods were not effective. Using atomized intranasal TXA (ATXA) as part of a stepwise treatment approach, we were able to achieve hemostasis and manage all three cases independently, and we did so without major complications in our emergency department (ED). Given recent literature showing the underuse of TXA in combat casualties, ATXA, if formulated and

Systematic Review: Rectal Administration of Medications for Pediatric Procedural Sedation.

BACKGROUND: Per rectum (PR) medication delivery is an alternative to traditional oral (PO), intravenous (IV), or intramuscular (IM) administration of medication for procedural sedation of pediatric emergency department patients. However, many emergency physicians are unfamiliar with its use, and there are no widely adopted guidelines or reviews dedicated to this topic. OBJECTIVE: Our aim was to provide emergency physicians with an overview of PR procedural sedation medications in pediatric patients. METHODS: We performed a PubMed literature search of relevant keywords limited to studies of h

Clinical Spectrum and Management of Caustic Ingestion: A Case Series Presenting Three Opposing Outcomes.

BACKGROUND: Ingestion of caustic substances is a medical emergency in both the adult and pediatric population and is associated with high morbidity and mortality. The extent of injuries after ingestion of caustic substances depends on the nature, amount, and concentration of the agent and on the exposure time. Acutely, caustic substances may cause massive hemorrhage and gastrointestinal tract perforation; the most markedly affected cases require urgent surgical treatment. Patients surviving the initial event may present with aorto-enteric or gastrocolic fistulae, esophageal strictures, dysphag

Influenza vaccination rates for hospitalised patients: a multiyear quality improvement effort.

BACKGROUND: Influenza vaccination is the most effective method for preventing influenza virus infection. Adult hospitalised patients form a particularly high-risk group for severe influenza given their advanced age and comorbidities. We sought to improve the influenza vaccination rates of hospitalised patients at the White River Junction Veterans Affairs Medical Center. METHODS: The improvement effort started in 2007 when our baseline vaccination rate was about 60%. An interprofessional team analysed the influenza vaccination process for hospitalised patients. During the course of six influen

System-level quality improvement initiatives: lessons from Keystone and other large-scale projects.

Health care systems around the world are seeking system-level interventions to improve the quality and safety of care because of increasing awareness that many patients do not receive recommended therapies or suffer preventable complications. This has also been motivated by a drive to deliver health care more cost-effectively, and to be more accountable to payers and other stakeholders. The Keystone Project in Michigan is one example of a large-scale system-level initiative that successfully changed the "culture of safety" in the intensive care units of participating hospitals and led to impro

[Does addiction to antidepressants exist? About a case of one addiction to tianeptine].

We report on a tianeptine dependence lasting for eighteen months in a 42 year old patient. The patient had a previous history of addiction to opiates, amineptine, cocaïne and alcohol. He also had a family history of addiction to alcohol and opiates. Tianeptine was prescribed for a major depressive disorder. The patient alleged a "flash sensation" like with heroin since the very first doses with a physical and psychological well-being sensation, better psychomotor performances and transient mood elation. His addiction to tianeptine was immediate and heavy. The positive reinforcement faded away
