Intradermal application of yeast phase paracoccidioidin antigen obtained by sonication and lysis from the fungus to 111 residents of Veraguas province (68 men and 43 women) and 130 Cocle province residents (57 men and 73 women) elicited a positive reaction in 9.01% (1/111) and 26.15% (34/30) of the Veraguas and Cocle residents respectively. A case of cold urticaria, thought to be due to IgM, is presented. Passive transfer (PT) activity was manifested in cryoglobulin-free serum and in up to 20-fold dilutions of fraction I, the IgM-containing fraction obtained by Sephadex G-200 column chromatography. PT test results were positive with serum absorbed with cold agglutinins, and with heat-treated (56 degrees C, 4 hr) serum. Detailed allergologic investigations, allergy histories and skin tests in 80 patients with perioral dermatitis led to the conclusion that the disease has to be considered a manifestation of atopy. Thus another clinical entity would have to be added to the hitherto known manifestations of the atopic constitution. The findings reported by other investigators may thus be interpreted as conditional or aggravating pathogenetic factors.

Following insect sting anaphylaxis, twenty-five patients of over 500 evaluated, did not have detectable serum venom-specific IgE. Twenty-two were evaluated within 1 year of the reaction, fifteen within 6 months. Anaphylaxis occurred in six patients after their first sting exposure. The clinical features of the sting reaction were typical of the reaction occurring in insect-allergic patients with serum venom-specific IgE.
