Malignant Common Bile Duct Stricture Secondary to Metastatic Squamous Cell Carcinoma of the Cervix.

Squamous cell carcinoma of the cervix (SCCC) commonly metastasizes to lymph nodes, lungs, liver, and bones, but involvement of the biliary system is rare. We present a 39-year-old woman with a history of stage IVa SCCC in remission for 18 months who developed obstructive jaundice due to a common bile duct stricture. Endoscopic retrograde cholangiopancreatography with cholangioscopy confirmed metastatic SCCC. This case highlights a rare manifestation of SCCC presenting with biliary obstruction, even at uncommon metastatic sites.

Hepatic Artery Aneurysm-A Rare Cause of Cholestasis.

Hepatic artery aneurysms (HAAs) are rare vascular malformations that can arise from atherosclerosis, trauma, or iatrogenic injury. HAAs can be symptomatic and lead to serious complications. We present the case of a patient with painless jaundice caused by obstruction of the distal common bile duct by a HAA on a replaced right hepatic artery. This was further complicated with cholangitis. After endovascular stenting of the aneurysm, cholestasis decreased. Learning point: Hepatic artery aneurysms can cause common bile duct obstruction resulting in cholestasis.

Spontaneous perforation of the extrahepatic biliary tract.

Spontaneous perforation of the extra-hepatic biliary tract presenting with pseudocyst is rare. We report the case of a two-month old infant who presented to us with increasing jaundice and progressive loss of weight. Ultrasonography, magnetic resonance cholangiopancreatography and hepatobiliary iminodiacetic acid scan revealed features of obstructed choledochal cyst. Laparotomy revealed a small, walled-offcollection of bile near the confluence of the cystic and common bile ducts. Distal obstruction was excluded and biliary tract drainage was successful in treating the infant.

Collision tumour of the ampulla of Vater: carcinoid and adenocarcinoma.

Obstructive jaundice is most commonly due to luminal stones or lesions of the head of the pancreas and more rarely ampullary and primary common bile duct lesions. Obstruction due to lesions of the ampulla of Vater may be due to adenocarcinoma which has a significantly better long term prognosis than carcinomas located in the head of the pancreas. A case is presented where two tumours were identified at the ampulla of Vater of the resected specimen one an adenocarcinoma and the other a carcinoid tumour representing a collision tumour.

Ultrasonography and percutaneous cholangiography in children with dilated bile ducts. Report of six cases.

Six children had dilation of intrahepatic and extrahepatic bile ducts. The dilation was associated with or was secondary to lithiasis in two patients, spontaneous bile duct perforation in two others, and an abnormal pancreaticobiliary junction in the last two. All the children were screened by ultrasonography; five were subsequently examined by percutaneous transhepatic cholangiography. Ultrasonography permits rapid detection of bile duct dilation, while percutaneous cholangiography aids in the choice of the surgical procedure by locating the site of obstruction and sometimes disclosing the ca

Congenital stricture of the common hepatic duct: an unusual case without jaundice.

A 3-yr 8-mo-old female was diagnosed as having a congenital stricture of the common hepatic duct. She demonstrated hepatomegaly with marked elevations of serum transaminases, alkaline phosphatase, and bile acids without clinical jaundice or hyperbilirubinemia. Liver biopsy suggested extrahepatic obstruction. Ultrasonography was nondiagnostic, but percutaneous cholangiography demonstrated blockage at the bifurcation of the hepatic ducts. A Roux-en-Y anastomosis of the jejunum to the common hepatic ducts relieved the obstruction and ameliorated clinical evidence of liver disease.

The Impact of Biliary Drainage on Quality of Life in Unresectable Biliary and Pancreatic Cancer.

Introduction Obstructive jaundice, a frequent complication of biliary tract and pancreatic malignancies, severely impacts patients' quality of life (QoL). This condition is prevalent in unresectable cancers, often causing symptoms such as pruritus, anorexia, nausea, vomiting, and cholangitis, which hinder daily activities. Given the global burden of these cancers, the primary aim of the study was to evaluate the impact of biliary drainage on QoL and pruritus in patients with unresectable biliary and pancreatic malignancies. Secondary objectives include assessing changes in individual QoL domai

A Rare Case of Ampullary Carcinoma with Complete Duodenal Obstruction as the Initial Manifestation.

INTRODUCTION: Obstructive jaundice is often the first symptom of ampullary carcinoma, with a straightforward preoperative diagnosis. We report a rare case of ampullary carcinoma without jaundice as the initial symptom. CASE PRESENTATION: A 53-year-old man was admitted with abdominal distension and recurrent vomiting. Esophagogastroduodenoscopy revealed a complete duodenal obstruction without malignant findings. Computed tomography revealed wall thickening in the second portion of the duodenum. The common bile duct and main pancreatic duct were not dilated. As there was no evidence of malignan

Adolescent Epstein-Barr virus-associated cholestatic jaundice with porta hepatis/pancreatic head lymphadenopathy and transient CA 19-9 elevation: a case report.

BACKGROUND: Cholestatic jaundice is uncommon in adolescents. Epstein-Barr virus (EBV)-related inflammatory lymphadenopathy can mimic malignant biliary obstruction and cause a transient rise in carbohydrate antigen 19-9 (CA 19-9), complicating diagnosis. CASE PRESENTATION: A previously healthy 15-year-old girl presented with 6 days of jaundice and dark urine without fever. Physical examination showed moderate generalized jaundice with mild scleral icterus; multiple mobile, non-tender lymph nodes in the bilateral cervical regions and right post-auricular area (largest ∼2 × 2 cm); palpable liver

[Clinical features and symptomatic treatment of liver metastasis in the terminally ill patient].

The incidence of liver metastasis is quite frequent in patients with advanced cancer. Some patients are asintomatic, but more often a correlation can be present between the clinical observation and the anatomic and functional alteration of the liver provoked by metastasis. Hepatomegaly may cause pain, dyspnea, hiccup. Biliary obstruction generates jaundice and itching. Portal hypertension may cause ascitis, encephalopathy, varices of the esophagus. Hepatic failure may produce symptoms like sopor, dysrasic oedema, coagulation problems, jaundice. The treatment of the symptoms listed above is cru

Intra and extra-hepatic cystadenoma of the biliary duct. Review of literature and radiological and pathological characteristics of a very rare case.

Painless obstructive jaundice is often associated with a malignant disease of the common bile duct or head of the pancreas. The authors present a unique case of a 62-year-old woman affected by an intrahepatic cystadenoma that extended into the common biliary duct. To our knowledge no previous case reports have been published on similar cases. After undergoing an en-block hepatic and bile duct resection, this patient is doing well without signs of recurrent disease.

[Adenocarcinoma of the Vater's ampulla associated with Von Recklinghausen's neurofibromatosis].

Von Recklinghausen's Neurofibromatosis is an autosomal dominant disease of variable manifestations. Association with gastrointestinal and hepatobiliary tumours is well documented, most of them being benign neurofibromas. Other tumours, such as leiomyomas, sarcomas, ganglioneuromas and carcinoids are less frequent. Much rarer is the development of epithelial malignancies and an ontogenic relation is not clear. The authors reports a case of adenocarcinoma of Vater's Ampulla and duodenal neurofibromas, in a patient with Von Recklinghausen's disease, presenting obstructive jaundice.

Work in progress: hypotension and high-volume biliary excretion following external percutaneous transhepatic biliary drainage.

A review of 120 consecutive percutaneous transhepatic biliary drainage (PTBD) procedures performed for high-grade obstructive jaundice identified seven patients whose bile output exceeded normal volumes. Three patients required intense fluid therapy for intravascular volume depletion. No patient exhibited fever or bacteremia. Hypovolemia due to high-volume biliary drainage was considered responsible for severe hypotension. Hypovolemia secondary to large-volume biliary secretion may complicate PTBD. The pathophysiology of high-volume biliary drainage unrelated to initial decompression is uncert

[Annular pancreas. Report of a case and review of the literature].

The authors present a case of newborn pre-term with birthweight of 2,250 g. There was polyhydramnios. On the first day she developed jaundice suggested upper intestinal obstruction. A plain radiograph of the abdomen confirmed the previous finding of obstruction of the duodenum, which showed a "double bubble" image with no other gas shadow in the rest of the abdomen. Contrast rodiologic examination of the abdomen confirmed total obstruction of the duodenum second portion. On the 2nd day after birth, laparotomy revealed an annular pancreas. The reconstruction was made by a side-to-side proximal

Hematologic and biochemical abnormalities associated with induced extrahepatic bile duct obstruction in the cat.

Bile duct obstruction was induced in 6 cats by surgical ligation and transection of the common bile duct. Clinical and laboratory changes were monitored weekly for 25 to 54 days. Clinical signs of obstruction were similar in all cats and included anorexia, pyrexia, lethargy, intermittent vomiting, weight loss, palpable gallbladder, hepatomegaly, and bleeding tendencies. Tissue jaundice and acholic feces were evident grossly as early as postsurgical day (PSD) 4 with a mean onset of jaundice at PSD 5.3 +/- 0.4. Hematologic changes were initially characterized by a mild neutrophilic leukocytosis

Endoscopic ultrasound-guided bridging technique for treatment of biliary-enteric anastomotic stenosis after pancreaticoduodenectomy.

Biliary-enteric anastomotic stenosis is one of the main long-term complications after pancreaticoduodenectomy, with an incidence of 2%-8%. Although the relevant reports and studies are relatively few, the consequences such as biliary obstruction and refractory cholangitis seriously affect the quality of life of patients. In this case, the patient is not willing to receive conventional surgery again. This paper provides a bridge technique of EUS-guided Biliary Drainage (EUS-BD) to treat biliary-enteric anastomotic stenosis and solve the problem of obstructive jaundice in the patient.

Biliothorax in a patient with unresectable cholangiocarcinoma.

Biliothorax is a rare but serious condition, as the presence of bile is damaging and can lead to empyema. Here, we report a case of a 51-year-old man recently diagnosed with unresectable cholangiocarcinoma, admitted to the hospital for malignant obstructive jaundice. After interventional management of biliary obstruction, the patient developed a significant right pleural effusion compatible with biliothorax, successfully managed with pleural drainage and antibiotic therapy. Resolution was possible with a conservative approach: biliary decompression, chest tube drainage and antibiotics.
