Medial Patellofemoral Complex Reconstruction (Combined Reconstruction of Medial Patellofemoral Ligament and Medial Quadriceps Tendon-Femoral Ligament) With Semitendinosus Autograft Resulted in Similar Clinical and Radiographic Outcomes to Medial Patellofemoral Ligament Reconstruction in Treating Recurrent Patellar Dislocation.

PURPOSE: To compare clinical and radiographic outcomes of medial patellofemoral ligament reconstruction (MPFL-R) and medial patellofemoral complex reconstruction (MPFC-R) for recurrent patellar dislocation. Outcome measures were compared based on the Insall-Salvati index. METHODS: Patients who were diagnosed with recurrent patellar dislocation and underwent either MPFL-R or MPFC-R (combined reconstruction of MPFL and medial quadriceps tendon-femoral ligament) were retrospectively analyzed. Group allocation was based on surgical procedure and patient characteristics were collected. Clinical as

Lateral osteoarthritis progression is associated with a postoperative residual tibiofemoral subluxation in Oxford UKA.

PURPOSE: Lateral compartment osteoarthritis progression (LOP) is a major complication after Oxford mobile-bearing unicompartmental knee arthroplasty (OUKA). This study aimed to identify the association between tibiofemoral subluxation (TFS) and LOP after OUKA. Patients whose TFS was uncorrectable according to preoperative stress radiographs were hypothesised to develop residual TFS even after surgery, and thought to be more likely to develop LOP. METHODS: The study included 201 patients who underwent medial OUKA. Fifteen patients showed increases in LOP of at least two Kellgren-Lawrence grade

Prognostic factors in patients who underwent surgery for common peroneal nerve injury: a nest case-control study.

BACKGROUND: Common peroneal nerve (CPN) injury is one of the most common nerve injuries in the lower extremities and the motor functional recovery of injured common peroneal nerve (CPN) was often unsatisfactory, the mechanism of which is still controversial. The purpose of this retrospective study was to determine the prognostic factors in patients who underwent surgery for CPN injury and provide a tool for clinicians to assess the patients' prognosis. METHODS: This is a retrospective cohort study of all patients who underwent neural exploration for injured CPN from 2009 to 2019. A total of 3

Transient lateral patellar dislocation: review of imaging findings, patellofemoral anatomy, and treatment options.

Transient patellar dislocation is a common sports-related injury in young adults. Although patients often present to the emergency department with acute knee pain and hemarthrosis, spontaneous reduction frequently occurs, and half of cases are unsuspected clinically. Characteristic magnetic resonance imaging (MRI) findings often lead to the diagnosis. The purpose of this review is to illustrate the MRI findings of lateral patellar dislocation and concomitant injuries, such as kissing contusions of the medial patella and lateral femoral condyle; osteochondral and avulsion fractures; and injurie

Patellofemoral Instability: Evaluation and Management.

Patellofemoral disorders are a common cause of knee pain and disability. A thorough history and a careful physical examination are essential to accurate diagnosis, and imaging modalities play an important role. Magnetic resonance imaging can provide information on malalignment and soft-tissue injuries. Although there is a continuum of diagnoses, most patellofemoral disorders can be divided into three distinct categories: soft-tissue abnormalities, patellar instability due to subluxation and dislocation, and patellofemoral arthritis. Many patellofemoral disorders respond to nonoperative therapy

Editorial Commentary: From Lateral Release and Medial Plication to Complex Medial Patellofemoral Ligament Reconstruction Techniques: Medial Quadriceps Tendon Femoral Ligament Reconstruction Is the Most Anatomic Repair.

Patella instability and dislocation are common in younger patients, and 1 in 5 patients are at risk of recurrent dislocations. Conservative treatment should be considered for first dislocations unless other concomitant injuries are present. Historically, lateral patella release and medial plication techniques were used for repair but have been superseded by medial patellofemoral ligament reconstruction. Overconstraint is a potential problem and often related to nonanatomic femoral tunnel position and graft tension, which could result in increased patellar contact pressures and graft failure. T

Traumatic Dislocation of the Proximal Tibiofibular Joint: A Systematic Review and 10-year Experience From a Level 1 Trauma Center.

INTRODUCTION: Traumatic proximal tibiofibular joint dislocations occur infrequently and are typically the result of high-energy trauma. These injuries can be a marker of limb injury severity because patients often sustain vascular injury and are at high risk of amputation. The purpose of this study was to present a systematic review of traumatic proximal tibiofibular joint dislocations and compare rates of associated injuries with a retrospective series of patients at a level 1 trauma center. The secondary objective was to report rates and clinical predictors of limb amputation. METHODS: A sy

Computer simulation of glenohumeral and patellofemoral subluxation. Estimating pathological articular contact.

Analytic stereophotogrammetry and an interactive computer graphics program were used to obtain first order assessments of joint contact patterns in patellofemoral and glenohumeral joints, simulating normal and abnormal articulations. Precise (90 microns accuracy) computer graphic representations of the humeral head, glenoid, patella, and femoral articular surfaces were obtained from cadaver knees and shoulders. These surface representations were then manipulated into an articulated position, and joint contact areas computed by a proximity criterion. Pathologic states were then simulated, and c

Patellar instability: combined treatment with Goldthwait technique and arthroscopic lateral release.

Patellar instability is a frequent cause of knee pain in childhood and adolescence. Treatment is usually conservative especially after a single episode of dislocation, although in cases of recurrence the surgical option needs to be evaluated. The techniques proposed involve only soft tissues in order to preserve the skeletal maturation. This study evaluates the long-term results on 20 knees in 19 adolescents affected by patellar instability treated with the distal realignment procedure involving medial transfer of the lateral hemi-patellar tendon according to Goldthwait technique, preceded by

Femoral anteversion angle is more advantageous than TT-TG distance in evaluating patellar dislocation: A retrospective cohort study.

PURPOSE: The purpose of this study is to report the parameter characteristics of the femoral anteversion angle (FAA) and tibial tubercle-trochlear groove (TT-TG) distance in patients with patellar instability compared to healthy individuals and to evaluate their reliability in predicting patellar dislocation, providing potential indications for osteotomy. METHODS: A retrospective collection of consecutive patients with patellar instability constituted the study group, while individuals without patellofemoral disorder served as the control group. Measurement of the FAA and TT-TG distance were

MPFL Reconstruction Combined with a Modified Elmslie-Trillat Procedure for Recurrent Patellofemoral Instability.

Several combined procedures have been reported for treating recurrent patellofemoral instability (RPI) with various types and severity of morphological abnormalities, but none have identified absolute threshold values as indications for surgery. We performed medial patellofemoral ligament (MPFL) reconstruction combined with a modified Elmslie-Trillat (ET) procedure on 24 knees (10 male and 11 female patients) to treat RPI with morphological abnormalities corresponding to elevated tibial tubercle-trochlear groove (TT-TG) distance, significant patella alta, and trochlear dysplasia. The inclusion

Short-/intermediate-term outcomes after medial patellofemoral ligament reconstruction in the treatment of chronic lateral patellofemoral instability.

PURPOSE: Disruption of the medial patellofemoral ligament (MPFL) is now considered the essential lesion of recurrent lateral patellar dislocation in patients with normal lower extremity alignment. Reconstruction of the MPFL is a technique gaining significant success in the treatment of patients with this disabling condition. HYPOTHESIS: Reconstruction of the MPFL in patients with chronic patellar instability and normal lower extremity alignment will improve knee function and symptoms, with a high percentage of patients achieving good to excellent results at early follow-up. Dynamic posterior shift test. An adjuvant in evaluation of posterior tibial subluxation.

The dynamic posterior shift test, a clinical method for evaluating both straight posterior instability and posterolateral rotatory instability, (PLRI), is a simple, dynamic, and reliable (reproducible) test that serves as an adjuvant to other clinical tests designed to evaluate an injury or insufficiency of posterior structures in the knee. The examiner maintains the hip at near 90 degrees of flexion to control rotation of the femur while slowly extending the knee passively. The hamstrings should be stretched to maintain their tightness. The tightened hamstrings assist gravity in subluxating t

Treatment of Osteochondral Fracture of the Lateral Femoral Condyle with TWINFIX Ti Suture Anchor "X"-Shaped Internal Fixation under Arthroscopy: A Surgical Technique and Three Cases Report.

Osteochondral fracture of the lateral femoral condyle is a rare intra-articular injury with or without patellar dislocation. This kind of disease is commonly seen in the knee joint sprain during strenuous activity. At present, open reduction is often used to treat osteochondral fractures. In recent years, with the development of arthroscopy, we have been able to complete the reduction and internal fixation of fractures under arthroscopy. In this paper, three cases of osteochondral fracture of lateral femoral condyle were treated with arthroscopic TWINFIX Ti suture anchor internal fixation, and
