[Systemic disease closely related to Cogan's syndrome (author's transl)].

In a 51-year-old patient with non-syphilitic interstitial keratitis and disturbances of inner ear function (Cogan's syndrome) the course of the disease could be followed for 8 years. Organ manifestations which indicated generalised vascular disease were present in accordance with reports in the literature of this rare syndrome. In addition to multiple digital arterial occlusions, recurrent pulmonary infiltrations were observed which have not previously been described in connection with this syndrome. With the exception of the inner ear hearing loss the symptoms could be improved by systemic ad

The Gut Microbiome May Play a Role in the Pathogenesis of Meniere's Disease.

Meniere's disease (MD) was first described 650 years ago. It is now considered to be a multifactorial disorder involving immunological mechanisms, blood-labyrinth barrier breakdown, endolymphatic hydrops, vascular compromise, and genetic susceptibility. Chronic inflammation from both innate and adaptive immunity is evident in the inner ear, with autoimmunity and allergy possibly playing a role. Despite its long history, significant knowledge gaps in its pathogenesis remain. For example, there may be root causes from elsewhere that are contributing to these pathological processes occurring in t

Meniere's disease: Pathogenesis, treatments, and emerging approaches for an idiopathic bioenvironmental disorder.

Meniere's disease (MD) is a severe inner ear condition known by debilitating symptoms, including spontaneous vertigo, fluctuating and progressive hearing loss, tinnitus, and aural fullness or pressure within the affected ear. Prosper Meniere first described the origins of MD in the 1860s, but its underlying mechanisms remain largely elusive today. Nevertheless, researchers have identified a key histopathological feature called Endolymphatic Hydrops (ELH), which refers to the excessive buildup of endolymph fluid in the membranous labyrinth of the inner ear. The exact root of ELH is not fully un

Control of Disabling Vertigo in Ménière's Disease Following Cochlear Implantation without Labyrinthectomy.

BACKGROUND: The placement of a cochlear implant (CI) can restore auditory function in the case of profound cochlear deafness, which may be due to Ménière's disease (MD) or be associated with symptoms related to endolymphatic hydrops. The usual treatment of disabling vertigo in MD is based on vestibular deafferentation by labyrinth ablation. The aim of the present study was to retrospectively evaluate the efficacy of the CI in the control of disabling vestibular manifestations in the case of MD unresponsive to medical treatments. METHODS: A case series of five MD patients with disabling vestib

Vestibular pathology presenting as behavioral problems in a child with Down Syndrome: a case report.

Here we present a case of superior semicircular canal dehiscence (SSCD) in a non-verbal 17-year-old male patient with trisomy 21 that presented with behavioral problems, instead of the more typical symptoms of episodic noise-induced vertigo. SSCD syndrome is characterized by sound-, or pressure-induced vertigo. Patients often complain of vertigo or oscillopsia evoked by loud noises, which may be either external sounds or those elicited by the patients themselves when they are speaking. In cases of SSCD, auditory testing will typically reveal a conductive hearing loss on the affected side while

Time course of inner ear degeneration and deafness in mice lacking the Kir4.1 potassium channel subunit.

The Kir4.1 gene (KCNJ10) encodes an inwardly rectifying K(+) channel subunit abundantly expressed in the CNS. Its expression in the mammalian inner ear has been suggested but its function in vivo in the inner ear is unknown. Because diverse human hereditary deafness syndromes are associated with mutations in K(+) channels, we examined auditory function and inner ear structure in mice with a genetically inactivated Kir4.1 K(+) channel subunit. Startle response experiments suggest that Kir4.1-/- mice are profoundly deaf, whereas Kir4.1+/- mice react like wild-type mice to acoustic stimuli. In Ki

Vertigo due to posterior circulation stroke.

Stroke in the distribution of the posterior circulation may present as acute onset spontaneous vertigo and imbalance. Although vertigo due to posterior circulation stroke is usually associated with other neurologic symptoms or signs, small infarcts in the cerebellum or brainstem can present with vertigo without other localizing symptoms. Approximately 17% of patients with isolated posterior inferior cerebellar artery territory infarction presented with isolated vertigo, nystagmus, and postural unsteadiness. A head impulse test can differentiate acute isolated vertigo associated with cerebellar

[Malfunction of the inner ear is the most frequent cause of dizziness].

Half of all dizziness patients suffer from a malfunction of the inner ear and benign paroxysmal positional vertigo causes 25-30% of these malfunctions. During the latest two decades new vestibular test equipment has been developed and has made it possible to refine the vestibular diagnoses and consequently treat patients more efficiently. This brief overview describes the benefit of Epley's Omniax Chair, video head impulse tests and the Vestibular Autorotation Test. The need for vestibular rehabilitation programmes and for a closer collaboration between physicians and the social authorities is

Electrocochleography in an experimental animal model of acute endolymphatic hydrops.

A reliable animal model of acute type of endolymphatic hydrops was made with injection of artificial endolymph into the cochlear duct. In 12 animals without Reissner's membrane rupture, the endolymphatic potentials (EP) were kept normal after injection of artificial endolymph, but their electrocochleography (ECoG) showed as: a rise of summating potentials (SP) amplitude, a decrease of action potentials (AP) amplitude, an increase of SP/AP ratio, as well as a delay of N1 latency. In 7 animals with membrane rupture, the EP significantly decreased. ECoG showed as: a decrease of hearing function i

Measurement of action potential thresholds in experimental endolymphatic hydrops.

Unilateral experimental endolymphatic hydrops was created by endolymphatic sac and duct obstruction in guinea pigs. Studies of action potential (AP) threshold were then performed and compared with histologic findings. Action potential thresholds were significantly increased in the hydrops ears. The close relationship between AP changes and hydrops was sufficient to allow the detection of hydrops by AP threshold measurements alone. The AP threshold changes were positively related to the degree of hydrops and postoperative interval in a manner consistent with hearing changes seen in Meniere's di

Case report: Endolymphatic system disease in elasmobranchs: clinical presentation, diagnosis, and treatment strategies.

The inner ear is an often overlooked system in elasmobranchs with few documented reports of disease or other abnormalities in the literature. Similar to terrestrial vertebrates, it is located in the cranium, and there are multiple components to the ear of elasmobranchs including a pair of membranous labyrinths each with three semicircular canals and four chambers or end organs (the saccule, the lagena, the utricle and the macula neglecta) making up the endolymphatic system (ELS). There is species variability among the inner ear anatomy of elasmobranchs, and this may play a role in disease deve

Superior semicircular canal dehiscence simulating otosclerosis.

This is a report of a patient with an air-bone gap, thought 10 years ago to be a conductive hearing loss due to otosclerosis and treated with a stapedectomy. It now transpires that the patient actually had a conductive hearing gain due to superior semicircular canal dehiscence. In retrospect for as long as he could remember the patient had experienced cochlear hypersensitivity to bone-conducted sounds so that he could hear his own heart beat and joints move, as well as a tuning fork placed at his ankle. He also had vestibular hypersensitivity to air-conducted sounds with sound-induced eye move

[Peripheral vertigo classification. Consensus document. Otoneurology committee of the Spanish otorhinolaryngology society (2003-2006)].

There are many different vertigo classifications and different denominations are frequently used for the same clinical processes. The Otoneurology Committee of the Spanish Society for Otorhinolaryngology and Head and Neck Pathology proposes an eminently practical classification of peripheral vertigo to facilitate a common terminology that can be easily used by the general ENT practitioners. The methodology used has been by consensus within our Society and especially among the most outstanding work groups in the area of otoneurology in Spain. Initially vertigo is divided into single-episode ver

[Effect of steroid hormone on experimental viral labyrinthitis in guinea pigs].

Clinical and experimental reports confirm that steroid treatment enhances viral pathogenicity in acute infections in vivo. Despite this, steroids are commonly used for the treatment of sudden deafness even though viral infection is thought to be one of the etiological factors involved in the disease. In order to investigate the effect of steroid hormones on viral labyrinthitis, hydrocortisone was administered intramuscularly to 45 guinea pigs before or after Sendai virus (HVJ) was inoculated into their cochleas. Tissue samples were later examined electron-microscopically. Virus-induced cytopat
