We describe the application of a life-trajectory model of suicide to the prison setting and its implication for suicide risk detection. A model has been developed that describes two distinct trajectories culminating in suicide: one with large amounts of adversity early in life with a young age of suicide and another with chronic, gradually accumulating adversity with a later age of suicide. Support for applying the life-trajectory model to the prison population is found in prison-centric models of suicidal behavior and clinical profiles of individuals at high risk of suicide in prisons. We als

Many systemic treatments used in genitourinary oncology negatively affect haematopoiesis, thus leading to neutropenia. Neutropenic patients are vulnerable to bacterial, and other infections. Often fever is the only symptom in these patients. Neutropenic fever is a major threat for these patients, as it may lead to life-threatening therapy complications that significantly impair the patient's quality of life, Moreover, it may also worsen the prognosis due to therapy delays or necessary dose modifications. Granulocyte colony stimulating factors (GCSF), which can improve neutrophil granulocyte fo

Myocarditis is an inflammatory condition of the myocardium and is usually categorised as acute nonfulminant and acute fulminant myocarditis. Myocardial injury can result via viral infections, direct injury or immune responses. Fulminant myocarditis can be characterised by severe and sudden cardiac inflammation that may result from cardiogenic shock, ventricular arrhythmias or multi-organ system failure. Extracorporeal membrane oxygenation (ECMO), also known as extracorporeal life support, is an effective technique for patients with fulminant myocarditis, providing heart and lung support and ad

Immunotherapy for cancer has undergone a rapid expansion in classes, agents, and indications. By utilizing aspects of the body's innate immune system, immunotherapy has improved life expectancy and quality of life for patients with several types of cancer. Adoptive cellular therapies, including chimeric antigen receptor T (CAR T) cell therapy, involve the genetic engineering of patient T cells to allow for targeting of neoplastic cells. Monitoring of patients during the lymphodepletion prior to therapy and following CAR T cell infusion is necessary to detect toxicity of therapy. Specific toxic

Loneliness is a major psychological challenge in older adulthood, contributing to increased risks of depression, anxiety, and mortality. Conversational agents - technologies that interact with users via natural language - have emerged as potential tools to support psychological well-being in later life. This systematic review and meta-analysis evaluated the effects of autonomous conversational agents, including robotic and nonrobotic systems, on loneliness, as well as social isolation, depression, and anxiety in older people without cognitive impairment. Seventeen studies with pre-post interve

Widowhood represents a critical life transition with profound implications for mental health, yet causal evidence from low and middle income countries (LMICs) remains scarce. This gap is particularly acute in contexts like India, where population ageing is accelerating rapidly, and traditional family support systems are evolving. The causal effect of spousal loss on depression is estimated using panel data from India's Longitudinal Ageing Study (LASI) and doubly robust difference-in-differences (DRDID) estimation. Depression trajectories were compared between 343 older adults who experienced w

Chronic non-communicable diseases account for the majority of global mortality and contribute substantially to long-term disability and healthcare expenditure worldwide, necessitating sustained, multidimensional management strategies beyond symptomatic control. Ayurveda, a traditional system of medicine based on holistic and individualized care, has gained increasing attention as a complementary approach for chronic disease management. This review synthesizes clinical evidence published between 2015 and 2025 to evaluate the role of Ayurvedic interventions across major chronic disease categorie

BACKGROUND: Understanding how multimorbidity affects activities of daily living (ADL) and instrumental activities of daily living (IADL) disability in older Irish adults is essential for planning responsive health and social care systems that can support quality of daily life. This study examines how chronic noncommunicable diseases multimorbidity affects older adults' ability to live independently in Ireland. It focuses on the relationship between multimorbidity and limitations in ADL and IADL.
METHODS: A cross-sectional 2019 Irish Health Survey data set was used. The study analyzed data fro

BACKGROUND: Patient organizations play a vital role in supporting individuals with chronic disorders by offering a sense of community and shared understanding to improve patient outcomes. However, systematically evaluating the long-term impact of such organizational activities remains a challenge. Ripple effect mapping (REM), a participatory evaluation tool, offers a structured approach to visually map patient organizations' intended and unintended outcomes.
METHODS: This study aimed to understand the impact of a Patient Organization for Hemophiliacs (POH) on people with hemophilia (PwH), who

OBJECTIVE: How do people perceive emotionally significant life events as impacting relapse risk? Investigating perceived risk is important for at least two reasons. First, perceived risk of relapse is associated with actual risk; individuals are insightful about their own recovery. Second, perceptions, whether accurate or not, can influence the behavior of individuals in recovery and those who want to provide support.
METHOD: We measured relapse risk perception using 16 vignettes that depicted emotionally significant events (4 positive-social, 4 negative-social, 4 positive-nonsocial, and 4 ne

INTRODUCTION: Familial chylomicronemia syndrome (FCS) is an autosomal recessive disorder that affects approximately 1 to 10 individuals per million and is caused by variants in the genes encoding for the lipoprotein lipase (LPL) enzyme. In addition to its heterogeneous clinical presentation, FCS is characterized by a higher risk of life-threatening, recurrent acute pancreatitis and type 3 diabetes. Since available evidence on FCS in Latin America is limited, there is a clear need for a consensus document that provides relevant recommendations to guide the management of suspected cases and opti

Background/Objectives: Facial palsy causes both functional and aesthetic impairments, with asymmetry significantly affecting quality of life. Botulinum toxin injections are increasingly used to restore facial balance by reducing contralateral hyperactivity, but outcome assessments remain largely subjective. The objective of this study was to evaluate the ability of three-dimensional (3D) stereophotogrammetry coupled with root mean square (RMS) surface analysis to objectively quantify improvements in facial symmetry following botulinum toxin treatment and to support clinicians in patient care.

Mechanical ventilation can cause acute diaphragm atrophy and injury, and this is associated with poor clinical outcomes. Although the importance and impact of lung-protective ventilation is widely appreciated and well established, the concept of diaphragm-protective ventilation has recently emerged as a potential complementary therapeutic strategy. This Perspective, developed from discussions at a meeting of international experts convened by PLUG (the Pleural Pressure Working Group) of the European Society of Intensive Care Medicine, outlines a conceptual framework for an integrated lung- and

Middle-aged and older adults with HIV experience double discrimination and cognitive impairment due to both their HIV status and their age. However, the relationship between perceived discrimination and self-reported cognitive ability in middle-aged and older people living with HIV (PLWH) is less clear. We measured self-reported perceived discrimination and cognitive ability using the Expanded Everyday Discrimination Scale and the subscale of the AIDS Health Assessment Questionnaire (AIDS-HAQ). The study sample included 324 middle-aged and older PLWH (over 45 years old) from five designated HI

BACKGROUND: Each year an estimated 2.3 million newborns die in the first 28 days of life. Most of these deaths are preventable, and high-quality neonatal care is fundamental for surviving and thriving. Service readiness is used to assess the capacity of hospitals to provide care, but current health facility assessment (HFA) tools do not fully evaluate inpatient small and sick newborn care (SSNC).
METHODS: Health systems ingredients for SSNC were identified from international guidelines, notably World Health Organization (WHO), and other standards for SSNC. Existing global and national service

BACKGROUND: Medical decision support systems (CDSSs) are increasingly used in medicine, but their utility in daily medical practice is difficult to evaluate. One variant of CDSS is a generator of differential diagnoses (DDx generator). We performed a feasibility study on three different, publicly available data sets of medical cases in order to identify the frequency in which two different DDx generators provide helpful information (either by providing a list of differential diagnosis or recognizing the expert diagnosis if available) for a given case report.
METHODS: Used data sets were n = 1

Severe right ventricular (RV) failure after left ventricular assist device (LVAD) implantation is a serious complication with a poor prognosis. The most effective therapy for these patients is an upgrade to biventricular mechanical support. However, it is well recognized that primary and secondary biventricular ventricular assist device implantation is associated with higher mortality rates. We report on three patients with cardiogenic shock, who were provided on an emergency basis with a percutaneous extracorporeal life support (ECLS) system by the femoral vessels. After stabilization, a LVAD
